Zusammenfassung Unter der Beachtung der Indikation zur Operation und der Richtlinien für ihre Durchführung gibt es keine Argumente mehr, die gegen die Bevorzugung der Frühoperation bei der akuten Galle sprechen. Die Letalität der Frühoperation ist mit 1,8 % niedriger als bei der Intervalloperation (3 %). Der Vorteil besteht in einer Verminderung der Intervallmorbidität und in einer Reduktion von Komplikationen. Krankenhausverweildauer und Krankheitsdauer werden wesentlich abgekürzt. Die diagnostische Absicherung kann dabei parallel zur präoperativen Vorbereitung erfolgen.

Summary Taking into consideration the indications for operation and the guidelines for its practice, there are no more arguments against the early operation in acute cholecystitis. The lethality of early operation (1.8 %) is lower than that of delayed operation (3.0 %). With the early operation there is a reduction in morbidity and in the number of complications. Length of hospitalization and duration of disease are also essentially reduced. Whilst making sure of the diagnosis the preoperative preliminaries can be performed and the operation can be done within 24–48 h.
