    Notes: To diagnose allergic or irritant contact dermatitis, a clinically relevant contact allergy has to be demonstrated or ruled out, respectively. Although patch testing has been used for 100 years, it remains the method of choice for diagnosing contact allergy. A disadvantage of patch testing is that reading is subjective, based on inspection and palpation of the test area, implying that the assessment is subject to the reader's knowledge and experience. This study was carried out to investigate the accordance in reading patch test reactions between 5 dermatologists. 4 groups, each with 10 patients, participated. Within each group, all 10 were allergic to one and the same sensitizer; nickel, epoxy resin, ethylenediamine, or Kathon CG. The sensitizers were tested in serial dilutions and applied randomly to the back. The tests were read independently by the dermatologists in a blinded fashion. A protocol was used where the dermatologists had to note the presence of the morphological features erythema, infiltration, papules, vesicles, and bullae. In this way, it was possible to allocate the various readings into 4 classification systems, 3 European and one American, although the definition of the various classifications might differ slightly. Based on the readings and classifications, it was possible to calculate the degree of accordance within the 4 systems used. It was also possible to analyze the degree of accordance for the various morphological features. Total accordance for the 5 reading dermatologists for positive and negative test reactions was noted in 36% and 46%, respectively. The 3 European systems require homogenous erythema and infiltration for a reaction to be classified as allergic, while the American system also requires the presence of papules. Therefore, a higher number of allergic reactions (9%) was obtained with the European systems. Between the 4 classification systems, there were statistically significant differences, with the highest number of deviations for the system in which papules had discriminatory significance. When looking at the various morphological features, papules were most difficult to assess, followed by vesicles. Preferably, a classification system should not depend on papules. In conclusion, there was good accordance among the dermatologists in reading patch test reactions, since the 5 dermatologists read 82% of all tests similarly.

    Notes: In a double blind experimental study, 20 patients with a contact allergy to gold sodium thiosulphate were challenged intramuscularly with the chemically similar gold sodium thiomalate and with placebo. The most spectacular clinical reaction in the 10 patients given the active agent, was an epidermal and dermal flare up of healed patch-test reactions to the gold salts, as well as a high, but transient, rise in body temperature. Previous intradermal tests were similarly reactivated. In addition, toxicoderma-like rashes were observed in several cases, but a flare up of a previous contact dermatitis site was seen in one patient only. The specificity of the positive provocations was demonstrated.

    Notes: ABSTRACT Alois Alzheimer published two papers on the disease which was named after him by Emil Kraepelin in 1910. Each of these papers contains clinical and pathological data on a patient Alzheimer had seen at the hospital. We have previously reported on the rediscovery of tissue sections from Alzheimer's second published case of Alzheimer disease, Johann F., which probably gave the disease its name (Neurogenetics 1997; 1 : 73–80). Here, we describe the histopathology and APOE genotype of Alois Alzheimer's first patient, Auguste D. As in the case of Johann F., a large number of tissue sections belonging to Alzheimer's laboratory, which was later headed by Spielmeyer, were found among material kept at the Institute of Neuropathology of the University of Munich. As described by Alzheimer in his original report (Allg Zeitschr Psychiatr 1907; 64 : 146–148), there were numerous neurofibrillary tangles and many amyloid plaques, especially in the upper cortical layers of this patient. Yet, there was no microscopic evidence for vascular, i.e., arteriosclerotic, lesions. Interestingly, Alzheimer's histological preparations did not include the hippocampus or entorhinal region. The APOE genotype of this patient was shown to be ε3/ε3 by PCR-based restriction enzyme analysis, indicating that mutational screening of the tissue is feasible. The historical importance of the case of Auguste D. lies in the fact that it marks the beginning of research into Alzheimer disease. In addition, neurofibrillary tangles were first described in this brain.

    Notes: Summary. The eigenproblem method calculates the solutions of systems of polynomial equations $ f_1(x_1, \ldots , x_s)=0,\ldots,f_m(x_1, \ldots , x_s)=0$ . It consists in fixing a suitable polynomial $ f $ and in considering the matrix $ A_f $ corresponding to the mapping $ [p] \mapsto [f\cdot p] $ where the equivalence classes are modulo the ideal generated by $ f_1, \ldots , f_m.$ The eigenspaces contain vectors, from which all solutions of the system can be read off. This access was investigated in [1] and [16] mainly for the case that $ A_f is nonderogatory. In the present paper, we study the case where $ f_1, \ldots , f_m $ have multiple zeros in common. We establish a kind of Jordan decomposition of $ A_f $ reflecting the multiplicity structure, and describe the conditions under which $ A_f $ is nonderogatory. The algorithmic analysis of the eigenproblem in the general case is indicated.

    Notes: Zusammenfassung In der vorliegenden Arbeit wurden 296 Fälle von gonorrhoischer Mon- und Polyarthritis nach verschiedenen statistischen Gesichtspunkten ausgewertet. Dabei zeigte sich in jeder Hinsicht, daß von einer bestimmten Gesetzmäßigkeit der gonorrhoischen Arthritis keine Rede sein kann; es bestehen keinerlei Zusammenhänge zwischen Beruf, Lokalisation, Lebensalter, Jahreszeit, Inkubation der Gonorrhöe und den verschiedenen Arthritiden. Lediglich eine Tatsache, die im übrigen schon in der Literatur niedergelegt ist, konnte auch an unserem Material bestätigt werden, das ist die prozentual stärkste Beteiligung der Kniegelenke bei den gonorrhoischen Gelenkentzündungen überhaupt. Ein Grund dafür läßt sich nicht ohne weiteres eruieren. Im übrigen sind die Bedingungen, die zum Entstehen einer gonorrhoischen Arthritis notwendig sind, noch durchaus unklar; Traumen und dauernde Gelenkanstrengungen sind nicht ohne weiteres dazu in Beziehung zu setzen. Wir müssen auch hier eben immer noch auf die Disposition zurückgreifen, ohne damit etwas Bestimmtes anführen zu können. Jedenfalls ist das eine sicher, daß die gonorrhoische Arthritis im Gegensatz zu manchen Angaben auch der neueren Literatur nichtüberwiegend monartikulär auftritt, im Gegensatz zu manchen anderen Angaben tritt sie aber auch nichtüberwiegend polyartikulär auf. Einem Befallensein von einem Gelenk in zwei Drittel aller Fälle steht bei unserem Material einem Befallensein von mehreren Gelenken in einem Drittel der Fälle gegenüber, eine Zahl, die sich mit verschiedenen anderen Literaturangaben deckt.

    Description / Table of Contents: Summary It has known for a long time that Sydenham’s chorea and tics, as seen in Gilles de la Tourette’s syndrome (GTS), are phenomenologically very similar. Tics may occur as symptoms of acute Sydenham’s chorea or persist over years as residual symptoms. Investigations of children suffering from GTS, including obsessive-compusilve symptoms, have provided signs of a poststreptococcal autoimmune process but also shown that treatment based on immunological interventions has been effective. We treated a 14-year-old boy showing all diagnostic criteria of GTS, familial susceptibility, and an increase in the antibody titer of streptococcal antigens with 7S immunoglobulins i.v. over 5 days. Response to this therapy was good regarding motor tics, vocal tics, and behavioral symptoms such as disturbed impulse control which still persisted after 9 months. These findings and the successful therapy underline reports of the literature and point to a pathogenetic mechanism of an immunologically triggered disturbance of the striatal dopaminergic system, at least in a subgroup of GTS.
    Notes: Zusammenfassung Es ist seit langem bekannt, daß Chorea Sydenham und Tics, wie sie im Rahmen eines Gilles-de-la-Tourette-Syndroms (GTS) auftreten, phänomenologische Ähnlichkeiten haben. Tics können sowohl als Symptome einer Chorea Sydenham auftreten als auch als Residualsymptomatik über Jahre persistieren. Untersuchungen aus den letzten Jahren zeigen, daß bei Kindern mit GTS nicht nur häufig Zeichen einer Poststreptokokkenerkrankung betehen, sondern die Symptomatik sich auch nach entsprechenden Therapien deutlich besserte. Wir behandelten einen 14jährigen Jungen, der alle diagnostischen Kriterien für ein GTS sowie eine genetische Belastung aufwies und eine deutliche Erhöhung des Antikörpertiters gegen Streptokokkenantigene zeigte, über 5 Tage mit 7S-Immunoglobulinen i.v. Hierunter zeigte sich auch im Langzeitverlauf eine deutliche Besserung der vorher ausgeprägten motorischen Tics und Vokaltics sowie von Verhaltensauffälligkeiten wie gestörter Impulskontrolle. Diese Befunde und die erfolgreiche Therapie unterstützen entsprechende Vorberichte und weisen, zumindest bei einigen Einzelfällen mit GTS, auf den pathogenetischen Mechanismus einer immunologisch getriggerten Störung des striatalen Dopaminsystems hin.

    Description / Table of Contents: Summary Dysmorphophobia as a new psychiatric syndrome was described by the italian psychiatrist Morselli in 1891. Dysmorphophobic symptoms are associated with several other psychiatric disorders. The term of dysmorphophobia will be discussed along a theoretical review. We present the case of a 20-year-old patient with dysmorphophobic symptoms. In the scope of ICD 10, DSM-III-R/DSM IV diagnostical criteria we outline problems of the diagnostical classification of dysmorphophobic signs.
    Notes: Zusammenfassung Die von Morselli 1891 als neues klinisches Syndrom beschriebene Dysmorphophobie findet sich bei verschiedenen psychiatrischen Krankheitsbildern. Anhand einer theoretischen Übersicht wird der Begriff der Dysmorphophobie skizziert. Dysmorphophobe Symptome bei verschiedenen psychiatrischen Erkrankungen werden erläutert. Anhand einer kasuistischen Darstellung werden Probleme der diagnostischen Zuordnung dysmorphophober Symptome in den modernen Klassifikationssystemen diskutiert.
