    Description / Table of Contents: Zusammenfassung Von den bis Juli 1974 am Texas Heart Institute durchgeführten 3780 aorto-coronaren Bypass-Operationen waren 95 Patienten 70 Jahre oder älter. Die anfänglich hohe Operationsmortalität in dieser Altersgruppe mit 27% stand in klarem Gegensatz zur Gesamtmortalität von 7,1% und konnte im letzten Jahr - hauptsächlich auf Grund von Verbesserungen in der Operationstechnik - von 27 % auf 4,8 % reduziert werden. Damit kann auch beim alten Patienten, im Einzelfall, eine coronare Bypass-Operation als Behandlungsmöglichkeit ins Auge gefaßt werden.
    Notes: Summary From October 1969 through June 1974, 95 patients over 70 years of age underwent aortocoronary bypass at the Texas Heart Institute. The primary indication for surgery was severe angina, which was present in 88 patients. The mean coronary artery score was 9.51. Improvements in surgical technique are responsible for a reduction in the hospital mortality from 22.1 percent in all 95 patients to only 4.8 percent in the 21 patients operated upon during the first 6 months of 1974.

    Description / Table of Contents: Zusammenfassung 225 Patienten wurden wegen aktiver bakterieller Endokarditis oder der Klappenkomplikationen operiert. Die Frühmortalität betrug 12%. Bei 16 Patienten kam es zur Prothesendehiszenz - davon überlebten 6 Patienten die notfall mäßige Reoperation und 7 Patienten verstarben -, eine Indikation zu ausgedehnter Resektion des erkrankten Gewebes. Das mittlere Operationsalter betrug 43 Jahre. Auf Grund der vorliegenden Daten darf angenommen werden, daß die Mortalität durch eine kombinierte klappenchirurgische- und Coronararterien-Operation - falls indiziert-weiter gesenkt werden kann.
    Notes: Summary Between 1959 and 1973, 225 patients were operated on because of active bacterial endocarditis or its valvular complications. The hospital mortality was 12 percent and 60 patients died an average of 2 years after the operation. Seven early deaths occurred among 16 cases of acute dehiscence of the prosthetic valve-an indication for extensive resection of all diseased tissues. The average age was 43 years and there were 21 deaths due to myocardial infarction. The authors believe that the operative mortality can be further reduced through combined valvular and coronary artery surgery where this is indicated.

    Notes: Abstract The current-induced step structure in the phase transition from the superconducting to the normal conducting state of whiskers of tin shows hysteretic behaviour. By shunting the whisker by a small normal conductor a stable reentrant part of theV(I)-characteristic (negative differential resistance) could be measured for the first step both by de and ac instruments. If the condition of stabilization is not fullfilled, the voltage in the hysteretic region shows oscillations. These oscillations were measured and calculated with a simple model.

    Notes: Summary The maturation of calcium phosphate crystals formed by the conversion of spontaneously precipitated amorphous calcium phosphate (ACP) was studied in aqueous media at temperatures ranging from 20° to 37°. Reaction pH was kept at 7.4 with either Hepes buffer or by the pH-stat addition of base. Reaction kinetics were followed by monitoring solution calcium and total phosphate, and, in the pH-stat controlled reaction, by recording the amount of KOH needed to maintain pH. Reaction products were examined chemically and by X-ray diffraction and transmission electron microcopy. The first crystals to form deviated markedly from apatite in morphology, composition, structure, and solubility. They were extremely thin and flaky in appearance, had a low Ca/P molar ratio (1.4), contained an appreciable amount of acid phosphate (16%), and had an exceptionally largea-axis (10.5 Å vs. 9.4 Å for apatite). With maturation, the crystals became thicker but smaller in lateral dimensions, more apatitelike in structure and composition, and less soluble. However, this ripening of the crystals was accompanied by unusual inflections in the solution Ca and total PO4 curves, and, in the case of the pH-stat experiments, in the OH consumption profiles as well. These anomalous post-ACP solution changes suggest that a phase change had taken place during crystal maturation. Although the observed structural and compositional changes are not inconsistent with the perfection of an initially defective apatite, the changes in crystal morphology and the anomalous behavior of the reaction solution may more accurately reflect a conversion of the ACP first into an OCP-like crystalline phase which subsequently hydrolyzes into apatite.

    Description / Table of Contents: Zusammenfassung Bei 17 narkotisierten Hunden wurden mehrere Seitenäste des Ramus descendens der linken Kranzarterie unterbunden. Die ST-Segmentelevation im epikardial abgeleiteten EKG stieg dabei bis auf 22 mV nach 5 min und 19 mV nach 20 min an. Aortendruck, enddiastolischer Druck im linken Ventrikel, Herzfrequenz und die hämostasiologischen Parameter (Plasmathrombinzeit, Thrombinkoagulasezeit, Reptilasezeit, Plasmafibrinogen, Staphylokokken-Clumping-Test) änderten sich nicht. 20 min nach Beginn der Okklusion wurden die Gefäße geöffnet. Nachdem sich die St-Segmenthebung zurückgebildet hatte, wurde eine kontrollierte Fibrinolyse (Streptokinase 1,5 Mega I.E. in 30 min initial, Fortführung mit 0,75 Mega I.E./h) eingeleitet. Wenn die hämostasiologischen Parameter eine effektive Lyse anzeigten, wurden die gleichen Gefäße erneut unterbunden. Auch jetzt änderten sich die hämodynamischen Größen nicht wesentlich. Die ST-Segment-Elevation war jedoch signifikant um 50% geringer als nach einfacher Ligatur. Eine Kontrollgruppe, die nur das Lösungsmittel der Streptokinase erhielt, zeigte dagegen einen unveränderten Anstieg der ST-Streckenelevation. Die Wirkung der Streptokinase wird auf das Auftreten von Fibrinogen-Spaltprodukten und die Senkung des Fibrinogenspiegels zurückgeführt, die eine Verbesserung der Mikrozirkulation hervorrufen.
    Notes: Summary In 17 unaesthetized dogs several side branches of the left descending coronary artery were ligated. The ST-segment elevation in the epicardial ECG ascended to 22 mV after 5 min and to 19 mV after 20 min. Aortic pressure, left ventricular enddiastolic pressure, heart rate and hemostasiological parameters (thrombin-time, thrombin-coagulase-time, reptilase-time, plasma-fibrinogen, staphylococcal clumping test) did not change significantly. 20 min after the beginning of coronary occlusion, the vessels were reopened. When ST-segment elevation had disappeared, a controlled fibrinolytic therapy (Streptokinase 1.5 Mega I.E. in 30 min, later on 0.75 Mega I.E./h) was induced. When an effective fibrinolysis could be demonstrated by the hemostasiological parameters, the same vessels were occluded again. Now the hemodynamic parameters too did not change significantly, but the ST-segment elevation was significantly diminished for more than 50% compared with simple ligation. A control group, which only got the solvent of the streptokinase, showed the same ST-segment elevation. This effect, induced by streptokinase is ascribed to fibrinogen degradation products and a diminution in the amount of fibrinogen which cause an improvement of microcirculation.

Das klinische Bild war geprägt von symmetrisch-paretischen Ausfällen mit dissoziierten Empfindungsstörungen. Histologisch fanden sich Amyloidablagerungen im peripheren Nerven. Der Fall wird mit den wenigen gleichgelagerten Fällen in der Literatur verglichen, und die sich daraus ergebenden Konsequenzen für die Überlegungen zur Pathogenese werden diskutiert.
    Notes: Summary A case with peripheral neuropathy due to monoclonal gammopathy is reported. There was striking, dissociated symmetrical sensory loss and paresis, the legs being affected predominantly. The case is compared with the few reported cases of myelomatous neuropathy, and the clinical and histological findings are discussed in view of the pathogenesis.
