    Notizen: Objective To establish reference ranges for the levels of alpha-fetoprotein, albumin, prealbumin (transthyretin) alpha-1-antitrypsin, transferrin, ceruloplasmin and total protein in the plasma of normal human fetuses and newborn babies.Design Prospective study of individual normal cases to fulfil objectives.Setting Pathology laboratories of the University of Edinburgh and the biochemistry laboratories of the University of Keele.Subjects Twenty-two normal fetuses 13 to 22 weeks of gestation and 66 babies born between 24 and 41 weeks gestation.Results Albumin is the predominant plasma protein throughout gestation. The levels of alpha-fetoprotein and prealbumin fell significantly with increasing gestation, whereas the concentrations of the other proteins studied increased. The ratios of individual proteins to total protein demonstrated similar trends.Conclusions This study provides developmental profiles of normal human fetal plasma proteins to serve as possible reference data for abnormal fetuses. Declining levels of prealbumin (transthyretin) were unexpected and suggest a functional role for this protein in early pregnancy.

    Notizen: An apparatus utilizing laser-induced fluorescence (LIF) has been developed to measure high-resolution angular distributions of sputtered neutral atoms. LIF provides sensitive detection, a feature necessary to monitor the low atomic fluxes inherent to angularly resolved sputtering measurements in the static sputtering regime. The apparatus incorporates a detector assembly which rotates about the sample in an ultrahigh vacuum (UHV) chamber, allowing a large range of angular measurements at different ion beam incidence angles. Laser light is brought to the detector in the UHV chamber via a single optical fiber and fluorescence photons exit the chamber via a fiber bundle. The optical fiber and fiber bundle are mounted in a fixed orientation on the detector, maintaining constant alignment as the assembly rotates. Angular resolution in the polar plane containing the incident ion beam and the surface normal is better than 3°. Angular resolution in the direction perpendicular to the polar plane is governed by the Doppler shift and is 0.028° for 3.2 eV atoms. Overall detection efficiency is estimated to be 5×10−9 counts per sputtered atom and 2×10−3 counts per atom entering the detection volume. Initial experiments using polycrystalline Zr to characterize the device are described.

    Notizen: Ion beams with energies in the range 1–2 keV are used to sputter neutral Zr atoms from a polycrystalline surface. Laser induced fluorescence detection is used to obtain angular distributions of sputtered neutrals as a function of ion impact direction, ion mass, ion energy, and spin–orbit state of the exiting atoms. About 40% of the sputtered atoms are excited. Angular distributions depend weakly on ion mass and energy. The angular distributions are fitted well by a modified form of the Roosendaal and Saunders model of sputtering.

    Notizen: A technique for fitting mixture distributions to discrete and continuous instrument count data is presented. The use of the EM algorithm for performing maximum likelihood estimation is introduced and advantages over other methods are discussed. Equations are presented for fitting mixtures of Poisson distributions and mixtures of normal distributions. Examples of the fitting of these two types of mixture distribution are given, and it is shown how standard errors of the parameter estimates can be obtained from within the framework of the fitting process.

    Notizen: We describe a plasma-based dry etching procedure which permits selective etching of Si over Ge with a Si/Ge etch rate ratio of over 70 and negligible etching of the Ge underlayer. This is achieved in a SF6/H2/CF4 gas mixture by the formation of a thin ((approximately-equal-to)3 nm) involatile etch stop layer on the Ge surface which consists of Ge-sulfide and carbonaceous material.

    Notizen: This study investigated the temperature of healthy oral mucosa and underlying bone. Using a fine thermocouple and digital thermometer, four groups of temperatures were measured: (i) adjacent to uncrupted third molars before and after surgical removal (n= 51); (ii) at the same site in patients not undergoing surgery (n= 30); and (iii) at the buccal incisor mucosa (n= 30). In each group sublingual temperatures were measured in order to calculate the temperature differential between measurement site and sublingual temperature to compensate for variations between sites and individuals. In the final group (iv) (n= 10) sublingual temperature was recorded in a closed mouth.Mean alveolar bone temperature was 5°C cooler than overlying mucosa, and mean post-operative mucosal temperature was some 2.5°C cooler than before operation (t= 2.32, P 〈 0.001). Sublingual temperature, in a closed mouth, remained relatively constant. In an open mouth, in both anterior and posterior regions, there were statistically significant decreases in sublingual temperature and in mucosal temperature differentials. Significant differences were also found between temperature differentials calculated using sublingual temperature measured at baseline and after 10 min. This indicates that sublingual temperature should be measured just prior to measuring a mucosal site.

    Notizen: At the 1990 World Congresses of Gastroenterology, the Working Party on Helicobacter pylori (H. pylori) recommended that, in suitable patients, the bacterium should be eradicated using a therapeutic regimen comprising a bismuth salt, tetracycline and metronidazole for two weeks. We have treated 40 patients infected with H. pylori with‘triple’ therapy consisting of 120 mg tripotassium dicitrato bismuthate q.d.s., 500 mg tetracycline q.d.s. and 400 mg metronidazole t.d.s. for two weeks.The success rate, in terms of bacterial eradication, was 19/21 (90.5%) in patients with metronidazole-sensitive organisms, compared with only 6/19 (31.6%) in patients whose H. pylori were resistant to metronidazole (P 〈 0.01). Side effects, particularly diarrhoea and vomiting/nausea, were common: 23/40 patients reported such symptoms during the 14-day course of therapy. Fifteen of these 23 patients completed the entire 14-day course, although suffering from significant side effects, while the remaining eight patients had to discontinue the treatment because side effects became intolerable.If a form of triple therapy is going to be widely used to eradicate H. pylori infection, the regimen will have to be simpler, shorter, produce fewer side effects and be more effective in patients with metronidazole-resistant bacteria.

    Notizen: We have performed a retrospective study of 103 patients with either peptic ulcer or non-ulcer dyspepsia, infected with metronidazole-sensitive strains of Helicobacter pylori (H. pylori), who were treated with a combination of tripotassium dicitrato bismuthate and metronidazole for a period of at least two weeks.Dual therapy with tripotassium dicitrato bismuthate plus metronidazole showed similarly high eradication rates (≥ 80%) of H. pylori from patients irrespective of age, gender or clinical diagnosis. Most importantly, dual therapy achieved a similar eradication rate of H. pylori infection in 41 patients who had previously been treated with tripotassium dicitrato bismuthate alone or in combination with an antibiotic other than metronidazole. It therefore appears that H. pylori does not become resistant to treatment with tripotassium dicitrato bismuthate.

    Notizen: Twenty-eight Helicobacter pylori-positive patients with metronidazole-resistant isolates and 25 with metronidazole-sensitive isolates were treated for 14 days with 40 mg omeprazole nocte plus 500 mg amoxycillin t.d.s. Eradication of H. pylori, defined as absence of the organism one month after cessation of treatment, was assessed using the [14C]urea breath test. The eradication rate in patients with metronidazole-resistant isolates was 14/28 (50%) while that in patients was metronidazole-sensitive isolates was 12/25 (48%). In contrast to these encouraging eradication rates, very poor results were obtained with a 7-day course of omeprazole (40 mg nocte) in combination with erythromycin ethylsuccinate (500 mg q.d.s.) and tripotassium dicitrato bismuthate tablets (120 mg q.d.s.). The latter eradication rates were 3/20 (15%) in patients taking erythromycin tablets and 3/19 (16%) in those taking a liquid formulation of erythromycin. All treatment regimens were well tolerated and all patients completed the prescribed course of therapy.
