    THE ACTION OF ASPIRIN ON PLASMA KININOGEN AND OTHER PLASMA PROTEINS IN RHEUMATOID PATIENTS: RELATIONSHIP TO DISEASE ACTIVITY (1980)

When six female seropositive rheumatoid patients were given placebo therapy for 48 h, their mean plasma kininogen level, 9.2 ± 0.7 μg bradykinin equivalents (bk eq) per ml, was found to be 59% greater than that of a group of eight healthy female volunteers (5.8 ± 0.5 μg/ml).2. When the rheumatoid patients received aspirin therapy for 1 week, their mean plasma kininogen concentration fell by 31% to 6.3 ± 0.8 μg Bk eq/ml. This was accompanied by a 20.4% fall in mean plasma α2-globuKn level. Haematocrit and total plasma protein were not significantly altered (P 〉 0.05).3. The fall in kininogen was very rapid, the main reduction occurring within the first hour.4. Aspirin therapy greatly reduced the pain assessments but had no effect on plasma concentrations of IgG, IgA, IgM, complement component C3, nor on ESR, haemoglobin, leucocyte count, nor ring size. Left hand grip strength was increased while right hand grip strength was unchanged.5. The action of aspirin on plasma kininogen and α2-globulin was similar to that of indomethacin. Plasma kininogen has been considered to be an acute phase reactant. The possible diagnostic value of plasma kininogen estimation is discussed.

    Description / Table of Contents: Zusammenfassung Ein neues Konzept der direkten zementlosen Fixation einer Hüftgelenkspfanne aus Polyaethylen bei der Totalprothesen-Arthroplastik der Hüfte wird vorgestellt. Die kugelförmige Pfanne wird mit Hülfe von zwei Zapfen and Schrauben direkt auf dem entknorpelten subchondralen Knochen der Hüftgelenkspfanne fixiert. Das Polyaethylen steht dabei in direktem Kontakt mit dem Knochen und folgt den elastischen Deformationen des Beckens (Isoelastizität). Knochengewebe kann in die Vertiefungen der Außenfläche des Implantates einwachsen. Dadurch wird die physiologische Kraftübertragung im Hüftgelenk am wenigsten gestört. Das Ziel dieses Konzeptes ist somit eine „Biomechanische Inkorporation” des Implantates in einem neuen stabilen Zustand. 250 auf these Weise zementlos fixierte Polyaethylenpfannen mit einer Beobachtungszeit von sechs Monaten bis vier Jahren und einem Durchschnitt von 19 Monaten wurden nachkontrolliert. Bis heute wurde noch keine aseptische Lockerung beobachtet.
    Notes: Summary A new concept of cementless fixation of a polyethylene acetabular component for total hip arthroplasty is presented. The spherical cup is fixed directly to the subchondral bone of the acetabulum by means of two pegs and screws. The polyethylene in direct contact to the bone adjusts to the deformation of the pelvis (Isoelasticity). Bone grows into the indentations of the outer surface of the implant. The elastic cup does not interfere with the physiological stress patterns of the hip joint so that a stable “biomechanical incorporation” of the implant is achieved. A follow-up of the first 250 cementless fixed polyethylene cups with an observation period of six months to four years with an average of 19 months is reported. So far no aseptic loosening of the implant has occurred.

Der neuentwickelte Fixateur interne, bei dem durch die Pedikel von dorsal her in die Wirbelkörper eingebrachte Schanzschrauben winkel- und rotationsstabil miteinander verbunden werden, zeigt folgende Vorteile: Fixation von nur 2 Segmenten, hohe Stabilität unabhängig von der Frakturform, hervorragendes Hilfsmittel zur Frakturreposition, klinische Bewährung in 35 Fällen.
    Notes: Summary From a posterior approach long Schanz screws are inserted through the pedicles into the bodies of the two vertebrae just adjacent to the lesion and connected by the threaded Fixateur-interne-rods. By tightening the nuts the Schanz screws are fixed in all directions. The advantages of the F.I.-system are: excellent reposition by the long lever-arm of the Schanz screws, immobilization of only two segments avoiding the iatrogenic loss of mobility and lumbar lordosis, stability against flexion forces better than with Harrington distraction rods, additional rotational stability.
