    Notes: To investigate respiratory symptoms, increased bronchial responsiveness, and signs of airway inflammation in elite swimmers, we examined 29 swimmers from the Finnish national team and 19 healthy control subjects (nonasthmatic, symptom-free). They answered a questionnaire and were interviewed for respiratory symptoms. Lung volumes were measured and bronchial responsiveness assessed by a histamine challenge test. Induced sputum samples were also collected. Fourteen (48%) of the swimmers and three (16%) of the control subjects showed increased bronchial responsiveness (P〈0.05). The sputum cell differential counts of eosinophils (mean 2.7% vs 0.2%) and neutrophils (54.7% V5 29.9%) from swimmers were significantly higher than those from controls (P〈0.01). Eosinophilia (sputum differential eosinophil count of 〉4%) was observed in six (21%) of the swimmers and in none of the controls (P〈0.05). Symptomatic swimmers had significantly more sputum eosinophils than did the symptom-free. The concentrations of sputum eosinophil peroxidase (EPO) and human neutrophil lipocalin (HNL) were significantly higher in swimmers than control subjects (P〈0.001 and P=0.05). We conclude that elite swimmers had significantly more often increased tjronchial responsiveness than control subjects. Sputum from swimmers contained a higher percentage of eosinophils and neutrophils, and higher concentrations of EPO and HNL than sputum from controls. Long-term and repeated exposure to chlorine compounds in swimming pools during training and competition may contribute to the increased occurrence of bronchial hyperresponsiveness and airway inflammation in swimmers.

    Notes: Recently, increased serum levels of eosinophil cationic protein (ECP) in cystic fibrosis (CF) have been reported which were closely related to the levels in sputum. In the present study we investigated other eosinophil proteins such as eosinophil peroxidase (EPO) and eosinophil protein X (EPX) in sera of patients with CF and their relation to pulmonary function.〈section xml:id="abs1-2"〉〈title type="main"〉MethodsSerum samples from 42 patients with CF and from 25 healthy control subjects were measured for ECP, EPO and EPX. Lung function tests were performed by using whole body plethysmographic technique, and the results were correlated with the levels of eosinophil granule proteins.〈section xml:id="abs1-3"〉〈title type="main"〉ResultsSerum ECP (median: 20.9 μg/L), EPO (median: 30.3 μg/L) and EPX (median: 37.9 μg/L) levels were significantly increased in CF compared with healthy controls (3.5 μg/L, P 〈 0.0001, 5.6 μg/L, P 〈 0.0001 and 14.6 μg/L, P 〈 0.0001, respectively) whereas eosinophil counts were not different. There was a strong correlation between the levels of eosinophil proteins and variables of pulmonary function, like between ECP and forced vital capacity (r = − 0.764, P 〈 0.0001). In addition, ECP concentrations were significantly related to the levels of EPO and EPX, albeit, in some patients with low ECP levels, increased EPO and EPX concentrations were observed.〈section xml:id="abs1-4"〉〈title type="main"〉ConclusionThese results indicate that in patients with CF, ECP, EPO and EPX concentrations also were increased with a significant relationship between these three eosinophil proteins. Since eosinophil activity in patients with CF is strongly correlated with pulmonary function, the assessment of eosinophil granule proteins might be useful for clinical monitoring in CF.

    Notes: Background: Harlier in vitro studies have suggested that the eosinophil may release its granule proteins selectively depending on the stimulus to which the cell is exposed.Objective: The object of the present study was to study the question of selective release in vivo by means of serum measurements of the two eosinophil granule proteins eosinophil cationic protein (ECP) and eosinophil peroxidase (EPO) in acute infections.Methods: Fourty-six subjects with acute infections were studied before treatment, 20 with bacterial infections and 26 with viral infections. Serum ECP, EPO and MPO were measured by specific RIA.Results: In acute bacterial infections ECP, but not EPO. was significantly raised in serum (P 〈 0.0001) compared with non-infected healthy subjects. In acute bacterial infections ECP was significantly correlated to the levels of the neutrophil marker myeloperoxidase (MPO) (rs= 0.96, P 〈 0.0001) but not to EPO. In acute viral infections neither ECP nor EPO were on average raised. However, almost 20% the patients had elevated levels of both proteins. In the viral infections the serum-levels of ECP and EPO were correlated (rs= 0.63, P 〈 0.001), but no correlation was found with MPOConclusion: It is concluded that eosinophils are activated during acute bacterial infections and that this activation results in the preferential mobilisation of ECP. The simultaneous assay of the two eosinophil proteins, ECP and EPO. may give new insight into the role of the eosinophil in disease.

    Notes: Background The eosinophil granulocyte takes part in allergic inflammatory diseases, like asthma and rhinitis. The aim of this study was to investigate the oxidative metabolism of the blood eosinophils and neutrophils from birch pollen allergic patients, during and after exposure to their allergen.Methods Thirteen birch pollen allergic patients, with seasonal symptoms of rhinitis, with or without asthma, were followed. The cells were purified using a percoll gradient and the MACS system. The eosinophil purity in all samples was 〉95%. The oxidative metabolism of both the PMN and the eosinophils was measured by means of a chemiluminescenee (CL) assay, with luminol or lucigenin as the amplifier, and using PMA (16nM) as the activator.Results In relative terms, the lucigenin CL by the eosinophils was 5-10-foId higher than that of the PMN, P= 0.002. The eosinophils of the birch pollen allergic patients produced less oxygen radicals during the season, compared to the reference group, measured both with luminol CL and lucigenin CL, P=0.01 and P= 0.02 respectively. Out of season, there was no difference. There was also no difference, during either period, between patients and references in PMN CL. Separating the eosinophils into hypodense and normodense fractions, showed a decreased oxidative metabolism by the hydrodense eosinophils.Conclusion We conclude that the eosinophils of birch pollen allergic patients have reduced production of oxygen radicals when the patients are exposed to their allergen, which could depend on higher amounts of hypodense eosinophils in the blood during season.

    Notes: Background The eosinophil granulocyte is an inflammatory cell that plays an active part in diseases such as asthma and rhinitis. This study aimed to investigate oxidative metabolism by blood eosinophils taken from allergic rhinitis patients, asthmatics, and nonallergic controls before and during the birch-pollen season. Methods Twenty patients with allergy to birch pollen and seasonal symptoms of rhinitis, some of whom were also asthmatic, were followed before and during the birch-pollen season in Sweden. The cells were purified using a Percoll gradient and the MACS system. Eosinophil purity in all samples was 〉95%. Oxidative metabolism was measured by a chemiluminescence (CL) assay, with luminol and lucigenin acting as enhancers, and PMA, serum-treated zymosan (STZ), interleukin (IL)-5. or RANTES as stimuli. Results The allergic subjects showed reduced luminol CL when activated before the season with PMA (P = 0.40) or STZ (P = 0.0055). This was not seen during pollen exposure. STZ-activated lucigenin CL was also reduced before the season (P = 0.0027). The reduction was most evident in the group with asymptomatic rhinitis. In terms of eosinophil stimulation. IL-5 and RANTES were equally effective in allergic and nonallergic subjects, both before and during the pollen season. Conclusions Blood eosinophils from asymptomatic allergies may have a lower capacity to produce oxygen-free radicals than eosinophils from nonallergics.

    Notes: Abstract. Objective and Design: The majority of patients with otitis media with effusion (OME) and atopy have been shown to have elevated levels of eosinophil cationic protein (ECP) in their middle ear fluid. The mechanism underlying these elevated levels of ECP is not clear. The purpose of this study was to investigate the feasibility of a quantitative determination of eosinophils and neutrophils in the middle ear lining by specific immunocytochemical markers, in order to study the extent of the involvement of these cells in patients with OME.¶Methods: Bilateral middle ear biopsies from five children with persistent OME and atopy confirmed by in vitro testing were evaluated for the presence of eosinophils and neutrophils with monoclonal antibodies against specific granule proteins. Five subjects who had no signs of effusion or infection but were undergoing routine tympanoplasty for dry perforations served as controls. The biopsies were embedded in a plastic resin to improve the structural preservation of the target cells and to increase the resolution in the light microscope. Dual markers were used to determine which marker was better for eosinophils and neutrophils, respectively. The following markers were used: eosinophil cationic protein (EG2), and eosinophil peroxidase (EPO) for eosinophils and myeloperoxidase (MPO) and human neutrophil lipocalin (HNL) for neutrophils.¶Results: Antibodies against EPO gave a more localized and intense staining than antibodies against EG2. Antibodies against HNL appear more specific to neutrophils than antibodies against MPO that also recognize monocytes. The number of cells was determined both in the tissue and in the mucus covering the epithelium. Eosinophils and neutrophils were present in the subepithelial connective tissue and in the mucus blanket in the middle ear of patients with OME in significantly higher number than in the control group. In general, there were more inflammatory cells in the mucus than in the tissue itself, but the number of inflammatory cells in the mucus showed a significant positive correlation with the number of inflammatory cells in the tissue. There was a significant positive correlation between the number of neutrophils and the number of eosinophils in the tissue as well as in the mucus, irrespective of which marker was used.¶Conclusion: The results of this study show the feasibility of using specific antibodies to identify eosinophils and neutrophils in the middle ear. The initial data suggest that atopic children with OME have higher numbers of such cells as compared to non-OME controls.

    Notes: Abstract Objectives: The aim of the present study was to examine the relationships between the ventilation rate and the type of ventilation system, on the one hand, and objective nasal measures, on the other. Methods: A standardized investigation, including acoustic rhinometry and nasal lavage, was performed in the school environment. All 279 school personnel working in the main buildings of 12 randomly selected primary schools in the municipality of Uppsala were invited, and 234 (84%) participated. The dimensions of the nasal cavity were measured with acoustic rhinometry. Eosinophil cationic protein (ECP), myeloperoxidase (MPO), lysozyme, and albumin were analyzed in the lavage fluid. The air exchange rate and the room temperature were measured in the classrooms. Relationships between nasal symptoms, nasal patency, and the concentration of biomarkers, on the one hand, and the type of ventilation system, the air exchange rate, and the temperature, on the other, were analyzed by both crude bivariate analysis and multiple regression models, controlling for the type of ventilation, the air exchange rate, room temperature, age, gender, smoking, atopy, and the urban vicinity of the school. Results: A lower degree of nasal patency as measured by acoustic rhinometry and increased levels of ECP and lysozyme in nasal lavage were associated with a lower air exchange rate in the schools. Although mechanically ventilated schools had higher air exchange rates, they were associated with more nasal symptoms, and nasal mucosal swelling and with increased lavage levels of ECP and lysozyme as compared with schools with natural ventilation only. In contrast, 12 subjects working in a school with mechanical displacement ventilation had more patent noses and lower levels of inflammatory markers as compared with the personnel in schools with natural ventilation only. Conclusion: Our results indicate that both a low air exchange rate and mechanical ventilation systems based on dilution can be associated with reduced nasal patency and an inflammatory biomarker response of the nasal mucosa among school personnel. The only school with sufficient ventilation according to the current Swedish recommendations had a displacement system and the fewest signs of nasal reactions among the personnel.

    Notes: Abstract Objectives: To study the relationships between dampness in concrete floors and building design on the one hand, and symptoms and medical signs of the eyes and nose in hospital workers, on the other. Methods: Four hospitals for geriatrics were selected to represent buildings with different ages and design, irrespective of symptom prevalence. The first building was built in 1925.The second, built in 1985, was known to have dampness in the floor. Conventional building techniques were used in the third building, built in 1993, and the last building was built in 1994, and was specially designed to include high ceilings, and minimal use of fluorescent lighting and interior plastic materials. The interior surfaces were painted with water-based beeswax glazing. All staff (n=95) working day shifts were invited to take part in a medical examination of the eyes and nose including acoustic rhinometry and nasal lavage, and a medical questionnaire, and 93% participated. Measurements of temperature, relative air humidity, air flow, illumination, volatile organic compounds (VOCs), molds, and bacteria were carried out in all buildings, together with measurements of formaldehyde, respirable dust, carbon monoxide (CO), carbon dioxide (CO2), nitrogen dioxide (NO2) and ozone. Statistical analyses were performed by bivariate analysis, and linear, ordinal, and logistic multiple regressions, adjusting for age, gender, tobacco smoking, atopy, and the perceived psychosocial work environment. Results: Dampness in the upper concrete floor surface (75–84%), ammonia under the floor [3 parts per million (ppm)], and 2-ethyl-1-hexanol in the air were detected in the two buildings built in 1985 and 1993. Increased occurrences of ocular and nasal symptoms, an increased concentration of lysozyme in nasal lavage, and decreased tear film stability were found in the subjects working in the damp buildings. Those in the specially designed building had fewer ocular and nasal symptoms, and increased tear film stability. All buildings had low levels of formaldehyde, molds, bacteria, ozone, and NO2. The lowest total concentration of VOCs, and the highest concentration of specific VOCs of microbial origin, were found in the building with special design. Conclusion: The study provides new evidence of the role of dampness-related alkaline degradation of di-(2-ethylhexyl) phthalate (DEHP) in polyvinyl chloride (PVC) building material. Emissions related to degradation of DEHP due to dampness in the floor, indicated by increased 2-ethyl-1-hexanol in the air, seem to increase both the secretion of lysozyme from the nasal mucosa and the occurrence of ocular and nasal symptoms. The indoor environment of the specially designed building with high ceilings and no fluorescent lighting or interior plastics seemed to have a positive influence on the nasal and ocular mucous membranes.

    Notes: Abstract Objective To investigate granulocyte activation, as well as hypoxanthine and free radical production in children during the first day after cardiopulmonary bypass.Design: A prospective study of pediatric patients undergoing either cardiac surgery with a cardiopulmonary bypass or thoracotomy and extracardiac vascular surgery not requiring a cardiopulmonary bypass. Setting Operative and intensive care units, Children's Hospital, University of Helsinki, Finland. Patients Seven consecutive patients undergoing elective correaction of a ventricular septal defect and six patients undergoing extracardiac surgery for ligation of a patent ductus arteriousus or repair a coarctation of the aorta. Measurements and main results Plasma concentrations of myeloperoxidase (140–334 μg/l preoperatively, 460–1692 μg/l at 0.2 h after declamping, 471–1386 μg/l at 0.5 h after declamping) and lactoferrin (77–258 μg/l preoperatively, 533–1783 at 0.2 h 404–1482 μg/l at 0.5 h) as markers of granulocyte activation, and hypoxanthine (0–5.7 μmol/l preoperatively, 4.3–17.0 μmol/l at 0.2 h, 6.5–17.9 μmol/l at 0.5 h) increased in a biphasic manner at 0.2–0.5 h and 6–10 h postoperatively (allp〈0.05). Expired ethane, as an index of free radical activity, increased at 10 h postoperatively (36–119 pmol/kg per min preoperatively, 72–152 pmol/kg per min,p〈0.05). Conclusion Granulocyte activation, and hypoxanthine and free radical production occur at least 10 h after cardiopulmonary bypass. In children undergoing open heart surgery, attempts to reduce free radical activity should be extended to the postoperative period.

    Notes: Abstract Introduction: There is a need to evaluate possible health effects of ventilation improvements and emissions from new buildings, in longitudinal studies. New methods to study biological effects on the eyes and upper airways are now available. Material and methods: A longitudinal study was performed on 83 trained social workers in two offices in Uppsala, Sweden. The exposed group (n= 57) moved to a newly redecorated building nearby. Low emitting building material had been used, including a new type of solvent-free water-based paint. The control group (n= 26) worked in the same office during the study period (November 1995 to February 1996). Hygiene management was carried out in both offices, at the beginning and the end of the investigation. Tear film stability (BUT) was measured. Nasal patency was measured by acoustic rhinometry, and eosinophilic cationic protein (ECP), myeloperoxidase (MPO), lysozyme and albumin were analyzed in nasal lavage fluid (NAL). Results: The relocation resulted in an increase in the personal outdoor airflow rate from 11 to 22 l/s. Indoor concentrations of terpenes were higher in the new building, and powdering of the new linoleum floor was observed. Measurements showed low levels of volatile organic compounds (VOC), formaldehyde, carbon dioxide (CO2), nitrogen dioxide, respirable dust, and microorganisms in the air of all buildings. The move resulted in an increased nasal patency and an increase of ECP and lysozyme in NAL, after adjusting for changes in the control group. No changes were observed for nasal or ocular symptoms. A seasonal effect, with a decrease of ECP, was observed in the control group. Conclusion: A well-ventilated office building can be redecorated without any major ocular or nasal effects, or measurable increase of indoor air pollution if low-emitting building materials are selected. In agreement with previous evidence, the improved ventilation flow may explain the increase of nasal patency. The increase of ECP and lysozyme in NAL suggested an inflammatory effect in the new building. Since this building had increased ventilation flow, increased concentrations of terpenes, and powdering from the polish on the new linoleum floor, identification of causative agents was difficult. The hygiene measures did not give any evidence that emissions from the new type of solvent-free water-based paints or building dampness were responsible for the observed nasal effects. Considering the higher emissions of VOC reported from older types of water-based latex paints and solvent-based wall paints, the new type of solvent-free water-based paint seems to be a good choice from the hygiene point of view.
