Intramedullary stabilization is one alternative to plate osteosynthesis for the treatment of humeral shaft fractures. In the last 50 years many wire and nail systems have been developed for this purpose and used in clinical practice, many of which have involved problems with rotational stability. One of the recently developed nails is the unreamed humerus nail (UHN) for antegrade and retrograde insertion. The nail system and the operative technique are presented and discussed, as are the results of a multicentre study and the author’s own results.

Die intramedulläre Stabilisierung von Oberarmschaftfrakturen ist eine Behandlungsalternative zur etablierten Plattenosteosynthese. Hierfür wurden in den letzten 50 Jahren viele Draht- und Nagelsysteme entwickelt, wobei die Technik der Verriegelung des Implantats zur Rotationssicherung der Fraktur entscheidend ist. Eine der neueren Entwicklungen stellt der solide Humerusnagel dar (UHN), der retrograd und antegrad eingebracht werden kann. Nagelsystem, Operationstechnik und Ergebnisse einer Multizenterstudie sowie eigene Ergebnisse mit Komplikationsanalyse werden dargestellt und verglichen.
