    Notes: The magnetic properties of an isolated ferromagnetic bond of coupling constant K embedded in a layered spin-1/2 Heisenberg antiferromagnet with interplane coupling strength J⊥=αJ(parallel) (0≤α≤1.0) have been studied using linear-spin-wave (LSW) theory. In the pure lattice, the sublattice magnetization 〈Szi〉 is 0.304 at α=0 and saturates to 0.422 at α=1.0. The spin correlations 〈SxiSxj〉 decrease with increasing α within the plane and increase along the interplane direction. The introduction of an isolated ferromagnetic bond into the host lattice tends to enhance 〈Szi〉 at lattice sites joining the bond as K approaches zero. Increasing K will drastically suppress 〈SxiSxj〉 across the bond. Whereas, 〈SxiSxj〉 is enhanced between the neighboring sites close to the impurity link resulting in the suppression of 〈Szi〉. This behavior persists for all α values. The LSW theory breaks down at a certain K value. Increasing α, the critical value of K extends to a larger number.

    Notes: Summary In 14 schizophrenic patients, treated with neuroleptic drugs, and in 7 patients, treated with high-dosage diazepam,β-endorphin-like immunoreactivity in plasma has been measured by use of a highly sensitive and relatively specific radioimmunoassay. Neuroleptic treatment induced a significant increase ofβ-endorphin-like immunoreactivity (β-ELI). The pharmacological and clinical implications of this finding are discussed. High-dosage diazepam treatment induces a slight reduction of plasmaβ-ELI, a finding which is attributed to antistress effect of diazepam.

    Notes: Abstract. A randomized, double-blind multicenter trial was performed to compare the safety and efficacy of a new low-molecular-weight heparin (LMWH) (LU 47311, Clivarine) and standard unfractionated heparin for the prophylaxis of postoperative venous thromboembolism. Altogether 1351 patients scheduled to undergo abdominal surgery were included. Main outcome measures included the incidence of thromboembolic events (deep vein thrombosis, pulmonary embolism, or both) and bleeding complications, including wound hematoma. A total of 655 patients received 1750 anti-Xa IU of LMWH plus a placebo injection daily; 677 patients received 5000 IU of unfractionated heparin (UFH) twice a day. Both drugs were found to be equally effective, as 4.7% of patients in the LMWH group and 4.3% in the UFH group developed postoperative thromboembolic complications. However, the incidence of bleeding complications was significantly reduced in the LMWH group: 55 (8.3%) patients in the LMWH group and 80 (11.8%) in the UFH group developed bleeding complications, a relative risk (RR) of 0.70 (95% CI 0.51–0.97; p = 0.03); wound hematoma occurred in 29 (4.4%) of the LMWH group compared with 55 (7.7%) in those in the UFH group for an RR of 0.57 (95% CI 0.37–0.88; p = 0.01). This study confirmed that a very low dose of 1750 anti-Xa IU daily of this new LMWH is as effective as 10,000 IU of UFH for preventing postoperative deep vein thrombosis. At this dose its administration is associated with a significant reduction in the risk of bleeding including wound hematoma.

    Notes: Summary The paper is concerned with a one-dimensional analysis of plane open-channel flow with continuous solidification. The process is of relevance for recent developments in the casting of steel and other metals. The bottom of the channel consists of a rotating casting roll and a horizontal cooling table, where the solidified material is withdrawn with given velocity. The study is restricted to the region downstream of the top of the casting roll. Surface tension is neglected. In the main part of the analysis inviscid fluid flow is considered since the Reynolds number is very large in the applications. It is found that the steady-state solutions are nonunique in a certain parameter range. In addition to a continuous solution, there are two solutions including hydraulic jumps, with one hydraulic jump being located on the casting roll, the other one on the cooling table. Regarding the stability of the non unique solutions, the evolution of disturbances is investigated numerically as an initial-value problem. It is concluded that the hydraulic jump on the cooling table is unstable, while the other discontinuous solution as well as the continuous solution are stable for sufficiently small disturbances. Which stable solution is attained in the steady state, depends on the history of the process. Friction at the liquid/solid interface is taken into account in the last part of the analysis. A constant friction coefficient is assumed. It is found that the history of the process determines the steady-state solution if, and only if, the friction coefficient is sufficiently small. For larger values of the friction coefficient, the steady-state solution is unique and independent of the history of the transient process. Furthermore, for sufficiently large friction coefficients, stable hydraulic jumps are found, in contrast to the inviscid case, also on the cooling table.

    Description / Table of Contents: Summary Background: Primary radiochemotherapy is gaining increasing importance for the treatment of advanced head and neck squamous cell carcinomas. However, there is a lack of clinical factors concerning prognostic information in relation to treatment. In this pilot study, we examined whether molecular factors can provide such information. Patients and methods: The expression patterns and their possible prognostic relevance of the cell cycle regulatory proteins p53, p21WAF/CIP1, Rb, p16INK4A, CDK4 and Cyclin D1, MIB1 (Ki-67) and BCL-2 were analysed in pretreatment tumor biopsies from 43 patients with advanced carcinomas of the oropharynx (n=26), hypopharynx (n=10) and larynx (n=7) by applying immunohistochemistry to paraffin sections of tumor specimens. All patients were treated by the same method of an accelerated ”concomitant boost” radiochemotherapy with carboplatin in a phase II study. Median followup time was 56 months. Results: No correlation was found between any of the markers and the remission rate, T-stage, N-stage, rate of loco-regional recurrences and distant metastases. However, independent of the tumor stage, patients with CDK4/cyclin-D1 overexpressing tumors had a significantly shortened overall survival (P=0.014 and 0.026, respectively). Conclusion: The results of this pilot study indicate that CDK4 and cyclin D1 over-expression provide useful prognostic information about clinical outcome after primary radiochemotherapy of head and neck cancer patients.
    Notes: Zusammenfassung Hintergrund: Bei fortgeschrittenen Kopf-Hals-Karzinomen wird in zunehmendem Maße die primäre Radiochemotherapie einer primär operativen Therapie vorgezogen. Klinische prognostische Faktoren für die Krankheitsverläufe nach primärer Radiochemotherapie fehlen jedoch. Die vorliegende Pilotstudie sollte klären, ob molekulare Faktoren prognostische Aussagen liefern können. Patienten und Methoden: In prätherapeutischen Biopsien von 43 Patienten mit fortgeschrittenen Karzinomen wurde die Expression der Zellzykluskomponenten p53, p21WAF/CIP1, Rb, p16INK4A, CDK4 und Cyclin D1, des Proliferationsmarkers MIB1 und des apoptosehemmenden Proteins BCL-2 immunhistochemisch bestimmt. Die Patienten wurden nach dem Protokoll einer akzelerierten „Concomitant-boost-Radiochemotherapie” mit Carboplatin im Rahmen einer Phase-II-Studie behandelt. Die mediane Beobachtungszeit zum Analysezeitpunkt betrug 56 Monate. Ergebnisse: Keiner der Marker korrelierte mit dem Tumorstadium und N-Status, den Remissionsraten nach Therapieende oder dem posttherapeutischen Auftreten von Rezidiven, Metastasen und Zweitkarzinomen. Dahingegen hatten Patienten mit CDK4- und Cyclin D1-überexprimierenden Tumoren unabhängig vom Tumorstadium eine signifikant kürzere Gesamtüberlebenszeit (p=0,014 bzw. 0,026). Schlußfolgerung: Die Ergebnisse der Pilotstudie indizieren eine prognostische Bedeutung der Überexpression von CDK4 und Cyclin D1 nach primärer Radiochemotherapie von fortgeschrittenen Kopf-Hals-Karzinomen.
