    Description / Table of Contents: Summary Complete blood counts, bone marrow aspirations and autoradiographs of bone marrow smears were performed in twelve persons to whom Thorotrast (thorium dioxyde) had been given intravenously about 20 yars ago. Peripheral blood cell counts were within normal limits in all subjects; however, marrow smears of ten persons showed increased erythropoiesis. Furthermore an increase of binucleated red cell precursors which normally are rarely found, was noted. Thorotrast deposits were present in all marrow smears, and autoradiographs demonstrated many alpha-tracks radiating from the incorporated radio-active material. The question arises whether the cytologic abnormalities in the erythroblasts are due to the effect of continous exposure to low levels of ionising radiation.
    Notes: Zusammenfassung Bei 12 Personen, die vor ca. 20 Jahren Thorotrast erhalten hatten, wurden eine Blut- und Knochenmarkuntersuchung durchgeführt und von den Markausstrichen Autoradiographien angefertigt. Während das periphere Blutbild keine wesentlichen Abweichungen von der Norm zeigte, fand sich in 10 Fällen eine gesteigerte Erythropoese im Mark und in 8 Fällen das gehäufte Vorkommen von binukleären roten Vorstufen. Das Vorhandensein von Thorotrast im Knochenmark wurde autoradiographisch durch den Nachweis von α-Spuren im Bereich phagozytierender Retikulumzellen bestätigt. Es wird diskutiert, ob es sich bei den beobachteten mitoseabhängigen Zellatypien um Spätfolgen einer sogenannten “low level exposure” handeln könnte.

    Description / Table of Contents: Zusammenfassung Die extrakorporale Blutbestrahlung wurde bei 6 Patienten mit chronisch lymphatischer Leukämie in verschiedenen Krankheitsstadien eingesetzt. Bei 2 Patienten im Präfinalstadium traten Fieberschübe auf, die zum Absetzen der Behandlungsversuche zwangen. Bei 4 Patienten stellten sich ohne relevante Nebenwirkungen Teilremissionen ein. Die Remissionsdauer lag bei einem Patienten ohne Weiterbehandlung bei 6 Monaten. Bei den übrigen Patienten waren zur Erhaltung der Teilremission extrakorporale Blutbestrahlungen in 1–2monatigen Intervallen notwendig. Hierdurch wurden Remissionszeiten von 6 und 16 Monaten erzielt; ein Patient befindet sich mehr als 2 Jahre nach Therapiebeginn noch in einer Teilremission.
    Notes: Summary Extracorporeal irradiation of blood (ECIB) was used in the treatment of 6 patients suffering of different stages of chronic lymphocytic leukemia. In 2 patients in the prefinal stage of the disease, severe fever was observed during irradiation which prevented the further use of this therapy. In 4 cases, partial remission could be induced without significant side effects. In one patient the duration of remission by this therapy was 6 months without any further treatment during the interval. In the other patients ECIB had to be continued at 1–2 monthly intervals in order to maintain the partial remission. One patient is still alive and still in remission more than 2 years after the onset of ECIB therapy.

    Description / Table of Contents: Zusammenfassung Die extrakorporale Blutbestrahlung ermöglicht durch die hohe Strahlensensibilität der Lymphocyten deren Zerstörung im Blut ohne zusätzliche Störung der Homöostase. Das Verfahren lieferte wesentliche Informationen über die Dynamik der Lymphocyten-Migration und-Proliferation und Funktion dieses Zellsystems. Bei klinischer Anwendung der extrakorporalen Blutbestrahlung konnten gute Therapieergebnisse bei der chronisch lymphatischen Leukämie erzielt werden. Hier ergibt sich aufgrund fehlender Nebenwirkungen die Möglichkeit der Behandlung von Frühstadien der Erkrankung. Es liegen außerdem Befunde vor, die zeigen, daß bei akuten Leukämien durch eine Reduktion des Pools leukämischer Zellen mit Hilfe der extrakorporalen Blutbestrahlung die restlichen Leukämiezellen zu einer verstärkten Proliferation angeregt werden können und somit eine verbesserte Situation für die cytostatische Therapie erreicht werden kann. — Der Einsatz der extrakorporalen Blutbestrahlung zur Immunsuppression kann unter geringen zusätzlichem Arbeitsaufwand bei Patienten vor der Nierentransplantation während der Dialyse erfolgen. Die bisher vorliegenden günstigen Ergebnisse lassen annehmen, daß diese Anwendungsmöglichkeit des Verfahrens eine besondere Bedeutung erlangen kann.
    Notes: Summary The high sensibility of lymphocytes renders possible their selective destruction by extracorporal irradiation of the blood without additional impairment of the homeostasis. This method provides a useful experimental tool to the study of the dynamics of lymphocyte migration, proliferation, and function. Good therapeutic results could be achieved by applying extracorporeal irradiation of the blood in the treatment of chronic lymphocytic leukemia. This form of therapy would permit to treat early stages of the disease without concomitant side effects. Furthermore, there is evidence that the reduction of the pool of leukemic cells in acute leukemia by means of extracorporal irradiation of the blood can stimulate resting leukemic cells and thus provide better efficiency of cytotoxic therapy. Extracorporeal irradiation of the blood can be applied during dialysis in patients who are to undergo kidney transplantation. The favorable results thus far obtained led the authors to suppose that this application of the method can achieve particular significance.

    Notes: Summary From 1972–1983 53 patients underwent bone marrow transplantation. The median age was 18 years (3–41). 27 patients suffered from severe aplastic anaemia, 22 patients had acute leukaemia and 4 patients had chronic granulocytic leukaemia in chronic phase. Out of 22 patients with acute leukaemia, 2 had florid leukaemia, 2 had an early relapse and 18 patients were in first or second remission of their disease. 2/53 patients received a syngeneic transplant, 51/53 patients an allogeneic transplant. 47/51 patients had a HLA-A, B, C-identical, MLC-negative sibling donor, 1/51 had a HLA-A, B-C-identical, MLC-positive sibling donor, 2/51 a HLA-phaenotypical identical parental donor and 1/51 a HLA-identical, MLC-negative unrelated donor. The comparison of the results obtained in patients with severe aplastic anaemia transplanted from 1972–1979 with those transplanted from 1980–1983 shows that the bone marrow transplantation has to be performed in an early stage of the disease before the patients become multiple transfused, sensitized and severely infected and that the conditioning regimen for polytransfused patients has to be more intensive than in untransfused patients. From the patient group transplanted 1972–1979, only 1/14 patients is a long-term survivor in contrast to 8/13 patients transplanted from 1980–1983. 11/22 patients with acute leukaemia are alive between more than 5 years and 14 days after bone marrow transplantation. Only 1/4 patients, who were transplanted not in remission, is alive. For patients with acute leukaemia the bone marrow transplantation should be performed in an early stage of their disease when the tumor burden is small and when the patients are in good clinical condition. 2/4 patients with CGL are alive between 12 months and 3 months after bone marrow transplantation. In our patient group graft versus host disease was the most important problem with a high mortality due to GvHD associated infections.
