    Description / Table of Contents: Abstract Objective: The aim of this study was to compare low dose dopamine and dopexamine with respect to of liver-venous oxygen saturation, oxygen delivery and – demand, liver function tests and cardiocirculatory effects in the reperfusion period during a hemihepatectomy operation with occlusion of the liver hilus. Methods: Twenty patients were studied in a randomised, doubleblind setting. They either received 2 µg/kg per min dopamine or 0.5 µg/kg per min dopexamine perioperatively. For monitoring purposes a pulmonary artery and a liver venous catheter were placed. At four different time points hemodynamic parameter were assessed and blood samples were drawn. Results: Significant changes between groups were found 5 min after opening the liver hilus for the cardiac index and the systemic oxygen delivery, as well as at the end of the operation for pulmonary shunt volume, which had increased more in the dopexamine group. No significant difference between liver venous oxygen saturation and liver function tests was found. Conclusion: Until more detailed studies concerning the influence of dopamine on the hepatic-splanchnic region during liver surgery are performed, dopexamine can not be considered superior to dopamine during these operations.
    Notes: Zusammenfassung Fragestellung: Gibt es einen Unterschied bezüglich der leber-venösen Sauerstoffsättigung, des Sauerstoffangebots und -verbrauchs, der Leberfunktion sowie der kardiozirkulatorischen Effekte während der Reperfusion zwischen niedrig dosiertem Dopamin und Dopexamin bei Leberteilresektionen mit Leberhilusokklusion? Methodik: 20 Patienten wurden randomisiert, doppelblind in zwei Gruppen eingeteilt und erhielten entweder 2 µg/kg/min Dopamin oder 0,5 µg/kg/min Dopexamin. Für das perioperative Monitoring wurden ein pulmonalarterieller- und ein Lebervenenkatheter gelegt. Zu vier Meßzeitpunkten wurden Parameter der Hämodynamik erhoben und Blut abgenommen. Ergebnisse: Ein signifikanter Unterschied zwischen den Gruppen bestand 5 min nach Eröffnung des Leberhilus in einem stärker angestiegenen Cardiacindex und systemischen Sauerstoffangebot sowie am OP-En-de in einem stärker angestiegenen pulmonalen Shuntvolumen in der Dopexamingruppe. Es gab keinen Unterschied bezüglich der leber-venösen Sauerstoffsättigung und der Leberfunktionsparameter. Schlußfolgerung: Bis weiterführende Untersuchungen mit differenzierter Betrachtung der Wirkung von Dopexamin bei leber-chirurgischen Eingriffen vorliegen, ist Dopexamin bei diesen Operationen gegenüber Dopamin nicht als überlegen zu betrachten.

    Notes: Abstract In ten piglets (body weight 8.2–11.6 kg), acid base, electrolyte and anion gap changes were investigated during almost total plasma replacement with hydroxyethyl starch (HES) and modified fluid gelatin (GEL) in saline solution using a cell saver autotransfusion technique. During the study, there were only moderate acid base changes, but marked disturbances in anion balance. At study end, the mean chloride concentration was significantly higher (mmol/l: normal values 97–108, HES 116 ± 1.5, GEL 108 ± 1.1, p 〈 0.01) and the mean anion gap was significantly lower in the HES group in comparison to the GEL group (mmol/l: normal values 5–14, HES 3 ± 1.7, GEL 11.9 ± 0.9, p 〈 0.01). It is concluded that plasma replacement with electroneutral HES, but not with negatively charged GEL, can lower the anion gap irrespective of the underlying disease. This can be misleading when the anion gap is used for differential diagnosis of metabolic acidosis in patients after large volume infusion of synthetic colloids.

    Notes: Abstract A method is presented for calculating the absorption spectrum, the film thickness and the index of refraction from IR reflection spectra. Considering real test conditions (partial incoherence, scattering) a routinely usable instrument was created with this direct calculation of the absorption spectrum, which provides a very good correspondence between the measurement and simulation also without any initial information about the expected spectrum. This method is applied to characterize hard coatings deposited on iron substrates by glow discharge assisted CVD processes with organic silicon and boron compounds as precursors. In addition to the qualitative microstructural characterization of these coatings, this method is suitable for a rapid, exact and non-destructive determination of the film thickness and the index of refraction.

    Notes: Abstract Reinforcement effects in composites are widely influenced by fibre coatings. A detailed understanding of their microstructure and chemical composition is of great interest. Boron nitride films were deposited continuously on fibre rovings of various ceramics in CVD reactors of vertical as well as horizontal position. XPS depth profilings show that the film compositions are close to stoichiometric BN with carbon and oxygen impurities in the range of 10 at%. Cross-sections of separated fibres were investigated by HREM and TEM diffraction. All BN films are hexagonal turbostratic. The (002) layers with an increased distance (about 0.36 nm) showed a mean stacking sequence near to graphite and a characteristic orientation to the fibre in the interface region. We assume the gas flow type and hence the exchange rate of matter and energy determines the film structure in this region. With increasing film thickness the (002) layers fold randomly in all directions or form nanocrystals at elevated temperatures.

    Notes: Abstract A plasma-electrochemical synthesis was used to prepare chromium and sodium tantalate layers. These layers were deposited on a tantalum anode surface as ceramic compounds from aqueous electrolytes. The typical pore structure morphology of the tantalate layer was characterised by SEM as well as fractures which provide evidence of an intimate contact between layers and substrate. An XRD-study showed that the layers are composed of a mixture of Ta2O5 and either NaTaO3 or CrTaO4 depending on the electrolyte composition. Quantitative characterisation by EPM indicated higher chromium and tantalum concentrations on the electrolyte/layer interface than at the layer/tantalum interface. The chemical state of tantalum was investigated by means of XPS.

    Notes: Abstract The extent of complement and contact activation is related to outcome in sepsis. A low functional index of their main blocker C1-esterase inhibitor (C1-INH) is considered as a relative deficiency of C1-INH and might contribute to the development of fatal complications in the intensive care unit. The first results of therapeutic intervention with C1-INH concentrate in septic shock are promising. We report on our experience of C1-INH concentrate administration in a young woman with Caroli's disease as ultimate rescue therapy for septic shock with capillary leakage syndrome after combined liver and kidney transplantation. No focus of infection was detectable and thus surgical intervention was not indicated. Antibiotic therapy at that time included vancomycin, tobramycin, meropenem and fluconazol. Hemodynamic stabilization occurred within hours after administration of C1-INH concentrate. Simultaneously a reduction in vasopressor medication was possible and negative fluid balance was achieved.

    Notes: Abstract The aim of the present study was to characterize the surface morphology, microstructure and the chemical composition of anodic spark-converted titanium surfaces. The coatings were prepared in an electrochemical cell by the anodic spark deposition technique in an aqueous solution of CaH2PO4)2. The coatings were characterized by X-ray photoelectron spectroscopy (XPS), scanning electron microscopy (SEM), electron probe microanalysis (EPMA) and X-ray diffraction (XRD). The properties of the coatings are described in terms of morphology.
