    Notes: Immunoprecipitation of radioiodinated hairy cell leukaemia (HCL) cell lysates with monoclonal antibody (MoAb) HML-1. originally reported to recognize intraepithelial T cells, and with MoAb B-ly7, originally reported to react with HCL. led to identical biochemical characteristics. In SDS PAGE under reducing conditions, a major band of 143 kDa. a broad band ranging from 112 to 122 kDa. and two additional faint bands of 175 and 100 kDa could be determined. Deglycosylation of N-linked sugar moieties by treatment of immunoprecipitates with endoglycosidases indicated that the two main protein cores of the antigen are predominantly if not exclusively glycosylated by complex and hybrid types of oligosaccharide chains. Competitive binding inhibition demonstrated that both MoAb are directed against different epitopes. Immunohistochemically. the staining patterns obtained with both MoAb in normal tissues, in T-and B-cell lymphomas, and in HCL were identical except for a single case of HCL which was HML-1−/B-ly-7+, We conclude that MoAb HML-1 and B-1y7 recognize the same antigen.

    Notes: Summary The myocardium was studied histologically at autopsy in seven patients who died 9–85 days (median 22 days) after autologous bone marrow or blood-derived stem-cell transplantation. Clinical investigations including echocardiography suggested normal cardiac function in all patients prior to transplantation. Myeloablation was performed by total body irradiation (1200–1560 cGy) and cyclophosphamide (200 mg/kg). Morphological findings were graded semiquantiatively and correlated with previous and current therapy. Histological alterations did not correspond to the dosages of myeloablative therapy. However, cardiac failure, the primary cause of death in four patients, was associated with coagulative fiber necroses and contraction band necroses, which may be related to acute cyclophosphamide toxicity. In three of these patients high-dose cardiotoxic pretreatment with anthracyclines and mitoxantrone has been performed beyond the critical cardiotoxic level. High-dose pretreatment was correlated with histological hallmarks of anthracycline heart disease: marked chromatin clumping of myocardial cell nuclei (3/3 patients), occurrence of plurivesicular “adria” cells (3/3 patients), and diffuse interstitial myocardial fibrosis (2/3 patients). Our morphological observations indicate for the first time that pretreatment with anthracyclines and mitoxantrone may enhance cardiotoxicity of myeloablative therapy in bone marrow transplantation.

    Notes: Summary A human monoclonal IgM-K cold agglutinin Ad, reacting with immunodominant neuraminyl groups on red cell membrane sialoglycoproteins, as typical for anti-Pr CA, was studied for its subspecificity. Inhibition experiments with chemically modified red cell sialoglycoproteins and agglutination studies with animal red cells showed that the CA Ad did not fit into the subspecificities anti-Pr1h,-Pr1d,-Pr2,-Pr3. Cold agglutinin Ad presents a new specificity within human monoclonal antibodies against immunodominant neuraminyl groups.

    Notes: Summary An autologous bone marrow transplantation (ABMT) was performed in a 45-year-old male patient with AML in therapy-resistant first relapse including CNS-disease. The marrow graft was harvested 18 months before, at the beginning of first remission, and was subjected to an in-vitro incubation with 4-hydroperoxycyclophosphamide (4-HC) prior to cryopreservation to eliminate residual clonogenic tumor cells. After myeloablative therapy with high-dose Cyclophosphamide (CY), total body irradiation (TBI) and intrathecal application of Methotrexate (MTX), the manipulated marrow graft was reinfused. The myelopoietic reconstitution started already 6 days after ABMT and was completed 40 days thereafter. There is evidence for a complete remission in marrow and spinal fluid observed over a period of 4 months.

    Description / Table of Contents: Summary The leukocyte alkaline phosphatase (LAP) levels were determined in 183 patients with malignant diseases and 71 normal controls. The median LAP scores were 64 units (range 0 to 290) for the patients and 55 (range 2 to 158) for the controls, respectively, and no significant difference could be established. When analyzed according to primary malignancy, only in patients with Hodgkin's disease (n = 14) was the median value higher than normal (p 〈 0.001). In patients with distant metastases (n = 48), higher LAP levels were demonstrated (M = 76, range 21 to 290) as compared to patients with no evidence of metastases (M = 53, range 0 to 229), (p 〈0.01). Thus, LAP activity has very limited value in the diagnosis of malignancies. Its elevation in the presence of malignant disease might, however, indicate metastases.
    Notes: Zusammenfassung Bei 183 Patienten mit malignen Erkrankungen und 71 Kontrollpersonen wurde der ALP-Index untersucht. Der Median für die Patienten war 64 (range 0–290) und für die Kontrolle 55 (range 2–158). Dieser Unterschied ist jedoch statistisch nicht signifikant. Bei der Analyse der ALP-Indices nach Tumorgruppen konnte eine signifikante Erhöhung des ALP-Index nur bei Morbus Hodgkin (n = 14) festgestellt werden (p 〈0,001). Der ALP-Index war auch bei Patienten mit Metastasen (n = 48) (M = 76, range 21 zu 290) im Vergleich zum ALP-Index bei Patienten ohne Metastasen (n = 118, M = 53 range 0–229) erhöht (p 〈0,01). Die ALP-Aktivität hat also nur einen beschränkten Wert in der Diagnostik bei malignen Erkrankungen. Eine Erhöhung kann jedoch auf das Vorliegen von Metastasen hinweisen.

    Notes: Summary We investigated the immunological heterogeneity of plasma Tg in thyroid cancer patients using monoclonal antibodies in an immunoradiometric assay and a conventional RIA system with a polyclonal rabbit antibody. The results were compared with measurements of plasma Tg in patients with nonmalignant disease. We can demonstrate an increased immunological heterogeneity in tumor patients compared with patients with non-malignant thyroid diseases. In one case the Tg value measured by a monoclonal test system exceeded the value obtained by a polyclonal RIA system in the same sample by a factor of 25. It has to be further investigated whether this increase in heterogeneity is of diagnostic value in the follow-up of thyroid cancer patients.

    Notes: Summary Hybridomas secreting monoclonal antibodies against purified cold agglutinins (CA) from lymphoma patients were screened by a cold hemagglutination inhibition test. Supernatants from positive clones were further tested against several purified CA and paraproteins of different immunoglobulin (Ig) classes. It was shown that most monoclonal antibodies raised by immunization with CA had reactivity against the constant region of IgM. However, clone H-1 produced an anti-idiotypic antibody that reacted exclusively with the CA used for immunization. Using this anti-idiotype antibody, the idiotype could be demonstrated on 25% of the patient's peripheral mononuclear cells. So far, the idiotype could not be demonstrated on the patient's T-cells. Monoclonal antibodies against lymphoma idiotypes are powerful tools for studying the immunobiology of these malignancies and may be useful as specific therapeutic reagents.

    Notes: Summary In an attempt to induce in vitro differentiation we exposed cells from patients with chronic lymphocytic leukemia (CLL) and with immunocytoma (IC) to 12-0-tetra-decanoylphorbol-13-acetate (TPA) at 160 nM. After 3 to 5 days of culture cells became enlarged and the CLL cells developed a basophilic cytoplasm with an excentric nucleus. In some instances cells with many fine projections were seen. We then employed the monoclonal antibody (MAB) HD 6, which was generated against hairy cell leukemia (HCL) cells and reacts most strongly with such cells but is unreactive with plasmocytoma cells and with most CLL cells. TPA treatment induced a greatly enhanced HD 6 binding in CLL and IC cells compared to controls as demonstrated by indirect immunofluorescence. Cytochemical studies revealed that at the same time an acid phosphatase was induced, which was found to be tartrate-resistant in every instance tested. Thus, several features of HCL can be induced in CLL and IC demonstrating the close relatedness of these entities.
