    Notes: The H− source for HERA is based on the design of that at FNAL. It was modified according to the Deutsches Elektronen-Synchrotron requirements. There is only one H− source connected to the H− LINAC. It must run for long uninterrupted periods with a low duty factor and a high reliability. The measured cesium distribution showed that it is necessary to keep the cold box as a barrier. The operation of the cold box was improved with a new type of high voltage insulator and a special protection against condensation at the feedthrough for high voltage and −30 °C freon. The bending magnet of the cold box will be further improved by using a permanent magnet instead of the electromagnet. A unit was designed that includes all the essential elements which are under vacuum. It can be replaced in minutes and contains a newly designed H2 piezovalve and a remodeled magnetron. The cathode of the magnetron can be heated. It is now possible to run at a low repetition rate of 1/4 Hz and thus reduce Cs and H2 consumption.

    Notes: An electronic pepper pot has been developed at DESY for analyzing the H− beam. It consists of a number of slits in front of a harp with a high wire density. Multiplexing of the harp signals is done in the vacuum. The number of slits and the distance between the harp and the slits can be varied according to the beam parameters. The first version consists of a harp with 484 wires which are 0.1 mm wide with a spacing of 0.1 mm. They are printed on a board. The storage and readout is done using three chips in the vacuum. External transfer of the analog signals is done with a few lines. One may measure the current distribution and the emittance within one shot of the beam and is free to choose the time for the sample. This is of special interest for sources with a low repetition rate such as the HERA H− source. The present arrangement is only a first step. Further designs are discussed.

    Notes: Abstract Extracorporeal shock wave lithotripsy has recently been introduced as the first non-operative treatment alternative for patients with sialolithiasis. Using conventional multipurpose lithotripters, however, successful treatment was achieved in only 36%–53% of patients. Therefore we developed an miniaturized lithotripter meeting the special requirements for extracorporeal shock wave treatment in the head and neck region. During a 1-year prospective trial clinical efficacy and safety were compared in 40 patients treated with a conventional electromagnetic lithotripter (group A) to 33 patients treated with the newly developed, miniaturized device (group B). The groups did not differ statistically regarding stone size or number or the proportion of stones located in the submandibular or parotid gland. Successful stone targeting, a prerequisite for shock wave treatment, was achieved by means of in-line ultrasonography in 30 of the 40 patients in group A and in 29 of 33 patients of group B. The number of shock wave impulses administered per session and the maximum shock wave intensities did not differ in the two groups. Significantly more frequent treatments with a longer mean duration of each session were required in group A (2.4 ± 1.0 treatments, 47 ± 11 min) than in group B (1.9 ± 0.7 treatments, 28 ± 9 min; P 〈 0.05). After a 3 month follow-up significantly more patients were free of stones in group B (22/33) than in group A (16/40; P 〈 0.05). Correspondingly, the number of patients free of complaints was significantly higher in group B (27/33) than in group A (22/40; P 〈 0.05). Side effects did not differ between the two types of lithotripters, and they were moderate in intensity, completely reversible, and therefore without influence on the patient's outcome. It is concluded that the new miniaturized lithotripter provides a substantial progress for the feasibility and efficacy of shock wave treatment in sialolithiasis.

    Description / Table of Contents: Abstract. Securing the airway following surgery can be a problem in certain patients. Preparation, therefore, should include a plan for safe reintubation, if necessary. We report two patients with a difficult airway (recurrent thyroid carcinoma, inoperable carcinoma of the hypopharynx) who were at a high risk for intubation under general anaesthesia. In addition, one of the patients suffered from an acute abdomen and ileus. We used a guidewire placed into the trachea via a fiberscope to facilitate fiberoptic reintubation.
    Notes: Zusammenfassung. Die Sicherung der Atemwege nach Operationen im Bereich der oberen Luftwege kann bei bestimmten Patienten problematisch sein. Für eine sichere Reintubation im Bedarfsfall ist deshalb Sorge zu tragen. Berichtet wird über zwei Patienten, die wegen eines papillären Schilddrüsenkarzinomrezidivs bzw. inoperablen Hypopharynxkarzinoms erhebliche Intubationsschwierigkeiten und damit hochgradige Risikofaktoren für eine Intubation und ggf. Reintubation in Allgemeinanästhesie aufwiesen. Zusätzlich bestand bei einem Patienten eine Ileussymptomatik. Ein fiberoptisch plazierter endotrachealer Führungsdraht diente dabei als post- bzw. präoperative Reintubationshilfe.

    Notes: Summary The role of Epstein-Barr virus (EBV) in the pathogenesis of Hodgkin's disease (HD) has not yet been clarified. Using RNA in situ hybridization (ISH) and immunohistochemistry (IHC), the occurrence of small Epstein-Barr virus encoded RNA (EBER) and latent membrane protein-1 (LMP-1) was studied in 22 tissue samples from 21 patients between 4 and 17 years of age with Hodgkin's disease. EBER was detected in eight of 21 patients (38%) in Hodgkin and Reed-Sternberg cells and reactive lymphocytes irrespective of initial clinical stage and histological subtype, whereas LMP-1, positive in ten of 21 patients (48%), was restricted to neoplastic cells. All cases positive for EBER expressed LMP-1 as well. Additionally, oncoprotein Bcl-2 was identified in nine of 21 patients (43%), indicating, besides immortalization of HD cells by EBV, a further growth advantage due to apoptosis prevention by overexpression of this protein. Proliferation-associated antigens Ki-S1 and Ki-S5 were highly expressed in Hodgkin and Reed-Sternberg cells. CD 30 antigen was found in most cases, using two different antibodies (90% and 80%). The presence of this protein, which belongs to the family of nerve growth factor receptor (NGFR), is related to high expression of Ki-67 protein, detected by Ki-S5. CD 20 antigen was detectable in only three of 21 patients (14%). If we compare results of ISH and IHC with clinical data, the occurrence of EBV genome in children with HD seems to have no adverse effect on the final outcome of these patients.
