    Notes: Summary Peripheral nerve meningiomas are exceedingly rare neoplasms of controversial origin; only four cases have been reported. Proposed origins of ectopic meningiomas include extradural trapping of arachnoid cells during embryogenesis, ectopic migration of arachnoid cell nests with the developing peripheral nerve, and metaplasia of mature peripheral nerve sheath cells or a common progenitor cell. In this report of a meningioma of the brachial plexus, immunohistochemical and ultrastructural examinations of the tumor matched all the criteria of a traditional meningioma but failed to clarify the origins of such neoplasms.

    Notes: Summary A melanotic ganglioglioma was biopsied in the pineal region of a 12-month-old girl who preoperatively underwent a ventriculo-peritoneal shunt for hydrocephalus and postoperatively received radiotherapy. The tumor was subtotally excised when the girl was 7 years and 4 months of age. Histologically, it demonstrated mature neurons in disorganized clusters and in well-differentiated cerebrum-like tissue, rare binucleated neurons, glia similar to normal gray matter, and bands of fibrous tissue containing heavily pigmented cells. Ultrastructurally, melanosomes of stages I to IV were identified in the pigmented cells. An origin involving retinal differentiation of the primitive pineal gland was not supported; the tumor was negative for both retinal S-antigen (MAbA9-C6) and cellular retinal-binding protein (CRALBP). This report demonstrates the ability of a cerebral neoplasm to contain neurons, glia, and melanin-containing cells; all of which are neuroectodermally derived.

    Notes: Summary Perineurial cell basement membrane (PCBM) thickening is a consistent feature in diabetes mellitus (DM) and may have relevance to the cause of DM neuropathy. In this ultrastructural morphometric study of identical twins discordant for DM, we found that the PCBM was significantly thicker in the dermal nerves of the diabetic twin. Following pancreas transplantation (PT) and a 2-year period of euglycemia, the PCBM in both dermal and sural nerves was significantly thinner. At the end of the 2nd year post-PT, the PCBM thickness in the dermal nerves of the diabetic was not significantly different from the non-DM twin. The correction of diabetic dysmetabolism may have played a role in the regression of PCBM. These data suggest that PCBM thickening may not be a permanent legacy of DM.

    Notes: Summary Localized hypertrophic neuropathy (hypertrophic mononeuropathy) is a rare benign condition that generally occurs in people under 40 years of age. Our immunocytochemical (S-100 protein) study of four new cases confirms previous observations that the cells forming the hypertrophic onion bulb are composed of perineurial cells. These observations and previously published illustrations, reveal a curious hyalinization of the outer perineurium of affected fascicles which suggests the absence of a perineurial barrier. Compartmentation (compartmentalization) of the endoneurium in hypertrophic mononeuropathy closely mimics the transient compartmentation which occurs in the distal nerve stumps of axotomized nerves, particularly in nerves in which re-innervation is prevented. Compartmentalization also can be produced by resection of the perineurial sheath. These findings suggest that hypertrophic mononeuropathy may be a reactive condition due to focal damage to the perineurial barrier.

A comparison is made between brushed and unbrushed tubes in summer, and unbrushed tubes in winter. Detrital deposition and epiphytic growth may decrease surface area and, by uptake and regeneration, influence nutrient flux through the membrane.

    Notes: Abstract Islet autotransplantation offers the potential for preventing the surgically induced diabetes that is an inevitable consequence of total pancreatectomy. This paper describes the first islet autotransplant programme in the United Kingdom and the first series in the world to use the spleen as a site for the islet graft. Over an 11 month period, 7 patients underwent total pancreatectomy for chronic pancreatitis combined with a simultaneous islet autotransplant. All 7 patients had normal glucose-tolerance levels and normal C-peptide levels pre-operatively. In 6 patients, islets were embolized into the liver via the portal vein (median transplanted volume=8.5 ml). In addition, 3 patients received islets into the splenic sinusoids via a short gastric vein (median transplanted volume=4 ml). One patient received islets into the spleen alone. One patient died of a stroke 4 weeks post transplantation. Two patients have achieved insulin independence, with a further two patients achieving ”transient” insulin independence (〈1 month). The remaining 2 patients, although requiring reduced insulin doses, have not achieved insulin-independence. However, all patients have C-peptide levels within the normal range. In trying to explain these findings, split proinsulin levels were measured and found to be elevated. High levels of split proinsulin cross react with the C-peptide assay and this would explain the falsely elevated C-peptide levels. Indeed insulin levels in these patients were all below the normal range. These findings would suggest that the use of C-peptide levels as the ”gold standard” for monitoring islet autograft function, may require reappraisal.

    Notes: Summary 1. Rubredoxin was isolated from the facultative anaerobe Spirochaeta aurantia and from the obligate anaerobe Spirochaeta stenostrepta. The spirochetal rubredoxins contained one atom of iron per molecule and no inorganic sulfide. Their molecular weight (approximately 6000), spectral properties, and amino acid composition resembled those of rubredoxins obtained from anaerobic bacteria. A rubredoxin or a rubredoxin-like protein was present in the host-associated Treponema denticola. Rubredoxin was detected in extracts of both anaerobically- and aerobically-grown cells of S. aurantia. 2. An unstable ferredoxin was isolated from cell extracts of anaerobically-grown S. aurantia, but was not detected in aerobically-grown cells. The spectral characteristics and amino acid composition of this protein were similar to those of other bacterial ferredoxins. Four atoms of iron and four acid-labile sulfide residues were present per molecule. The molecular weight of S. aurantia ferredoxin was near 6000. S. aurantia ferredoxin preparations stimulated the formation of acetyl phosphate from pyruvate by diethylaminoethyl cellulose-treated extracts of Clostridium butyricum, whereas purified spirochetal rubredoxin did not.

    Notes: Abstract  The present study has examined the distribution of axons of differing sizes in the optic pathway of the ground squirrel. Axon diameters were measured from electron micrographs at various locations across sections of the optic nerve and tract, and total distributions and numbers were estimated. In both the nerve and tract, roughly 1.2 million optic axons were present. The population of optic axons had a unimodal size distribution, peaking at 0.9 μm in diameter and having an extended tail toward larger diameters. Local axon diameter distributions in the optic tract indicated distinct (though partially overlapping) axon diameter classes, including one of fine sizes peaking at 0.8–0.9 μm, a second of medium sizes peaking around 1.7–1.8 μm, and a third composed of the larger fibers with diameters up to 4.8 μm. The fine-caliber axons were found at all locations in the tract, and were the only axons present immediately adjacent to the pia, while the medium- and coarse-caliber axons were found at deeper locations. Curiously, the larger axons were found primarily in the medial parts of the tract, where axons from the dorsal retina normally course. A similarly restricted distribution of the larger axons was observed in the dorsotemporal parts of the optic nerve, suggesting that this difference in the tract may relate to an asymmetric distribution of ganglion cells on the retina giving rise to these axons. Measurements of axonal size taken within the optic fiber layer in dorsal and ventral parts of the retina confirmed this asymmetry, consistent with previous demonstrations of soma size differences in the dorsal versus ventral retina. The partial segregation of axons by size in the optic tract of the ground squirrel then reflects both the asymmetric distribution of retinal ganglion cell classes and the chronotopic reordering of optic axons that occurs within the chiasmatic region.
