    Description / Table of Contents: Zusammenfassung Fibrinogen und Fibrin, Endsubstrat und Endprodukt des Gerinnungssystems, scheinen bei der Entwicklung und Ausbreitung von Malignomen die Rolle eines pathogenetischen Bindegliedes zu spielen. In der Literatur zeigen die Forschungsergebnisse, daß Fibrin insbesondere in die Initialphase der hämatogenen Metastasierung eingreift. Für die häufig anzutreffende, oft reichliche Fibrinablagerung in Tumoren konnten mehrere tumorspezifische Mechanismen nachgewiesen werden. Die Ergebnisse der zitierten Forschungsarbeiten weisen auf die Bedeutung des intratumorös abgelagerten Fibrins für Tumorzelle und -zellverband hin. Die von verschiedenen Autoren berichteten Fibrinanreicherungen in tierexperimentellen Tumoren werden semiquantitativ verglichen. Die Diskussion der therapeutischen und diagnostischen Konsequenzen, die auf einer möglichen Schlüsselstellung des Fibrin(ogen)s in der Tumorpathologie beruhen, zeigt bekannte und noch zu untersuchende Möglichkeiten.
    Notes: Summary It seems evident that fibrinogen and fibrin representing the final substrate und product of clotting system act as pathogenetic connecting links for development and spread of malignant tumors. The results reported demonstrate the influence of fibrin on the initial phase of haematogenic metastasis particularly. Several tumor-specific mechanisms are shown to cause frequently an accumulation of fibrin in malignant tumors. The results discussed here stress the importance of fibrin in tumors for tumor cells and tumor cell units. In the literature a large number of indications to fibrin depositions in experimental tumors is found, an attempt is made to compare these findings semiquantitatively. The last part of this article discusses the therapeutic and diagnostic consequences based on the possible key position of fibrin(ogen) in the tumor pathology.

    Description / Table of Contents: Zusammenfassung Die Überwachung der Gravidität und Leitung der Geburt bei gleichzeitig bestehender idiopathischer thrombozytopenischer Purpura (ITP) erfordern besondere prophylaktische und therapeutische Maßnahmen, die sich aus der Kenntnis der Pathophysiologie und des Blutungsrisikos ergeben. Bei einer Schwangeren mit einer seit der Kindheit bekannten ITP wurde in der 33. Schwangerschaftswoche eine erhebliche Exazerbation der Erkrankung beobachtet. Unter einer ausreichend hochdosierten Glukokortikoid-Medikation stiegen die Thrombozytenzahlen an, und es konnte unter Anwendung der Überwachungskriterien einer Risikogravidität die 39. Schwangerschaftswoche erreicht werden. An die aus mütterlicher und kindlicher Indikation durchgeführte elektive primäre Sectio caesarea wurde die Splenektomie angeschlossen, die auf Grund der langen Dauer der ITP angezeigt erschien. Eine Thrombozytentransfusion wurde erst nach Abklemmen der Milzgefäße begonnen. Die anschließend erwartete Thrombozytose machte eine vorübergehende Behandlung mit Azetylsalizylsäure erforderlich. 18 Monate später wies die Patientin eine konstante Remission auf. Die zunächst ausgeprägte neonatale Thrombozytopenie normalisierte sich ohne spezifische Behandlung und ohne stärkere klinische Symptome. Die Entfernung der Milz im Anschluß an eine Entbindung ist ein eher ungewöhnliches Vorgehen, dessen Planung und Durchführung eingehend geschildert und diskutiert werden.
    Notes: Summary We report a patient who had a marked exacerbation of idiopathic thrombocytopenic purpura at 33 weeks of pregnancy. The platelet count rose in response to treatment with high doses of glucocorticoids and it was possible to allow the pregnancy to continue until 39 weeks. The patient was then delivered by elective Caesarean section and she had a splenectomy at the same time. A transfusion of platelets was administered as soon as the splenic vessels had been clamped. The resultant excess of platelets necessitated treatment with acetylsalicylic acid. At 18 months after Caesarean section and splenectomy the thrombocytopenia had not recurred. The infant had transient neonatal thrombocytopenia but required no active treatment for this.

    Notes: Summary The procoagulant activity of amniotic fluid (AF) increases with advancing gestational age. AF samples (N = 276) were measured by a modified recalcification time method. The results were expressed as equivalents of Russell's viper venom (RVV) which acts in a similar way as AF by way of direct activation of coagulation factor X. After week 35 of pregnancy RVV values reached or passed 0.09 μg/ml. This value represents a treshold indicating fetal lung maturity. In 124 cases postnatal lung maturity was related to RVV equivalents. The predictive value of a mature result was 99.1%, that of an immature one 52.2%. When comparing RVV values with the lung status, surface tension measurements, and L/S ratio a high correlation was found. L/S ratio and procoagulant activity had a correlation coefficient ofr = 0.60. Betamethasone seemed to produce the expected changes in the results of RVV assays and surface tension measurements.

    Notes: Zusammenfassung In einem Kollektiv von 8544 Neugeborenen blieb während eines Zeitraumes von 8 Jahren bei einer stark schwankenden Morbiditätsrate der Neugeborenen (15–22%) die Verlegungsfrequenz ziemlich konstant (4–7%). Die Schwankungen wurden verursacht durch die Diagnose Hyperbilirubinämie. Bei intensiver Betreuung auf der Neugeborenen-Station der Geburtsklinik konnte in Zusammenarbeit mit dem konsiliarisch tätigen Pädiater ohne Beeinträchtigung der Sicherheit der Kinder die Verlegungsrate dennoch klein und konstant gehalten werden.

    Notes: Summary ITP in pregnancy may lead to fetal thrombocytopenia caused by the transplacental passage of maternal antiplatelet antibody. The most hazardous complication in the infant is intracranial hemorrhage. In addition ITP in pregnancy is reported to be associated with an increased abortion rate and an elevated fetal morbidity and mortality. Therefore obstetric management must aim at increasing maternal and fetal platelets. Severel therapeutic approaches to the treatment of ITP in pregnancy are evaluated. Two cases of ITP in pregnancy are reported. Administration of high-dose intravenous immunoglobulin is introduced as a new therapy for ITP in pregnancy. The rapid reversal of thrombocytopenia following immunoglobulin G administration suggests that it is useful especially as emergency treatment for ITP in pregnancy.

    Notes: Summary In pregnancy and neonatal period both mother and child are endangered by bleeding complications due to maternal idiopathic thrombocytopenic purpura. Obstetrical and perinatal management therefore must aim at increasing maternal and fetal platelet count. In our paper six patients in nine pregnancies are reported. Two of them (five pregnancies) were treated with corticosteroids, four of the patients were successfully treated with i.v. immunoglobulins (IgG). Longterm steroid application and splenectomy during pregnancy may be hazardous for mother and fetus. IgG i.v. administration in contrast offers a new and safe way to control maternal and fetal platelet counts during pregnancy, delivery and the neonatal period.
