    Notes: Sixty-nine healthy women in the early menopause were assessed in terms of calcium metabolism, coronary risk factors, Kuerman index, and various serum hormones before and during replacement therapy with l7β-oestradiol plus oestriol and norethisterone acetate. The patients were followed for 1 year, and the daily doses used were: 17β-oestradiol: 2 mg from day 1 to day 22, 1 mg from day 23 to day 28; oestriol: 1 mg from day 1 to day 22, 0.5 mg from day 23 to day 28; and norethisterone acetate: 1 mg from day 13 to day 22. Group A (n=22) received 17β-oestradiol and norethisterone acetate; group B (n=20) received 17β-oestradiol plus oestriol and norethisterone acetate and group C (n=23) received a placebo. The responses in groups A and B were similar with no significant difference between the two therapy regimens. The results suggest that in the dose given, oestriol does not add significantly to the beneficial effect of combined hormonal prophylaxis against early postmenopausal osteoporosis.

    Notes: Summary. Changes in sex hormones and bone turnover were studied longitudinally in 31 women aged 47–54 years who were approaching the menopause. Every 6 weeks for 2–3 years, hormones and biochemical estimates of the bone turnover were determined and the bone mass was measured at two forearm sites by single photon absorptiometry. Spinal bone mass was measured every 6 months. The bone turnover was normal in women aged 47–54 years with regular menstruation, whereas the estimates of bone resorption were high in the women with irregular menstruation. In nine women, who reached the menopause during the study, bone resorption increased significantly, whereas bone formation showed only a small increase. When the results of the nine women were combined with those of 50 women, who had passed a natural menopause within the preceding 3·5 years, the bone resorption indices reached their peak within the first postmenopausal year, whereas bone formation increased until 1·5–2 years after the last menstrual cycle. At the ultradistal forearm site the rate of bone loss was maximal (5% per year) immediately after the menopause and subsequently declined, which suggests that trabecular bone is more sensitive than cortical bone to changes in bone turnover. Spinal bone loss was identical in late peri-and early postmenopausal women. We conclude that bone resorption starts to increase during the last perimenopausal years, with a beginning acceleration in bone loss, which then becomes sharp after the menopause. The changes are related to the decline in oestrogens, but other mechanisms may also play a role.

    Notes: In a double blind, randomised, controlled investigation the analgesic effect of a continuous block of the lumbar plexus with bupivacaine compared with sodium chloride was examined in 20 patients with postoperative pain after knee-joint surgery. The infusion was given through a catheter inserted in the neurovascular fascial sheath of the femoral nerve, according to the three-in-one block technique. The patients treated with bupivacaine had significantly lower pain scores and a significantly lower demand for morphine. Side effects related to the catheters or the infusions of bupivacaine were not observed.

    Notes: A prospective, randomised study was performed to investigate the influence of temperature on sensory blockade in spinal anaesthesia. Three ml of plain bupivacaine 0.5% were injected intrathecally at either 4°C, room temperature, or 37°C. There were 10 patients in each group, who were kept sitting for 2 minutes after injection. The maximum level of sensory blockade was significantly higher (p 〈 0.01) in the group who received the solution adjusted to 37°C, and variability of level was smaller (p 〈 0.05). Time to two-segment regression was shorter in the 37°C group than in the 4°C group (p 〈 0.05). Hypotension required administration of ephedrine more often in the 37°C group (p 〈 0.05). It is concluded that the use of plain bupivacaine 0.5% adjusted to 37°C results in a higher and more predictable sensory blockade.

    Notes: In a prospective randomised study in 20 insulin-dependent diabetics who had minor surgery under general anaesthesia we compared the metabolic responses to intravenous glucose-insulin-potassium infusion with those who had conventional subcutaneous insulin administration. The former treatment resulted in lower blood glucose levels both during the infusion period (p 〈 0.05) as well as the entire observation period (operative, first and second postoperative days; p 〈 0.01). More blood glucose values were within the intended range of 5 to 10 mmol/litre in the glucose-insulin-potassium as compared to the conventional group (48% versus 24%; p 〈 0.01). The levels of lactate, 3-hydroxybutyrate, glycerol, alanine, glucagon, insulin and growth hormone did not differ between the two groups. The infusion regimen resulted in better glycaemic control both peri-and postoperatively than the conventional subcutaneous insulin regimen in insulin-dependent diabetic patients who have minor surgery.

    Notes: This study investigated the regional distribution of fatty and lean tissue in long-distance runners, and the relation to training, sex hormones, and serum lipids. One hundred and twenty lean men (22 elite, 86 recreational runners and 12 non-running controls) aged 32±8.1 years (mean±SD) participated. Body composition (adipose and lean tissue) was measured by dual-energy x-ray absorptiometry in the total body and in the abdomen, the arms and the legs. Regional and total body fat correlated inversely with the performance at an incremental treadmill exercise test (−0.61〈r〈−0.52, P〈0.0001), and the fat percentage in the abdomen and in the legs was 42% and 36% lower in the elite runners in comparison with the non-running controls. Sex hormonal status and serum lipids were unrelated to training. After multiple regression analysis the most significant determinant of the fat percentage in the legs was the weekly distance run (partial r=−0.40, P〈0.0001), whereas in the abdominal region the free testosterone index also contributed strongly (partial r=0.39, P〈0.0001). In conclusion, long-distance runners had very low amounts of fatty tissue in the abdomen and in the extremities, and the fat percentages in the abdomen and in the legs were associated with both the training intensity and androgenic activity. Since the abdominal fatty tissue is a significant risk factor for cardiovascular disease, running may have a positive impact on the long-term risk.
