    Notes: The recent identification of a novel clinical entity, the severe acute respiratory syndrome (SARS), the rapid subsequent spread and case fatality rates of 14–15% have prompted a massive international collaborative investigation facilitated by a network of laboratories established by the World Health Organization (WHO). As SARS has the potential of becoming the first pandemic of the new millennium, a global warning by the WHO was issued on 12 March 2003. The disease, which is believed to have its origin in the Chinese Guangdong province, spread from Hong Kong via international airports to its current worldwide distribution. The concerted efforts of a globally united scientific community have led to the independent isolation and identification of a novel coronavirus from SARS patients by several groups. The extraordinarily rapid isolation of a causative agent of this newly emerged infectious disease constitutes an unprecedented scientific achievement. The main scope of the article is to provide the clinician with an overview of the natural history, epidemiology and clinical characteristics of SARS. On the basis of the recently published viral genome and structural features common to the members of the coronavirus family, a model for host cell–virus interaction and possible targets for antiviral drugs are presented. The epidemiological consequences of introducing a novel pathogen in a previously unexposed population and the origin and evolution of a new and more pathogenic strain of coronavirus are discussed.

    Description / Table of Contents: Résumé Dix patients non-diabétiques, atteints de maladie psychiatrique, n'ayant pas été traités auparavant par l'insuline, ont subi une thérapeutique par coma insulinique avec de l'insuline humaine contenant plusieurs fractions, recristallisée et mise en solution neutre. Le traitement a été administré pendant 1–3 mois, à des doses maximales de 96–196 unités par jour. Plusieurs patients ont élaboré des anticorps anti-insuline. La cause de la formation d'anticorps est discutée. La présence de petites quantités de la fraction-a dans l'insuline semble avoir une importance dans la formation des anticorps anti-insuline.
    Abstract: Zusammenfassung Zehn nicht-diabetische psychiatrische Patienten, die vorher nicht mit Insulin behandelt worden waren, wurden einer Insulinkomatherapie mit einem nicht-monokomponentartigen Humaninsulin in neutraler Lösung unterzogen. Die Behandlung wurde 1–3 Monate lang mit Maximaldosen von 96–196 E täglich durchgeführt. Mehrere Patienten erzeugten Insulinantikörper. Die Ursache der Antikörperentstehung wird diskutiert. Die Anwesenheit kleiner Mengen der a-Fraktion des Insulins könnte für die Insulinantikörperbildung von Bedeutung sein.
    Notes: Summary Ten non-diabetic psychiatric patients, who had not previously been treated with insulin, underwent insulin coma therapy by recrystallized, non-monocomponent human insulin in neutral solution. The treatment was given for 1–3 months, in maximum doses of 96–196 units daily. Several patients formed insulin antibodies. The cause of the antibody formation is discussed. The presence of small quantities of the a-fraction in the insulin is assumed to be of importance in the insulin antibody formation.

    Notes: Summary Side effects of insulin treatment (red spots at the injection site, lipoatrophy and insulin antibody formation) are primary related to the purity and the species of the insulin preparations used in treatment. To evaluate the clinical significance of these factors daily insulin requirements were registered in 1685 protamine insulin treated cases; red spots and lipoatrophy recorded in 500 NPH insulin treated patients and insulin antibodies determined in 173 patients. 51 of the patients were treated for more than 12 months with highly purified porcine insulin (proinsulin contamination 〈0.5 mmol/mol). 96 patients were treated for 1–4 years with porcine NPH insulin (proinsulin contamination 12–13 mmol/mol), and most of the remaining patients have been treated with mixed porcine-bovine insulin preparations in earlier periods. It was shown that high purification of porcine NPH insulin is of importance in reducing insulin dose, lipoatrophy and insulin binding capacity of serum. Thus insulin antibodies could be demonstrated in only 3 of 51 patients after 11/2 year of treatment with highly purified porcine NPH insulin. None of these patients developed red spots at the injection site and only one patient developed a small shallow depression of the subcutaneous tissue on one thigh. The daily insulin requirement was significantly lower when compared with the insulin requirement of a porcine NPH insulin treated control group.

    Notes: Summary The present study was partly a clinical pharmacological analysis of the action of imipramine in nocturnal enuresis and partly an attempt to select children who would benefit most from this treatment. 61 fairly severe cases of nocturnal enuresis were studied as out-patients, divided by the criteria primary/secondary enuresis and with/without associated behavioural disturbances. The patients received imipramine, imipramine-N-oxide, emepronium, and a placebo in random sequence in a double-blind cross-over trial. There was no difference in effect between placebo and emepronium; after imipramine the frequency of enuresis fell to 58% of the figure during placebo treatment, i.e. 31% of the children became dry; imipramine-N-oxide reduced the number of wet nights only to 78% of the number on placebo, 18% of the children became dry. When the patients were classified into groups imipramine and imipramine-N-oxide were found to have had the greatest effect in cases of presumed psychogenic aetiology, a 65% reduction of the frequency of wet nights whilst taking the placebo. The most favourable results were in children with combined nocturnal and diurnal enuresis and in those with urinary frequency and imperative urgency. The effect was maintained during treatment for 3 months with the most suitable drug. The dose was 50 mg regardless of the body weight of the patient. There were no serious side effects. It was concluded that the psychopharmacological actions of imipramine and imipramine-N-oxide appear to be important in their effects on enuresis. Imipramine might have been the best drug because success may require the combination of antidepressant and anticholinergic activity. It was not possible to assess in detail its presumed effect on the level of sleep. Thus many children can be treated successfully and the final cure of enuresis may be hastened by breaking a vicious circle of psychological phenomena.

    Notes: Abstract  The effects of cadmium exposure and dietary status on cadmium accumulation, fatty acid (FA) content and profiles were investigated in two colour forms of the shore crab Carcinus maenas. Groups of shore crabs were either starved or fed with blue mussels, Mytilus edulis, during a 40 d exposure period to 2 or 6 μM Cd2+ (as CdCl2). Starved green individuals accumulated more cadmium in haemolymph and hepatopancreas than did red crabs and green crabs fed during the experiments. In the red colour form, no difference in cadmium accumulation was observed between starved and fed individuals. In both colour forms, hepatopancreas contained more FA than gills and muscle. The FAs often present in the largest amounts in the tissues were 16:0, 16:1ω7, 18:1ω7, 18:1ω9, 20:4ω6, 20:5ω3 and 22:6ω3. However, saturated (SAFA) and mono-unsaturated fatty acids (MUFAs) were dominant in hepatopancreas, whereas poly-unsaturated fatty acids (PUFAs) were dominant in gills and muscles. At the beginning of the experiment, the total FA content in the hepatopancreas was 111.6 mg g−1 (dry weight) for red crabs and 78.4 mg g−1 for green shore crabs. During the experiment, however, the FA content decreased in red crabs. This decrease was more pronounced for starved individuals than for fed individuals. Also, the decrease in FA content was more pronounced in crabs exposed to 6 μM cadmium compared to crabs exposed to 2 μM or crabs not exposed to cadmium. No change in FA content was observed in green shore crabs, irrespective of diet and cadmium exposure. For both colour forms, no change in FA content was observed for gills and muscle. In red crabs, a decrease was observed for all FAs in the hepatopancreas. This decrease, however, was more pronounced for SAFAs and MUFAs than for PUFAs, indicating that the metabolism of FAs during starvation and cadmium exposure is selective. The experiments indicate that green colour forms of shore crabs are more tolerant of natural stress such as starvation and anthropogenic stress, e.g. cadmium exposure, than are red colour forms of shore crabs.

    Notes: Abstract  The relationship between intermoult duration (coloration), sex, size and seasonal variations in fatty acid (FA) profiles was studied in a population of shore crabs, Carcinus maenas, inhabiting the Isefjord, Denmark. For male shore crabs, the total hepatopancreas FA content was high in July and December (12.7 to 16.0 mg g−1 dry weight, dw) but lower in May and September (7.3 to 10.0 mg g−1). This indicates that male shore crabs are in relatively good condition before winter, when the crabs migrate off shore, but in relatively poor condition when they return to shallow waters during spring. The hepatopancreas FA content also decreased over the mating season. After the mating season the hepatopancreas FA content of males had decreased to approximately 60% of that prior to the mating season. Female shore crabs had significantly higher hepatopancreas FA levels than males in May (11.7 mg g−1 dw), September (12.6 mg g−1 dw) and December (17.9 mg g−1 dw) but lower levels in July (9.5 mg g−1 dw). This indicates that the spawning season is the most energy-demanding part of the female reproductive cycle. For all seasons, the hepatopancreas FA content of green shore crabs was significantly higher than that of red shore crabs. For both colour forms, the amount of polyunsaturated fatty acids (PUFAs) was significantly higher than that of saturated fatty acids (SAFAs) and monounsaturated fatty acids (MUFAs), with the relative proportion of PUFAs increasing when the total hepatopancreas FA content decreased. For both genders and colour forms, the most dominating SAFA was palmitic acid (16:0). Palmitoleic acid (16:1ω7), vaccenic acid (18:1ω7) and oleic acid (18:1ω9) were the three MUFAs found in highest concentrations. The most dominating PUFA was eicosapentaenoic acid (EPA, 20:5ω3). Docosahexaenoic acid (DHA, 22:6ω3) and arachidonic acid (AA, 20:4ω6) were also abundant in all samples. The results demonstrated that season, sex, size and intermoult duration all influence the amount of FAs present in the hepatopancreas of shore crabs.

    Notes: Abstract  Effective antiemetic treatment of patients who have previously experienced chemotherapy-induced nausea and vomiting is difficult. The aim of this study was to evaluate the antiemetic efficacy of a single intravenous dose of granisetron plus a 3-day oral treatment with prednisolone 25 mg once a day plus metopimazine 30 mg four times a day in patients refractory to previous antiemetic treatment with granisetron or with prednisolone plus metopimazine. The study population was made up of 25 consecutive women with stage I or II breast cancer, who were treated with multiple cycles of adjuvant cyclophosphamide, fluorouracil plus methotrexate or cyclophosphamide, epirubicin plus fluorouracil given i.v. every 3 weeks. Patients received the three-drug combination of antiemetics during a total of 113 cycles of chemotherapy. No emetic episodes were reported in 88.9% cycles on day 1, in 94.7% cycles on days 2 through 5 and in 85.8% cycles on days 1 through 5 after chemotherapy. No nausea was reported in 43.4% cycles on day 1, in 49.6% cycles on days 2 through 5 and in 34.5% cycles on days 1 through 5. Nineteen patients (76.0%) completed the scheduled nine cycles of chemotherapy, 1 being withdrawn because of ≥5 emetic episodes and 5, because they were not satisfied with the antiemetic treatment. The treatment was well tolerated. In conclusion, granisetron plus prednisolone plus metopimazine is a highly effective antiemetic treatment in patients receiving moderately emetogenic chemotherapy refractory to antiemetic therapy with granisetron or prednisolone plus metopimazine.
