Zusammenfassung Die Mikrofiltration soll Aggregate und Zellzerfallsprodukte aus dem Transfusionsblut entfernen, ohne den Blutdurchfluß zu beeinflussen, einen Anstieg des freien Hämoglobins zu verursachen und ohne die Blutgerinnung zu aktivieren. Im Hinblick auf diese Anforderungen haben wir das Standard-Transfusion-Set, den Pall- und MF-10-Mikrofilter untersucht. Mit gesteigerter Blutmenge fanden wir leichte Anstiege des freien Hämoglobins und der Durchflußzeiten, deren Ausmaße mit der Porengröße der verwendeten Filter korrelieren. Bei Massivtransfusionen müssen deshalb 101i Porenfilter öfter gewechselt werden als 40 μ Porenfilter. Die Mikrofiltration von Blut aktiviert die Gerinnung nicht, sondern entfernt gerinnungsfördernde Substanzen.

Summary Ideally, microfilters should remove all the aggregates and debris of the transfusion blood, while at the same time allowing an unimpeded flow of blood and not causing an increase of the free hemoglobin or an activation of blood coagulation. With regard to these requirements we have tested the standard transfusion set, the Pall- and the MF-10-microfilter. We observed a slight increase of free hemoglobin and blood flow with increasing numbers of blood units. This was correlated with the pore size of the filters. Thus 10 μm filters must be changed more often than 40 pin filters in massive transfusions. Microfiltration does not activate blood coagulation, but removes procoagulant material.
