Summary The clinical picture and the course of a IgG-plasmocytoma with medullary and extramedullary involvement in a 64 year old patient are presented. The course and treatment of the disease were dictated by the involvement of the pleura with exsudate formation by the plasmocytoma. Immunelectrophoretic findings and the results of lightmicroscopy of bone marrow aspiration and biopsy, of pleural exsudate, and tissues obtained at autopsy are discussed. Of particular interest is the fine structure of the plasmocytoma cells of bone marrow, peripheral blood, and of postmortem tissues. Lobulated nuclei, perinuclear fibrillar structures and other abnormalities were found. The difficulty of exact classification of neoplasms like the case demonstrated and the diagnostic possibilities of electron microscopy are discussed.

Zusammenfassung Klinik und Verlauf eines IgG-Plasmocytoms mit ossärer und tumorartiger extraossärer Ausbreitung bei einer 64jährigen Patientin werden beschrieben. Pleurabefall mit Exsudatbildung des Plasmocytoms bestimmten Verlauf und therapeutische Bemühungen wesentlich. Immunelektrophoretische Befunde sowie lichtmikroskopische Besonderheiten dieses Plasmocytoms in Sternalpunktat, Pleuraexsudat, Myelotomiepräparat und autoptisch gewonnenen Geweben werden dargestellt. Von besonderem Interesse sind die feinstrukturellen Befunde an den Plasmocytomzellen aus Sternalmark, peripherem Blut und postmortal gewonnenem Gewebe, vor allem die reichliche Ausbildung fibrillärer Strukturen in Nachbarschaft der gelappten und segmentierten Kerne. Die Problematik der genauen Einordnung derartiger Krankheitsbilder und der Rolle des Elektronenmikroskops hierbei werden diskutiert.
