    Notes: Abstract The authors report some unusual pediatric conditions where grey scale ultrasound was the most useful diagnostic tool. Cases of pericardial cyst, polycystic kidney disease of the adult type, hepatic hamartoma and hepatocarcinoma, gallstones, renal pseudo tumors, calcified inferior vena caval thrombus, acute pancreatitis, multicystic kidney and retroperitoneal lymphangioma are briefly described and compared to the literature. In a patient presenting with hereditary tyrosinemia, ultrasound not only disclosed evidence of hepatocarcinoma but was shown to be the procedure of choice for following the course of the disease.

    Notes: Abstract Depression in children is currently an area of considerable controversy, as is the use of potent psychopharmacologic agents in children. Since EEG sleep techniques have proven to be useful in under-standing the mechanisms of depression in adults and in predicting their response to antidepressants, a pilot study employing these techniques was undertaken in a population of hospitalized children. The EEG sleep of 12 children with significant depressive symptomatology was first examined after a two-week drug-free period and again approximately three weeks later when an optimum dose of imipramine had been maintained for at least 7–10 days. Changes in sleep continuity, as reflected in increased wakefulness and a decreased sleep efficiency, as well as an increase in Stage 2 and a decrease in Stage 4 sleep, were observed throughout the entire sample. REM suppression was also noted, but tended to be most pronounced in those children who improved on imipramine.

    Notes: Summary In 108 two-and three-year-old children the concentration of lead, cadmium, manganese, and zinc in blood, of free erythrocyte porphyrin in erythrocytes, and of haemoglobin and haematocrit were measured. The relation with distance of residence to a secondary smelter and with socio-economic class was studied. The ranges were for PbB 45–383 ppb, CdB 0.2–3.5 ppb, ZnB 2.0–9.7 ppm, MnB 2.7–14.1 ppb, FEP 10.5–138.9 μg/100 ml rbc. PbB was influenced independently by the distance to the source of emission and by the socioeconomic class. FEP increased with PbB; Hb and Ht were not affected. The results suggest that the Biological Quality Guide for blood lead as adopted in the European Community may have to be lowered for young children.

    Notes: Summary A case of primary leptomeningeal melanoma in a child is presented. The patient was studied with multiple neuroradiological methods, however computed tomography (CT) provided the most important information. The CT findings with leptomeningeal tumor, although not completely specific, do suggest the diagnosis, thus providing important information to explain the acute onset of symptoms in a previously healthy child. This case also differs from previously reported cases in terms of the type of hydrocephalus associated with the tumor.

    Notes: Abstract The purpose of this article is to present radiographic aspects of obstructive anatomical alterations of the upper airway before and during sleep in a 4 1/2-year-old boy affected with pycnodysostosis. The striking length of the uvula and the inspiration-exspiration changes in its position was an important factor in the obstruction of the upper airway. For better and earlier understanding of the actual mechanism of upper airway obstruction, the radiological data should be obtained during the same circumstances that the patient is experiencing his maximum discomfort.

    Notes: Abstract The value of radiological examinations in hypertension was analyzed in a series of 44 children. An i.v. urography had been performed in 43 cases with a pathological finding in 19 (44%). Renal angiography, employed in 19 cases, revealed abnormal findings in 12 (63%) patients. Micturating urethrocystography performed in 16 children gave no additional important information. The only complication noted was thrombosis of the femoral artery subsequent to renal angiography in one child less than one year of age. The diagnosis of hypertension based mainly on the i.v. urography in 12 cases but the renal angiography gave additional important information in 6 children. One child with obstructive hydronephrosis was also found to have a renal artery stenosis at renal arteriography. Based on these results, and particularly because secondary hypertension may frequently be treated surgically, we consider extensive radiological investigation with renal angiography is mandatory before receiving a final diagnosis of essential hypertension, and before starting long-term treatment.

    Notes: Abstract In a population of 158 women with a breast concern, two-thirds examined their own breasts. Most examiners (80%) practiced breast self-examination at least once a month, and over 60% had been regular examiners for more than 2 years. No significant difference in frequency of breast self-examination was found between women who had been examiners for longer and shorter time periods. Examiners had two main reasons for beginning and continuing breast self-examination: (1) an awareness that it is desirable to detect breast cancer early and (2) an awareness that they themselves could get breast cancer, while nonexaminers tended to deny this possibility. Therefore, to become examiners, nonexaminers may need to be convinced not only that early detection is effective but also that they too are at risk of breast cancer. The importance of informing, but not alarming, women about their breast cancer risk is discussed.

    Notes: Abstract Forty-two female subjects, constituting 14 groups, palpated a silicone breast model in a lump detection task. Three variables, number of trials (amount of practice), search pattern requirement (restricted vs. unrestricted), and the presence or absence of lumps, were manipulated according to 3 X 2 X 2 design. The effect of all modes of practice on detection performance in posttest 1 was evidenced by an increase in percent of lumps detected and a decrease in detection threshold, but no form of practice was superior to the others. Following posttest 1 a brief search-training procedure was administered to 39 subjects which further lowered detection threshold, increased percent of lumps detected, and increased trial duration in posttest 2. The results indicate the effectiveness of the training procedure for improving detection ability and suggest the need for a more complex model to determine more specifically the effects of the practice models employed.

    Notes: Summary A case of a 5 1/2 years old boy with a plexiform ameloblastoma of the jaw is reported. 6 years after resection of the primary tumor metastases developed in a lymph node at the left side of the neck and in both lungs. Thoracotomy revealed many small metastases spread all over the lungs. Light and electron microscopical studies of the primary tumor showed a typical plexiform ameloblastoma without signs of malignancy. In the metastases, however, these studies revealed the histological criteria of a malignant tumor with marked pleomorphism of tumor cells, increase of heterochromatin, enlargement of nucleoli and invagination of nuclear membranes. After enucleation of two metastases and therapy with cytostatic agents for several months no further metastases have been observed within a period of 6 years. Comparative light and electron microscopic studies of both primary tumor and metastases demonstrate that there are no histological criteria to identify the primary tumor as a malignant ameloblastoma. Ameloblastoma with distant metastases is a rare tumor in children, this case is only the third reported in the literature.

    Description / Table of Contents: Zusammenfassung Von 1971–1978 wurde bei 14 Kindern im Alter von 3 –14 Jahren mit Morbus Hodgkin eine Staging-Laparotomie durchgeführt. 6mal mußte die nach klinischen, radiologischen und szintigraphischen Befunden festgelegte Stadieneinteilung postoperativ geändert werden: I nach III (4ma1), II nach III (1mal) und III nach IV (1mal). Operationsbedingte Komplikationen traten nicht auf. Ein 3 Jahre alter Junge starb 6 Monate nach der Operation während der Chemotherapie an einer interstitiellen plasmacellulären Pneumonic.
    Notes: Summary From 1971–1978 14 children, 3–14 years old, with Hodgkin's disease, were submitted to a staging laparotomy. Staging according to clinical, radiological and scintigraphical findings had to be changed postoperatively in 6 cases: I to III (4 cases), II to 111 (1 case) and III to IV (1 case). None of these children had postoperative complications; while under chemotherapy a 3-year-old boy died of intestinal plasmacellular pneumonia.

    Description / Table of Contents: Zusammenfassung Die Behandlungsergebnisse von 33 schwersttraumatisierten kindlichen Extremitäten rechtfertigen fast immer einen operativen Erhaltungsversuch. Als limitierender Faktor erweist sich die Überschreitung der Ischämiezeit. Typische Komplikationen wie Infekt, Compartment-Syndrom und trophische Störungen bei Gefäß-/Nervenläsionen beeinflussen die Prognose. Defektfrakturen können infolge großer reparativer Potenzen bei Kindern unter Längenerhaltung stabil versorgt und sekundär mit autologer Spongiosa aufgebaut werden. Der Hautverschluß muß grundsätzlich spannungsfrei erfolgen.
    Notes: Summary The results of the treatment of 33 severely injured limbs of children justify aiming at surgical preservation. A limiting factor is the period of ischaemia. The prognosis is adversely affected by typical complications such as compartment syndrome and damage to vessels and nerves. Fractures with bone defects are stabilised under preservation of the length and built up with cancellous bone graft. Wound closure without tension is fundamental.

In einer Reihenultraschalluntersuchung wurden 305 Kinder nach dem B-Scan-Verfahren mit dem Videoson 735 und dem Multison 400 untersucht. In 5% war ein pathologischer Ultraschallbefund des Abdomens und des Retroperitonealraums zu erheben. Alle pathologischen Nierenbefunde wurden durch ein i.v. Pyelogramm weiter abgeklärt. Ergebnisse: Nierenagenesie (1), solitäre Nierencysten (2), doppeltes Nierenhohlsystem (3), Glomerulonephritis (3), Pyelonephritis (2), Splenomegalie (1), Steatose der Leber (4).
    Notes: Summary The value of abdominal ultrasonic scanning in children as a screening investigation is demonstrated. B-mode ultrasound was performed on 305 children. We examined the abdominal organs and the kidneys with the Videoson 735 and Multison 400 equipment. In 5% of the cases we diagnosed pathological processes: renal agenesis (1), renal cysts (2), duplex kidney (3), glomerulonephritic scarring (3), pyelonephritic scarring (2), splenomegaly (1), steatosis of liver (4).
