    Notes: Summary Eleven cases of aneurysm persisting after direct intracranial operation have been reported from a series of 315 aneurysms (274 cases) treated in our clinic. Postoperative angiography is necessary in every case because it reveals whether a clip or ligature is effective or not. An aneurysm remaining after direct operation disappears spontaneously if its neck has been narrowed by a clip or ligature. Reoperation must be considered if the clip has been placed on the aneurysmal body, if the clip has slipped off the aneurysm, or if the residual aneurysm is seen at follow-up angiography to be enlarging.

    Notes: Summary The clinical aspect and mechansims of production of ocular motor disturbances appearing as false localizing signs are discussed in four patients who were observed in a series of 102 consecutive admissions of patients with ruptured intracranial aneurysms. The aneurysms were located on the anterior communicating artery and the precommunicating segment of the right anterior cerebral artery. In these patients bilateral abducens nerve palsy, unilateral partial oculomotor nerve palsy and bilateral ophthalmoplegia totalis were observed. Nuclear or internuclear palsy due to vasospasm of the pontine branches of the basilar artery, peripheral oculomotor nerve palsy due to concomitant megadolichobasilar anomaly, and transtentorial axial descent of the midbrain were considered to be responsible.

    Description / Table of Contents: Sommaire Il a été énoncé qu'une méthode de pléthysmographie d'impédance dans les mesures de courants sanguins cérébraux était une aide puissante, mais l'origine du changement d'impédance n'a pas été théoriquement clarifié. L'impédance d'électrode dans une solution saline physiologique, dans du plasma sanguin, dans le sang et dans du subcortex cérébral du chien ont été étudiées en détail. Il a été trouvé que la pléthysmographie d'impédance cérébrale, dans laquelle des électrodes du genre aiguille sont généralement utilisées, ne reflète que très rarement le courant sanguin cérébral régional. Le changement d'impédance des électrodes dans le subcortex n'a montré que les mouvements mécaniques des électrodes qui ont été induits par des déplacements de cerveau se rapportant au cycle cardiaque ou à la respiration. Le débit d'écoulement sanguin dans les vaisseaux étroits peut être observé directement à mesure que l'impédance change, lorsque des électrodes minces sont insérées dans les vaisseaux.
    Abstract: Zusammenfassung Es ist ein Bericht über ein Verfahren der Impedanz Plethysmographie in Blutstrommessungen innerhalb des Gehirns als ein wirksames Mittel gegeben worden, aber der Ursprung der Impedanzänderung ist theoretisch nicht klargestellt worden. Es wurde die Elektrodenimpedanz in physiologischer Salzlösung, im Blutplasma, im Blut und im Subkortex eines Hundegehirns in Einzelheiten untersucht. Man fand, dass Impedanz Plethysmographie innerhalb des Gehirns, wobei allgemein nadelartige Elektroden benutzt werden, selten oder nie den regionalen Gehirnblutstrom reflektiert. Die Impedanzänderung der Elektroden in dem Subkortex zeigte nur die mechanischen Bewegungen der Elektroden an, die durch Gehirnverdrängungen hervorgerufen wurden, welche sich auf den Herzkreislauf oder auf die Atmung bezogen. Die Blutstromgeschwindigkeit kann in engen Gefässen direkt als Impedanzänderung beobachtet werden, wenn dünne Elektroden in die Gefässe gesteckt werden.
    Notes: Abstract A method of the impedance plethysmography in intracerebral blood flow measurements has been reported as a powerful tool, but the origin of the impedance change has not been clarified theoretically. The electrode impedance in physiological saline, in blood plasma, in blood and in cerebral subcortex of a dog were studied in detail. It was found that intracerebral impedance plethysmography, in which needle type electrodes are generally used, seldom if ever reflected the regional cerebral blood flow. The impedance change of the electrodes in the subcortex showed only the mechanical movements of the electrodes which were induced by brain displacements relating to the heart cycle, or to the respiration. The blood flow rate in narrow vessels may be observed directly as impedance changes, when fine electrodes are inserted into the vessels.
