    Notes: Summary We compare radiosonde observations of relative humidity with NWP versions of the Meteorological Office Unified Model, and attempt to understand the causes of the systematic differences seen. The differences are found to have a different structure in cyclonic and anticyclonic situations over the UK. In cyclonic situations the mid-tropospheric temperature and humidity differences could be due to model biases, consistent with the conservation of energy; the latent heating from precipitation of the model's excess moisture would remove the model's cold bias. There is also some evidence for observational bias. Wetting of the sonde sensor in cloud can cause a moist bias at higher levels. The Väisala RS80 sonde also appears to have a dry bias near saturation. The Unified Model has a parameterisation for stratiform cloud which calculates the fractional cloud cover in a gridbox from the box-average relative humidity, allowing for sub-grid-scale variability within the box. This scheme has been tuned to give reasonable cloud amounts with the model's relative humidities. The cloud amounts implied (by the scheme) for radiosonde relative humidities are systematically less than the observed cloud. So assimilation of the observed humidities can significantly degrade analyses and predictions of cloud. Bias corrections for the radiosonde humidities have been calculated to compensate for this. Experiments have been performed to test the effect of the bias correction on the assimilation and prediction of cloud and precipation. With the control system, cloud cover and precipitation spins-up during the forecast period; the bias correction improves this. A large improvement was also found when the relationship between the temperature and humidity assimilation was changed; it is better to assume that temperature and relative humidity errors are uncorrelated, rather than temperature and specific humidity.

    Notes: Abstract Patient satisfaction can be a useful marker in evaluating the quality of psychiatric care. However, this form of measurement has been hampered by the lack of attention paid to the psychometric properties of instruments devised. The Psychiatric Care Satisfaction Questionnaire (PCSQ) was developed and tests of acceptability, validity and reliability were undertaken using 52 inpatients. Content validity was assessed by surveying psychiatrists, other mental health professionals, MIND workers and patient groups. Thirty-six (67.9%) of all mental health professionals felt that the questionnaire was a useful measure of patients' satisfaction with their care, and many comments also suggested high consensual validity. In terms of content validity, responses from patient groups indicated that the majority (63.6%) felt it to be a useful measure of patient satisfaction and 70.5% felt all the areas covered were important. Concurrent and criterion validity were also good, with high correlations with existing scales and with those on a section being significantly less satisfied on the PCSQ (F = 13.3, P = 0.0004). Test-retest reliability was adequate (Cohen's κ 0.48–0.80) and the internal consistency of the PCSQ was good, at 0.82 (Cronbach's α). The PCSQ performed well in all aspects of validity and reliability, indicating that it has good psychometric properties and can be a useful tool for measuring patient satisfaction. However, there remains the need to analyse more closely the concept of satisfaction and its determinants.

    Notes: Abstract We examined whether psychiatric patients knew their diagnosis, the significance they attached to it, and the impact of being informed in a systematic fashion according to their wishes. We also assessed whether the nature of the psychiatric diagnosis influenced what patients were told by their psychiatrists. The three parts of the study included questionnaire responses from 28 consultant psychiatrists; case-note reviews and questionnaire responses of 200 adult psychiatric in-patients; and a pilot study informing 28 adult psychiatric day hospital patients of their diagnosis according to their wishes. The results showed that of 126 in-patients, 53% had not been told their diagnosis, although most wanted to know. Of those informed, 75% agreed with their diagnosis. The majority of patients considered a psychiatric diagnosis to be as real as a physical diagnosis and helpful in their treatment. Patients with schizophrenia were less likely to have been informed of their diagnosis, and psychiatrists were also more reticent regarding the diagnosis of personality disorder. All patients who were systematically informed, in the pilot study, agreed with their diagnosis. We conclude that most patients agree with their diagnosis and its usefulness. All patients should be asked whether they want to know their diagnosis and be informed appropriately.

    Notes: Summary The incidence of diabetes among people aged 18–50 years has been studied prospectively in nine towns, chosen to encompass the range of socioeconomic conditions and spread of latitude in England and Wales. The incidence of Type 1 (insulindependent) diabetes in this age group varied little between the towns. However, the incidence of Type 2 (non-insulin-dependent) diabetes was markedly higher in the towns with ‘worse’ socio-economic conditions. The mean incidences were 23 per 100000 for the three ‘worse’ towns and 10 per 100000 for the three ‘better’ towns. This was not explicable by an association between the disease and social class. This finding has to be reconciled with known and hypothesised influences in the aetiology of Type 2 diabetes.

    Notes: Abstract Mutations in the basement membrane collagen gene COL4A5 cause the progressive renal glomerular nephropathy and typical hearing loss that occur in X-linked Alport syndrome. Nearly all cases involve distinct mutations, as expected for an X-linked disease that significantly reduces the fitness of affected males. A few exceptional COL4A5 mutations appear to be associated with a reduced disease severity and may account for a significant proportion of late-onset Alport syndrome in populations where a founder effect has occurred. The novel mutation reported here, COL4A5 arg1677gln, has been detected in three independently ascertained Ashkenazi-American families, causes a relatively mild form of nephritis with typical onset in the fourth or fifth decade, and may be involved in the etiology of a large proportion of adult-onset hereditary nephritis in Ashkenazi Jews.

    Notes: Abstract   Diurnal changes in titratable acidity, photosynthesis, energy dissipation activity, and the carotenoid composition of differently oriented cladodes of the cactus Opuntia macrorhiza were characterized during exposure to full sunlight in the field. Four cladode faces were chosen such that each was exposed to maximum photon flux densities (PFD) at different times of the day in addition to receiving different daily integrated PFDs. The sum of all carotenoids per chlorophyll was found to increase with increasing exposure to PFD, with the carotenoids of the xanthophyll cycle present in the most exposed face at more than twice the concentration found in the least exposed face. All faces exhibited large increases in xanthophyll cycle-dependent energy dissipation as the sun rose in the morning, even those receiving only minimal levels of diffuse radiation. The transient high levels of energy dissipation in those faces that did not receive direct sunlight in the morning may have been due to low temperature inhibition of photosynthesis (predawn low of 2°C). For the two faces receiving peak PFDs in the morning hours (north and east faces), the level of energy dissipation activity increased rapidly during exposure to direct sunlight in the early morning, gradually declining in the late morning under warm temperatures, and was negligible during the afternoon low light conditions. Changes in the xanthophyll cycle paralleled the changes in energy dissipation with the majority of the cycle present as violaxanthin (V) prior to sunrise, largely de-epoxidized to zeaxanthin (Z) and antheraxanthin (A) during exposure to direct sunlight, and reconverted to V during the afternoon. For the two faces receiving peak PFDs in the afternoon (south and west faces), energy dissipation activity increased dramatically during the early morning low light period, subsequently decreasing during midday as decarboxylation of malic acid proceeded maximally (providing a high concentration of CO2 for photosynthesis), and then increased to the highest level in the late afternoon as the supply of malic acid was depleted and rates of photosynthetic electron transport declined. The xanthophyll cycle, largely present as Z and A prior to sunrise in the south and west faces, was de-epoxidized to the greatest extent in the late afternoon, followed by epoxidation back to the predawn level by sunset. In all cladode faces high levels of energy dissipation activity were accompanied by decreases in the intrinsic efficiency of photosystem II (PSII), indicative of a regulatory process that diverted the excess energy away from the reaction centers during periods of excess light. Furthermore, the overnight retention of Z and A by the south and west faces was accompanied by a sustained reduction in PSII efficiency (i.e., “photoinhibition”). We suggest that this “photoinhibition” represents the sustained engagement of nocturnally retained Z and A in the photoprotective down-regulation of PSII.

    Notes: Abstract Expression of plant genes involved in the symbiosis between alfalfa (Medicago sativa) and Rhizobium meliloti has been studied by comparing root and root nodule mRNA populations. Two-dimensional gel electrophoretic separation of the in vitro translation products of polyA+ RNA isolated from either roots or effective root nodules has allowed us to identify thirteen nodule-specific translation products, including those corresponding to the leghemoglobins (Lb). These translation products, representing putative nodulin mRNAs, are first detected between 9 and 12 days after inoculation, a result which has been confirmed for Lb mRNA by Northern blotting and hybridization with a Lb cDNA probe. Analysis of three different types of ineffective root nodules arrested in different stages of development has led to the following conclusions. (i) The transcription of eleven nodule-specific genes, including the Lb genes, is independent of nitrogen-fixing activity. (ii) Differentiation of the primary nodule structure does not require the transcription of any of these genes but can be correlated with a dramatic reduction in the level of at least five transcripts present in the root. (iii) There is enhanced expression of certain plant genes in the case of nodules elicited by an Agrobacterium strain carrying the symbiotic plasmid of R. meliloti.

    Notes: Abstract An experimental method is described for measuring the dynamic initiation toughness of a sharp stationary crack. A plane specimen is utilized which consists of a central region 50-mm wide and 200-mm long with integral dog-bone ends. The loading is accomplished by the detonation of four small explosive charges which produce two tensile stress waves upon reflection from the dog-bone ends. The stress waves meet at the midpoint of the specimen and reinforce to produce a relatively large, uniformly stressed region with a very high loading rate. The crack is positioned at the midpoint of the specimen at the location where the reinforcing tensile stress waves meet. A series of photoelastic experiments were conducted using Homalite 100 as the model material to observe, in a full-field view, the arrival of the dilatational waves, the subsequent development of the stress field at the tip of the stationary crack and the initiation of the crack. The isochromatic fringe pattern was also used to determine the instantaneous value of the stress-intensity factorK(t) after the characteristic fringe loops developed in the region near the crack tip. Finally,K(t) was measured using a single strain gage positioned and oriented so that its signal output was proportional toK(t) and independent of the next two higher order terms in the series representation of the strain field. A method was developed to determine the instant of initiation from the strain-time trace. Results obtained from the photoelastic and strain measurements of the dynamic-initiation toughnessK ID were consistently higher than the static value ofK IC .

    Notes: Abstract The mixed-mode, elastodynamic state of stress in the neighborhood of a constant-velocity crack tip is used to generate numerically unsymmetric isochromatics. Unsymmetry associated with the third-order terms of a mixed-mode stress field, with and without the Mode II singular stress term, is also investigated. In extractingK I from an unsymmetric isochromatic pattern, errors in the Mode I fracture parameters due to the assumed presence ofK II in aK I stress field were found to be significant when data are taken more than 4 mm from the crack tip.
