    Description / Table of Contents: Zusammenfassung Für die Zukunft ist eine verstärkte Kooperation der verschiedenen Disziplinen der geburtshilflichen Arbeit (Entbindungsbereich, Wochenbettstation, Elternschule) anzustreben (geburtshilfliches Team). Dabei sollte versucht werden, die personelle Kontinuität bei der Versorgung der Gebärenden zu gewährleisten. Da die Lösung dieser Forderung sich als schwer erweist, kann die Einbeziehung der freiberuflichen Hebammen in die Klinik nach dem Charité-Virchow-Modell evtl. hilfreich sein. Wichtiger für die Akzeptanz der klinischen Geburtshilfe als „alternative Gebärform“ ist die Anteilnahme an der Gebärenden, ihrer Versorgung, ihrem Wohlbefinden durch die Mitglieder des geburtshilflichen Teams.
    Notes: Summary In the future the goal should be increased cooperation between the different disciplines of obstetrical work (maternity ward, lying-in ward, parents' school). At the same time, personnel continuity in patient care should be ensured. Since the solution is difficult, inclusion of freelance midwives in the clinic like the Charité-Virchow model could be helpful. More important for the acceptance of clinical obstetrics than the means “alternative” delivery is how interested the members of the obstetrics team are in the patient and her care and well-being.

    Notes: Zusammenfassung Die Frühgeburt ist eine wesentliche Ursache kindlicher Mortalität und Morbidität. Wir bemühen uns, die Rate der Frühgeburten durch geeignete soziale und medizinische Maßnahmen zu senken, wobei besondere Hoffnung auf die Prävention gesetzt werden muß. Um präventiv tätig werden zu können, ist die Identifikation der frühgeburtsgefährdeten Frauen mit Hilfe von anamnestischen, klinisch befundbaren, ultrasonographischen, laborchemischen und mikrobiologischen Frühgeburtsmarkern notwendig.

    Notes: Zum Thema Es wird die Frage erörtert, wann Mehrlingsschwangerschaften durch Sektio beendet werden sollen und was bei vaginaler Entbindung zu beachten ist. Bei Drillings- und höhergradigen Mehrlingsschwangerschaften kommt es gehäuft zu Lageanomalien, regelwidrigen Einstellungen und intrapartalen Komplikationen. Deshalb stellt sich die Indikation zur elektiven Sektio. Hieraus ergibt sich als zusätzlicher Vorteil die Möglichkeit der Terminierung des geburthilflichen und neonatologischen Teams und der neonatologischen Intensivbetten. Bei Zwillingsschwangerschaften ist die Indikation zur primären Schnittentbindung von der Einstellung der Zwillinge abhängig. Befindet sich der erste Zwilling in Nichtschädellage, so ist eine Sektio anzuraten. Zeigt dagegen der 2. Zwilling eine Nichtschädellage, so kann auch die vaginale Entbindung mit einer äußeren Wendung oder alternativ mit einer Entwicklung aus BEL versucht werden. Bei monoamniotischen Zwillingen können durch Nabelschnurbehinderungen Komplikationen eintreten, so daß auch hier eher zu einer Sektio zu raten ist. Bei Zwillingen unter einem geschätzten Geburtsgewicht von 1800 g oder einer Gewichtsdifferenz zwischen dem 1. und dem 2. Zwilling von mehr als 20 % wird ebenfalls die Sektio bevorzugt.

    Notes: Abstract The preventive effect of maternal avoidance of cow’s milk and eggs on the development of atopic dermatitis and sensitization to food allergens was studied in a prospective trial with families at high risk for atopy. intervention included encouraging exclusive breast feeding for at least 3 months and delaying the controlled introduction of solid foods. In addition, one group received a maternal diet with the complete avoidance of milk and eggs in the last trimester of pregnancy and during the period of exclusive breast feeding, another group had the same diet starting after delivery, and mothers of the third group had no dietary restrictions. The period prevalences of atopic dermatitis as well as the rates of specific sensitization to eggs and milk at 6 and 12 months were not significantly different between groups. We were unable to demonstrate a significant preventive effect of maternal diet. Conclusion While breast feeding should be promoted for children at risk for atopy, mothers can be encouraged to stay on normal diet during pregnancy and the breast feeding period.

    Notes: Abstract l-Selectin, a leukocyte surface glycoprotein involved in white blood cell extravasation, is rapidly down-regulated after leukocyte activation. We prospectively determined lymphocytel-selectin expression in freshly obtained cord blood samples of 98 neonates (gestational age 25–42 weeks). In eight infants with bacterial infection, the mean percentage ofl-selectinhigh lymphocytes was 32.5% (SD 20.1%), compared to 60.1% (SD 18.7%) in the control group (P〈0.01). A percentage ofl-selectinhigh lymphocytes of less than 42% had a sensitivity of 75% and a specificity of 82% in identifying infected newborns. Cord blood lymphocytel-selectin expression was independent of gestational age, birth weight, umbilical artery pH, hematocrit, white blood cell count, absolute neutrophil count, C-reactive protein level, or maternal fever before delivery while there was a weak correlation with the newborn's immature/total ratio and platelet count. To our knowledge, this is the first report demonstrating downregulation of human lymphocytel-selectin expression following activation of the immune system in vivo.
