    Notizen: The Prognostic significance of lymphvascular invasion (LVI) in stage: IB cervix cancer remains controversial in spite of several recent reports in this literature. Only LVI showed a significant correlation (P = 0.0002) with local failure. Thirty-two patients with squamous cell carcinoma and LVI were identified. Seventeen of these patients received postoperative radiotherapy and 15 patients received no adjuvant therapy. Both groups were traced for outcome. Four of the 15 patients (27%) with LVI treated with surgery alone developed local recurrence, compared to 0/17 patients who received postoperative radiotherapy (P = 0.038). The 5-year and 7-year survival of the entire group was excellent, 97.2% and 94.9% respectively. There was a trend towards significance (P = 0.07) in survival between LVI+ and LVI− patients.Lymphvascular invasion was significantly associated with increased local failure in our analysis. Although prospective randomized trials are in progress to further evaluate this issue there is growing concern about low accrual because of physicians biases regarding this controversial issue. Adjuvant postoperative radiotherapy appears beneficial in improving local control for LVI patients in our study, but its impact on survival is still questionable. We recommend enrollment of patients in a national cooperative trial whenever possible. Treatment of patients off protocol solely for LVI+ is still quite controversial and must be left to the discretion of the clinician.

    Notizen: Soft materials have long been sought after for use in devices such as actuators, artificial muscles, separators, switches, sensors, memories, and so forth. We developed a soft, optically transparent material using polymer gels that can not only be activated by visible light (switched on) but also deactivated (switched off) by altering the local environment using three different means: pH, temperature, and light. This copolymer gel is a covalently cross-linked network of N-isopropylacrylamide, sodium acrylate, and a chromophore, which is found to undergo phase transitions exhibiting large hysteresis in the degree of swelling in response to pH, temperature, and light. In each system, between the transitions for swelling and shrinking, the gel can show either a swollen or a collapsed state, which can be selected according to the history of the variables. It has been established that a thermoresponsive gel with chromophore exhibits a local volume phase transition upon illumination with visible light. By making use of this phenomenon, we have successfully controlled the phase in which a gel exists with visible light: Without light illumination the gel stays in the swollen state. Upon illumination beyond a threshold intensity, however, a volume transition is locally induced, thereby forming a material in which both phases coexist stably for at least several hours after the light source has been removed. The phenomenological stability of the material in the coexistence state is discussed on the basis of the Landau theory. © 1996 American Institute of Physics.

    Notizen: Abstract. We performed intraoperative ultrasonography with a miniature probe to explore the biliary anatomy, especially the cystic duct, during laparoscopic cholecystectomy. By using this radial-type probe introduced into a hard metal sheath with a balloon at the end, the plane containing Calot's triangle can be scanned easily when the gallbladder is extracted to the right side, thereby facilitating the identification of the cystic duct as well as the common ducts. In 30 cases, no common duct stone was found and the cystic duct was clearly identified. This radial-type miniature probe can be used to locate the cystic duct and avoid inadvertant incision or division of the common ducts.

    Notizen: Abstract  Gluteal stepladder V-Y advancement musculocutaneous and fasciocutaneous flaps were used for resurfacing of various shaped sacrogluteal defects. A total of 27 patients with sacrogluteal defects were treated using this technique. Twenty-four patients had sacral pressure sores, 2 patients had a pilonidal sinus, and 1 had a low thermal burn of the gluteal region. The shape of defects varied including 12 elliptic, 5 inverted heart-shaped, 4 pentagon, 4 diamond, and 2 other shapes. There were no problems regarding flap survival even at the tip of step segments. In all patients, easy resurfacing of the defects and satisfactory recontour of the gluteal region was obtained without linear scars crossing the gluteal fissure. Since the segments of the flap are similar to the defect, defects of various shapes are easily covered, and trimming of normal skin tissue becomes unnecessary or minimal. On inserting a triangular flap in the gluteal fissure, excellent recontour of the sacrogluteal region can be achieved.

    Notizen: Abstract We present ab initio calculations of electric field gradients (EFGs) at impurity sites in ionic crystals TiO2, Al2O3 and CaCO3. The electronic structure was calculated self-consistently by the KKR method in the framework of the local spin density approximation of the density functional theory. The system with a single impurity was simulated by the super cell method. It was found that EFGs for the transition metal impurities (Sc, Nb, Cd and Ta) in TiO2 were well reproduced by the calculations if the charge state of them in TiO2 was taken into account. The present method was applied to the determination of the implantation sites of N and O nuclei in TiO2. The calculation of EFGs at a Si impurity in Al2O3 and at Ca site in CaCO3 were used to derive the quadrupole moments of 27Si and 39Ca from their quadrupole coupling constants.

    Notizen: Abstract The nuclear isomer 178Hfm2, expected to be the most promising candidate for the gamma-ray medium, is rather difficult to produce in large quantities. There are a few ways to create this isomer, such as the irradiation of 179Hf with high-energy neutrons through the (n,2n) reaction, the irradiation of 176Yb with high-energy α particles through the (α,2n) reaction, the irradiation of 181Ta with protons through the (p,2p2n) reaction. In some of these reaction schemes the isotopically pure target works better than the natural one from the viewpoint of spectroscopic purity, handling of radioactive materials and productivity. However, isotope separation of heavy elements for producing a precursor as a target material is difficult in terms of cost/effectiveness. The atomic vapor laser isotope separation (AVLIS) method is expected as the most efficient way compared with the normal electromagnetic separation method.
