    Notes: A simple and efficient method to prepare isomorphous derivatives of protein crystals with xenon as a heavy atom is described. The method consists of exposing a crystal to xenon gas of pressures above 5 atm ( ̃0.5 MPa) for several minutes and subsequently shock-freezing the crystal to immobilize the xenon dissolved in the mother liquor and bound to the protein. Diffraction data can the be collected with the established techniques of protein cryocrystallography. Two types of high-pressure device are described to expose a protein crystal to the required xenon pressure, permitting rapid freeze-quenching after xenon exposure. One of these devices can be used for gas pressures up to and exceeding 5.0 MPa, with a gas consumption of a few millilitres of uncompressed gas. The technique has been tested with monoclinic crystals of sperm-whale metmyoglobin, which has four xenon binding sites. The results of these experiments are described and discussed. Potential applications of this technique include-besides the classical multiwavelength anomalous diffraction (MAD) or single isomorphous replacement with anomalous scattering (SIRAS) experiments-the derivation of low-angle phase information by modifying the electron density of the solvent regions within the crystal.

    Notes: Summary Diabetic nephropathy represents a major complication in patients with insulin-dependent diabetes mellitus (IDDM). Intervention trials using angiotensin-converting enzyme (ACE) inhibitors have pointed towards the important pathogenetic role of the renin-angiotensin system. Recently an insertion/deletion (I/D) polymorphism for the gene encoding the ACE has been described, the deletion type being associated with higher plasma ACE levels. As the intrarenal renin-angiotensin system might also be activated in this setting, we determined the ACE genotype together with other risk factors for the development of diabetic nephropathy in 122 patients with IDDM from a single centre with (n = 63) and without (n = 59) nephropathy. Long-term glycaemic control was evaluated using mean HbA1c values from the last 10 years. The two patient group were comparable with regard to duration of diabetes and glycaemic control as assessed by current HbA1c values. However, mean long-term HbA1c values were significantly higher in patients with diabetic nephropathy as was systemic blood pressure. The DD genotype was more prevalent in patients with renal disease. In the subgroup of patients who had had diabetes for more than 20 years (n = 90), the DD genotype was even more frequent in patients with nephropathy, and blood pressure and long-term HbA1c values were also higher in patients with renal disease. Logistic regression analysis revealed long-term glycaemic control, blood pressure and the ACE genotype to be independent risk factors for the prevalence of diabetic nephropathy.     Notes: Abstract. Image guidance of instruments can be performed by fluoroscopy, ultrasound imaging, computed tomography (CT), magnetic resonance imaging (MRI), and by stereotactic navigation. During the last year, there has been significant progress in MRI instrument guidance in the field of interventional radiology. Our first 168 clinical cases of MRI-guided interventional procedures, e.g., aspiration biopsy of neoplasms and tomographic microtherapy with local interstitial chemo-ablation, confirm the feasibility of MRI guidance. An expansion of MRI guidance during surgical endoscopic procedures is currently under evaluation and the initial results of this development are presented. Tomographic-guided surgery and the implementation of MRI or CT scanning within the environment of an operating room (OR) entail specific technological requirements and OR design considerations. Fast sequences, interventional protocols, in-room monitor, as well as MR-compatible probes, cannulae, catheters, instruments, endoscopes, and auxiliary equipment that are necessary for this combined surgical image-guided approach are described.

    Description / Table of Contents: Zusammenfassung. Bei 1–6 % der Patienten, die sich einer endoskopisch retrograden Cholangiopancreaticographie (ERCP) unterziehen, wird ein Pancreas divisum diagnostiziert. Bei einigen dieser Patienten kommt es zu akut rezidivierenden Pankreatitisschüben und schließlich zu einer chronischen Pankreatitis (CP). Operative Verfahren bieten Aussicht auf langfristige Beschwerdebesserung und Heilung. Wir berichten über unsere Erfahrung mit der duodenumerhaltenden Pankreaskopfresektion (DEPKR) bei 12 Patienten mit Pancreas divisum und CP. In der präoperativen Abklärung fanden sich bei allen Patienten neben einem Pancreas divisum klinische, funktionelle und radiologische Zeichen einer CP. Die DEPKR wurde in allen Fällen ohne perioperative Komplikationen durchgeführt. Bei 10 Patienten zeigte sich postoperativ eine pathologische exokrine Pankreasfunktion (präoperativ bei 8 Patienten), während sich im postoperativen oralen Glucosetoleranztest (OGTT) die endokrine Funktion zweier Patienten normalisierte. Elf Patienten, die nach einer mittleren Beobachtungszeit von 31 Monaten (3–75 Monate) nachuntersucht wurden, waren bei fehlender spätpostoperativer Letalität schmerzfrei. Der OGTT zeigte bei zwei Patienten eine gestörte endokrine Pankreasfunktion. Neun Patienten mußten bei gestörter exokriner Funktion medikamentös substituiert werden. Eine Patientin wurde 10 Monate nach Erstoperation aufgrund einer Gangstenose im Bereich der Pancreaticojejunostomie mittels Neuanlage der Anastomose reoperiert. Zusammenfassend kann gesagt werden, daß die DEPKR bei Patienten mit Pancreas divisum und CP zu einer anhaltenden Beschwerdefreiheit und bei einem Teil dieser Patienten zu einer Verbesserung der endokrinen Funktion führen kann. Dieses Verfahren sollte den nicht resezierenden, operativen Eingriffen oder endoskopischen Maßnahmen vorgezogen werden.
    Notes: Summary. In 1–6 % of the patients who are investigated by endoscopic retrograde cholangio-pancreatography a pancreas divisum can be found. In some patients pancreas divisum can lead to an acute relapsing and finally chronic pancreatitis (CP). Surgical intervention in these cases seems to offer a good chance of recovery. We report our experience with the duodenum-preserving resection of the head of the pancreas in 12 patients with pancreas divisum and CP. In all patients the preoperative evaluation showed clinical, functional or radiological signs of CP. The duodenum-preserving resection of the head of the pancreas was carried out in all patients without perioperative mortality. Ten postoperative versus eight preoperative patients showed a pathological exocrine function of the pancreas. Endocrine function, measured by the oral glucose tolerance test (OGTT), improved postoperatively in two patients. Eleven patients who were investigated after a mean follow-up time of 31 months (3–75 months) were completely pain free. No late mortality occurred. OGTT revealed a diabetic endocrine function in two patients. Disturbed exocrine pancreatic function had to be substituted in nine patients. One patient had to be reoperated by duct incision and renewal of the pancreatico-jejunostomy 10 months after the first operation. In conclusion, the duodenum-preserving resection of the head of the pancreas reduced pain in all patients with pancreas divisum and CP and may lead to an improvement of endocrine pancreatic function. Other, nonresecting procedures or endoscopic interventional therapy should be avoided in these patients.

    Notes: Abstract The present study investigates the relationship between scale scores on the Dutch version of the Multidimensional Pain Inventory (MPI-DLV) and data derived from comparable items from an experience sampling procedure. Fifty-seven chronic pain patients participated in the study, which lasted 6 consecutive days. Special attention was given to the relationship between the MPI-DLV pain intensity score and the mean experience sampling pain intensity score. Significant correlations were found between the MPI-DLV scales Pain Severity, Interference, Solicitous, Punishing and Distracting Responses, and Household Chores and their experience sampling analogues. A marginally significant correlation was found with regard to the MPI-DLV Life Control scale. The General Activity and Affective Distress scales had no relationship with the analogous experience sampling items. The significant correlations were regarded as further validation of the MPI-DLV. A regression analysis revealed that 58% of the variance of the experience sampling pain intensity score could be explained by the MPI-DLV present pain intensity item score.

    Notes: Abstract The glycopeptide antibiotic vancomycin blocks cell wall synthesis in Escherichia coli only when it can reach its target site in the periplasm. In vivo, sensitivity to vancomycin is enhanced in the presence of the hemolysin (hly) determinant of E. coli or its translocator portion hlyBD. Two different mutations in hlyD alter the cell's susceptibility to vancomycin: mutations in the tolC-homologous region of hlyD increase vancomycin resistance, whereas mutations at the 3′-terminus of hlyD lead to hypersensitivity to vancomycin and to the accumulation of large periplasmic and cytoplasmic pools of this antibiotic in E. coli. These effects are only observed in the presence of functional HlyB and TolC, the two other components of the hemolysin secretion machinery. A defect in TolC causes hyperresistance to vancomycin, even when present together with a mutant HlyD protein which in the presence of TolC renders E. coli hypersensitive to vancomycin. Lipid bilayer experiments in vitro revealed specific interactions between TolC and vancomycin or HlyD protein. Second-site suppressor mutations in hlyD and hlyB were obtained, which abolish the hypersensitive phenotype caused by the 3′-terminal mutations in hlyD. Our results are compatible with the idea that (a) TolC, together with the TolC-homologous part of HlyD, forms a pore in the outer membrane through which hemolysin is released and vancomycin taken up; and (b) the C-terminal sequence of HlyD interacts with periplasmic loop(s) of HlyB to form a closed channel spanning the periplasm.

    Notes: Summary The gene for methylenetetrahydrofolate reductase (MTHFR) has shown polymorphism in the general human population. In its homozygous form, a C677T mutation occurs in more than 5% of the grown-up population and produces a thermolabile variant which reduces the overall enzyme activity to less than 30% of normal. We investigated patients with schizophrenia-like psychosis. If hyperhomocysteinemic, their DNA-genotype for thermolabile C677T mutation was determined. Seven of 11 patients, six males and one female, were homozygous for thermolabile MTHFR. One male patient was heterozygous and all three normal homozygotes were females. In the patients who were homozygous for the C677T mutation, the homocysteine concentrations did not respond to vitamin B12 but were normalized by folate supplementation. In the normal homozygotes, however, the homocysteine concentrations were reduced by vitamin B12 alone. Our results suggest that homozygosity for thermolabile MTHFR is a risk factor for schizophrenia-like psychosis. Possibly, this risk may be reduced by folate supplementation.

The structures of 3 (monoclinic; space group P21/c; a = 917.0, b = 1344.7, c = 1080.8 pm and β = 103.8°; Z = 10), 4 (orthorhombic; space group Pbcn; a = 1028.9, b = 1132.6, c = 1074.1 pm; Z = 8) and 6 (monoclinic; space group C2/c; a = 2041.1, b = 1101.4, c = 3365.6 pm and β = 153.8°; Z = 8) are determined by X-ray analysis. In 6 Sb is coordinated in a distorted octahedra by 6 Cl in three short (mean 245,5 pm; SbCl3) and three long distances (291 to 299 pm; Cl-). Two of the chloride ions connect the Sb atoms to infinite Sb … Cl … Sb chains. Except for 7 and 9 there are bridges between the NH2 groups and the halide ions. The NH valence vibrations are discussed in view of hydrogen bonding.
