    Notizen: : A modeling framework was developed to determine phosphorus loadings to Lake Okeechobee from watersheds located north of the lake. This framework consists of the land-based model CREAMS-WT, the in-stream transport model QUAL2E, and an interface procedure to format the land-based model output for use by the in-stream model. QUAL2E hydraulics and water quality routines were modified to account for flow routing and phosphorus retention in both wetlands and stream channels. Phosphorus loadings obtained from previous applications of CREAMS-WT were used by QUAL2E, and calibration and verification showed that QUAL2E accurately simulated seasonal and annual phosphorus loadings from a watershed. Sensitivity and uncertainty analyses indicated that the accuracy of monthly loadings can be improved by using better estimates of in-stream phosphorus decay rates, ground water phosphorus concentrations, and runoff phosphorus concentrations as input to QUAL2E.

    Beschreibung / Inhaltsverzeichnis: Résumé La scintigraphie avec des substances radio-isotopiques est d'usage courant comme examen de dépistage avant d'envisager des procédures plus invasives dans l'établissement du diagnostic de patients souffrant d'hémorragies gastro-intestinales basses (GI). Nous avons recontrôlé l'expérience que nous avons acquise dans notre institution avec des scintigraphies, à l'aide d'hématies marquées au Technetium (TRCS) dans le dépistage et la localisation des sources d'hémorragies gastro-intestinales basses. Cette étude porte sur un groupe de 72 patients qui ont subi 80 scintigraphies aux hématies marquées sur une période de 5 ans. Trente-huit scans étaient positifs (47,5%) et 42 négatifs (52,5%). Le lieu de l'hémorragie gastro-intestinale basse a été confirmé par endoscopie, artériographie, chirurgie et/ou pathologie dans 22 des 38 cas avec scintigraphie positive. Quatre scintigraphies étaient faussement négatives (9,5%). La sensibilité et la spécificité globale pour TRCS dans le dépistage d'hémorragies gastrointestinales basses et de 84,6% (22/26) et 70,4% (38/54). L'exactitude de la localisation des points de saignemenet chez nos patients dont la scintigraphie était positive est de 72,7% (16/22). Trente patients ont subi des artériographies mésentériques dans notre série, 11 artériographies ont été réalisées après des TRCS négatives: une s'est révélée positive. La scintigraphie aux hématies marquées au Technetium semble être un moyen utile de dépistage de patients hémodynamiquement stables présentant une hémorragie gastrointestinale basse. Cet examen permet d'éviter la morbidité potentielle d'une artériographie chez des sujets qui ne présentent pas un saignement actif.
    Notizen: Abstract Radionuclide scintigraphy is commonly utilized as a screening examination before performing more invasive procedures in the work-up of patients with lower gastrointestinal (GI) bleeding. We reviewed our institutional experience with technetium-labelled red blood cell scintigraphy (TRCS) in detecting and localising acute lower GI bleeding. The study group included 72 patients who had 80 red cell scans over a five year period. Thirty-eight scans were positive (47.5%), and 42 were negative (52.5%). Sites of lower GI bleeding were confirmed by endoscopy, arteriography, surgery and/or pathology in 22 of the 38 positive scans. There were four false-negative scans (9.5%). The overall sensitivity and specificity of TRCS in detecting lower GI bleeding was 84.6% (22/26) and 70.4% (38/54), respectively. The accuracy of localization of bleeding sites in the patients with confirmed positive scans was 72.7% (16/22). Thirty mesenteric arteriograms were performed on patients in this series. Eleven arteriograms were performed after negative TRCS; one was positive. Technetium-labelled red blood cell scintigraphy appears to be a useful screening examination for patients with lower GI bleeding who are hemodynamically stable. This may avoid the potential morbidity of arteriography in patients who are not actively bleeding.

    Beschreibung / Inhaltsverzeichnis: Durch das steigende Informationsangebot und einen relativ leichten Einstieg finden immer mehr Ophthalmologen Zugang zum Internet. Ein jederzeit und jederorts abrufbarer ophthalmologischer Befundbildatlas mit zugehörigem ICD-Schlüssel im World Wide Web bietet verschiedene Möglichkeiten, wie z.B. Aufruf eines typischen Befundbildes zum differentialdiagnostischen Vergleich, zur eigenen Weiterbildung oder Verwendung für den eigenen Bedarf (u.a. Präsentationen). Bis jetzt wurden bereits über 500 typische Befundbilder ausgewählt, digitalisiert und in eine Datenbank integriert, die über 4700 ICD-standardisierte und -codierte Diagnosen enthält. Bildbezogene Diagnosen und die Bilder werden mit dieser Datenbank verknüpft. Nach Eingabe eines diagnostischen Suchbegriffs bewirken spezielle Programmskripte die Datenbanksuche und die Ausgabe der Suchergebnisse in automatisch erstellten Web-Seiten. Die gefundenen Bilder werden zur Minimierung des Datentransfers zunächst in Kleinformat (Thumbnails) ausgegeben und sind auf Mausklick im Vollformat zu erhalten. Neben Diagnose und ICD-Schlüssel-Nummer werden zu jedem Bild der Autor, ein diagnostischer Hinweis (Hausdiagnose) und ein Kommentar bzw. Kommentarmöglichkeit angeboten. Der von uns entwickelte Gießener Ophthalmologische Bildatlas ist unter der Internetadresse www.med.uni-giessen.de [Überlokale Infosysteme] abrufbar. Er ermöglicht einen gezielten, raschen und kostenlosen Diagnose- und Befundbildabruf mit zugehörigem ICD-Schlüssel und stellt eine sinnvolle Ergänzung zum Studium von gedruckten ophthalmologischen Bildatlanten dar.
    Notizen: The Internet has become a powerful, international source for information, and it has shown an exponential growth because of the ease of access and the immediate availability of information. We introduced a new ophthalmological atlas including ICD coding on the World Wide Web. So far, more than 500 lantern slides of typical and interesting ophthalmological findings have been selected and digitized. These pictures were integrated in a data base, which was arranged for ease and speed of search and retrieval. In its background, the data base contains a list of over 4700 ICD-encoded diagnoses to which the picture-documented findings are linked. Comments on pictures can be added by the author or by users. The data base contains several lists, such as a list of ICD codes and diagnoses, a list of all pictures with corresponding diagnoses, a list of all diagnoses and number of picture, a list of those diagnoses for which the corresponding picture is available, as well as a list of comments on each picture. Special program scripts handle the user's search key words for diagnoses and extract the required information out of the data base, using Windows NT. Search results are presented on an automatically built-up webpage. To provide fast speed of search all pictures initially are shown in a small format (thumbnails) with little amount of data. The related full-size picture is retrieved by a single mouse click. Moreover, the name and institution of the author, diagnostical hints and comments on pictures by the author or by users are offered for each diagnosis available. The Giessen Ophthalmological Picture Atlas can be reached under www.med.uni-giessen.de in the Internet. It allows a fast search free of charge from all over the world and, therefore, offers an additional option to obtain specific ophthalmological information for various purposes.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung Zwischen Januar 1993 und Dezember 1995 haben wir bei 109 Patienten (Altersmedian 75 Jahre) insgesamt 112 extraartikuläre Frakturen des proximalen Femurendes einschließlich Kombinationsfrakturen Schaft – Trochanterregion in Serie mit unterschiedlichen „Gleitschrauben-Nagel-Implantaten“ [61 classic nail (IMHS), 51 γ -Nägel (GN)] versorgt. Beim Systemvergleich zeigten beide Implantatvarianten im Detail Vor- und Nachteile, erwiesen sich jedoch prinzipiell als gleichwertig. Komplikationen in Form von sekundärer Schenkelhalsvarisierung mit „cut-out“ (1,8 %) und ohne „cut-out“ (1,8 %) der Gleitschraube, 2 intraoperativ erzeugten und konservativ behandelten Schaftfissuren (1,8 %), einer Nagelperforation des Femur (0,9 %) sowie 3 Infekten (2,7 %) machten insgesamt 5 Reoperationen (4,5 %) nötig, wobei keine dieser Komplikationen dem Prinzip selbst oder den Implantatvarianten anzulasten ist. Alle Patienten wurden mindestens bis zum Ende des 1. postoperativen Jahres weiterverfolgt, 59 % der Patienten erreichten ihre Ausgangsmobilität wieder. IMHS und GN sind ausgezeichnete und gleichwertige Implantate, die den biomechanischen Anforderungen der obengenannten Frakturformen genügen.
    Notizen: Summary Between January 1993 and December 1995 we treated 109 patients (median age: 75 years) with 112 extraarticular hip fractures including combined trochanteric and shaft fractures using two different “sliding-screw-nail implants” (intramedullary hip screw = classic nail: n = 61; gamma nail: n = 51). Comparing the two systems in detail certain advantages and disadvantages were seen, with both being equivalent. We encountered the following complications: secondary varus malalignment of the collum femoris with “cut out” of the sliding-screw (1.8 %) and without “cut out” (1.8 %), fissure of the femoral shaft occuring intraoperatively and being treated conservatively (1.8 %), femoral perforation by the nail (0.9 %), infection (2.7 %). Thus, 5 reoperations (4.5 %) were necessary. None of these complications were attributable to the principle itself or to the different implants used. Each patient was followed-up for a minimum of 12 months postoperatively. In 59 % of all patients the pre-trauma range of mobility could be fully restored. Intramedullary hip screw and gamma nail are excellent and equivalent systems, which fully satisfy the biomechanical needs of above mentioned fractures.

    Notizen: Abstract Increasing numbers of severely mentally ill individuals are being treated in nonhospital, community-based settings and public concern about potential violence by these individuals has increased, often as a result of tragic, albeit uncommon events. The present study examines potential predictors of serious violence among persons with severe mental illness (SMI), with a specific focus on the joint effect of substance abuse and medication noncompliance. Subjects in the study are involuntarily admitted inpatients with SMI awaiting a period of court-ordered outpatient treatment, termed “involuntary outpatient commitment”. During enrollment in a longitudinal outcome study of the effectiveness of OPC, 331 subjects and, whenever feasible, family members or other informants were interviewed. In addition, complementary data were gathered by review of involuntary commitment records and hospital records. Data collection included sociodemographic characteristics, illness history, clinical status, medication adherence, substance abuse and violent behavior during the 4 months preceding hospitalization. Descriptive and multivariable logistic regression procedures were used to examine the association between serious violent acts and a number of personal, social, and clinical characteristics. The combination of medication noncompliance and substance abuse was a significant predictor of serious violent acts in the community. Individuals who had problems with both alcohol and illicit drug abuse appear to be at greatest risk for violence. These results suggest that reducing violence risk among persons with SMI requires an aggressive approach to improving medication adherence in the context of integrated mental health and substance abuse treatment.

    Notizen: Abstract The need to better understand and manage risk of violent behavior among people with severe mental illness in community care settings is increasingly being recognized, as public-sector mental health systems face mandates to provide more cost-effective services in less restrictive environments. The potential for serious violence in a small proportion of severely mentally ill (SMI) individuals has emerged as a key factor that increases cost and limits continuity and normalization of community-based services for populations with psychiatric disabilities. A major challenge to developing better strategies for risk assessment and management in community care settings involves specifying complex interactions between psychiatric impairment and the conditions of social life – including the quality and frequency of contact with others at close quarters. This is a study of the determinants of violent behavior in a sample of 331 adults with severe mental disorders in community-based treatment. An interaction between severity of functional impairment and frequency of social contact was found to be significantly associated with risk of violence. Among respondents with Global Assessment of Functioning (GAF) scores in the lowest 20%, more frequent contact with family and friends was linked to a higher probability of violent events. However, among better functioning respondents, frequent social contact was associated with lower risk of violence and greater satisfaction with relationships. These findings suggest that, where violence risk is concerned, the most salient feature of psychiatric impairment is the impairment of social relationships –the ways in which disorders of thought and mood not only distort one's subjective appraisal of experience and threat, but impair the ability to relate meaningfully to others, to resolve conflict and derive necessary support from family and friends. Thus, social contact may be a mixed blessing for SMI individuals. For some, it signals a positive quality of life, but for others – particularly those with extreme psychiatric impairment – frequent contact may add to conflict, stress, and increased potential and opportunity for physical violence. The impact of psychiatric impairment on violent behavior cannot be known in isolation, but must be considered in a social context. Effective community-based strategies to anticipate and prevent violence in the lives of persons with severe mental illness must take into account such interactions between social and clinical variables.

    Notizen: Abstract A large-scale cultivation system for the mass cell production and extraction of the protozoon Tetrahymena thermophila has been developed on the basis of a low-cost complex nutrient medium. Cell growth and the production of extracellular proteases were investigated using a 15-l stirred-tank reactor and 13-l and 1500-l airlift reactors. Processes using defined and complex medium formulations were compared. After cell mass production by 1200 l cell suspension in the large airlift bioreactor, two different extraction methods, based on the use of an extraction decanter and a sedimentation procedure, were compared and followed by cell lyophilization. Cell sedimentation was shown to be the more efficient extraction method as it enabled cell retention/separation while preserving the cell structure. Maximum cell growth was achieved in the stirred-tank bioreactor, supporting the hypothesis that higher shear forces reduce the particle size of the medium, which is responsible for an optimized nutrient supply. The highest glucose uptake rates were found in defined medium lacking the nutrient particles that are present in complex medium formulations. The cell-specific proteolytic activity in culture supernatants of airlift bioreactors using complex medium conditions was higher than that of a culture broth with cells grown under defined medium formulations.

    Notizen: Summary Islet cell antigen (ICA) 69 is a newly-recognized islet cell antigen to which autoantibodies have been observed in prediabetic relatives of patients with insulin-dependent-diabetes mellitus (IDDM). Here we extend the earlier analysis of ICA 69 antibodies to patients with recent-onset IDDM and to patients with other immune-mediated diseases. ICA 69 antibodies were determined by Western blot using an affinity purified recombinant fusion protein of ICA 69 and maltose binding protein. ICA 69 antibody quantities were determined as titres using a titration curve of a standard serum as reference. Mean logarithmic ICA 69 antibody titres were 3.4 (±1.4) in 99 patients with acute IDDM compared to 2.8 (±0.9) in 49 healthy blood donors (p〈0.001). A higher mean ICA 69 antibody titre of 4.1 (±0.8) was observed in 16 patients with rheumatoid arthritis in comparison to acute IDDM (p〈0.01) and healthy control subjects (p〈0.001). The percentage of sera with ICA 69 antibody titres above the 2 SD level of normal subjects was 21% in IDDM, 31% in rheumatoid arthritis and 6% in healthy blood donors. None of the patients with autoimmune thyroid disease (n=20), inflammatory bowel disease (n=9) or multiple sclerosis (n=7) had elevated ICA 69 antibodies. In IDDM, presence of ICA 69 antibodies persisted and the titre remained the same over 18 months of follow-up. The relationship of ICA 69 antibodies to islet cell antibodies (ICA) or insulin autoantibodies (IAA) was tested. The production of ICA 69 antibodies was not associated in diabetic patients with the presence of any of the two other autoantibodies. In conclusion, this study describes ICA 69 antibodies in acute IDDM and finds them to be independent of other islet autoantibodies. In addition ICA 69 is a target of humoural autoimmunity not only in IDDM but also in rheumatoid arthritis.
