    Notes: Summary We investigated the immunological heterogeneity of plasma Tg in thyroid cancer patients using monoclonal antibodies in an immunoradiometric assay and a conventional RIA system with a polyclonal rabbit antibody. The results were compared with measurements of plasma Tg in patients with nonmalignant disease. We can demonstrate an increased immunological heterogeneity in tumor patients compared with patients with non-malignant thyroid diseases. In one case the Tg value measured by a monoclonal test system exceeded the value obtained by a polyclonal RIA system in the same sample by a factor of 25. It has to be further investigated whether this increase in heterogeneity is of diagnostic value in the follow-up of thyroid cancer patients.

    Description / Table of Contents: Zusammenfassung Unter Patienten mit euthyreoter Struma bzw. Strumaresektion unter einer Langzeit-suppressionsbehandlung mit Schilddrüsenhormonen wurden 27 Patienten mit neg. TRH-Test ausgewählt. Die Hormontherapie wurde dann abgesetzt und nach 1, 2 und 4 Wochen der Verlauf der T3− und T4− Konzentrationen sowie der basalen und TRH-stimulierten TSH-Sekretion untersucht. Es zeigte sich ein gradueller Anstieg des mittleren basalen wie TRH-stimulierten TSG-Spiegels im Untersuchungszeitraum. 4 Wochen nach Absetzen der Substitution finden sich immer noch 2 negative TRH-Teste sowie eine Häufung von nur schwach positiven Testen. Für einen aussagefähigen TRH-Test nach einer Langzeit-suppressionsbehandlung ist deshalb ein hormonfreier Intervall von mindestens 4 Wochen zu fordern. Die niedrigsten T3-Spiegel wurden eine Woche nach der letzten Hormongabe gemessen. Dies könnte als Hinweis für eine passagere, thyreotrope Insuffizienz gewertet werden.
    Notes: Summary 27 subjects with a negative TRH test were selected among patients with euthyroid goiter treated by thyroid hormones. The hormone therapy was stopped and T3 and T4 concentrations, basal and TRH stimulated TSH levels were determined after one, two and four weeks. A gradual increase of the mean basal and TRH stimulated TSH plasma concentration could be observed. However, after four weeks the TRH test was still negative in two patients and weak positive in another case. T3 plasma levels were lowest after one week which might indicate the presence of an intermittent thyrotropic insufficiency. It is concluded that the hormone-free interval should last four weeks at least before a reliable TRH test can be obtained.

    Notes: Summary To investigate the effect of thyroid hormones on erythrocyte cation transport systems and intracellular electrolyte content we have measured the activity of Na-K ATPase, Na-Li countertransport, as well as red cell sodium and potassium contents in patients with hyperthyroidism and in euthyroid controls. Intracellular Na- and K-concentrations were determined in erythrocytes washed three times in isotonic MgCl2 solution. Ouabain-sensitive Na-transport was estimated as the increase of Na before and after addition of ouabain in an erythrocyte suspension in isotonic Na-free medium. Na-Li countertransport was measured according to the method described by Canessa et al. [2]. The patients with hyperthyroidism exhibited a significantly elevated intracellular sodium content as well as a highly increased Na-K ATPase activity. Intracellular potassium content was not altered in the hyperthyroid subjects, but Na-Li countertransport was markedly decreased as compared to the controls. The results indicate that different ion transport systems of the erythrocyte membrane are influenced by thyroid hormones. We suggest that the elevation of Na-K ATPase activity might be due to the increased intracellular sodium concentration which is caused by the diminished countertransport pathway. Furthermore, the activity of Na-K ATPase, Na-Li countertransport, and intracellular sodium content in erythrocytes might be a useful peripheral indicator of thyroid hormone excess.

    Description / Table of Contents: Zusammenfassung Es wurde eine radioimmunochemische Bestimmungsmethode für humanes, synthetisches Calcitonin (HCT) beschrieben. Antikörper wurden durch Immunisierung von Kaninchen mit an BSA gekoppeltem HCT gewonnen. Die Empfindlichkeit der Methode beträgt 5 pg. Bei 14 Normalpersonen wurden 50–150 pg/ml gemessen. 5 Normalpersonen wiesen Spiegel unter der methodischen Erfaßbarkeit auf. Die Halbwertszeit betrug 12 bzw. 17 min.
    Notes: Summary A radioimmunochemical method for determination of human calcitonin (HCT) is described. Antibodies were obtained by immunisation of rabbits with HCT conjugated to bovine serum albumine. The sensitivity of the assay is about 5 pg. In 14 normal subjects levels of 50–150 pg/ml were measured. In 5 subjects the level was below the sensitivity of the method. In two cases the half-life of injected synthetic HCT measured was 12 and 17 minutes, respectively.

    Description / Table of Contents: Summary Urinary cyclic AMP excretion per 24 h or per g creatinine in primary hyperparathyroidism (1° HPT) has been evaluated by several authors with conflicting results. In 50 patients with 1° HPT, 25 patients with secondary (2°) HPT and 35 healthy control persons we determined urinary cyclic AMP per 24 h or per g creatinine. These parameters did not satisfactorily discriminate patients from controls, especially when glomerular filtration rate (GFR) as determined by creatinine clearance was reduced. Since urinary cyclic AMP is derived from plasma by glomerular filtration and from kidney by tubular production—the amount of tubules is reflected by GFR—the cyclic nucleotide was related to GFR. In controls urinary cyclic AMP correlated better with GFR than with creatinine excretion. Additionally, in 45 of 50 patients with 1° HPT and in all with 2° HPT, urinary cyclic AMP/GFR was raised. In 1° HPT serum levels of parathyroid hormone correlated closer with urinary cyclic AMP/GFR than with urinary cyclic AMP/g creatinine. The ratio cyclic AMP/GFR decreased to normal or subnormal values after removal of adenomatous or hyperplastic glands in 1° HPT and during infusion of calcium in 2° HPT. In 50 patients with renal lithiasis caused by diseases other than 1° HPT (anatomical variations, pyelonephritis, immobilization after tetraplegia) the ratio cyclic AMP/GFR was not raised. Urinary cyclic AMP/GFR, therefore, reflects parathyroid hormone excess more reliably than cyclic AMP/g creatinine.
    Notes: Zusammenfassung Parathormon erhöht die renale Ausscheidung von cyclischem AMP (cAMP). Die renale Ausscheidung von cAMP/24 h oder cAMP/g Kreatinin wurde bei primärem Hyperparathyreoidismus (1° HPT) von verschiedenen Untersuchern gemessen. Die Resultate waren widersprüchlich. Wir bestimmten die renale Ausscheidung von cAMP/24 h oder cAMP/g Kreatinin bei 50 Patienten mit 1° HPT, 25 Patienten mit sekundärem (2°) HPT und 35 Kontrollpersonen. Die Patienten schieden im Mittel mehr cAMP/24 h oder cAMP/g Kreatinin aus als die Kontrollpersonen. Eine Überlappung mit dem Normalbereich wurde besonders bei reduzierter Kreatininclearance beobachtet. Da im Urin ausgeschiedenes cAMP zum Teil aus dem Plasma (glomeruläre Filtration) und zum Teil aus der Niere (tubuläre Produktion) stammt und die Anzahl der Tubuli von der GFR reflektiert wird, bezogen wir die renale Ausscheidung von cAMP auf die GFR (bestimmt als Kreatininclearance). Bei den Kontrollpersonen korrelierte die renale Ausscheidung von cAMP besser mit der Kreatininclearance als mit der Kreatininausscheidung. 45 von 50 Patienten mit 1° HPT und alle Patienten mit 2° HPT wiesen eine erhöhte renale Ausscheidung von cAMP/GFR auf. Die radioimmunologisch gemessenen Serumspiegel von Parathormon korrelierten bei 1° HPT besser mit der renalen Ausscheidung von cAMP/GFR als mit cAMP/g Kreatinin. Nach Entfernung von adenomatösen oder hyperplastischen Nebenschilddrüsen bei 1° HPT oder nach Infusion von Calcium bei 2° HPT fiel die Ausscheidung von cAMP/GFR auf normale oder erniedrigte Werte. 50 zusätzlich untersuchte Patienten mit Nephrolithiasis, die nicht durch 1° HPT bedingt war, wiesen keine Erhöhung der renalen Ausscheidung von cAMP/GFR auf. Die renale Ausscheidung von cAMP/GFR ist ein zuverlässigerer Indikator der Nebenschilddrüsenüberfunktion als die Ausscheidung von cAMP/Kreatinin.

    Notes: Summary The follow-up of 15 patients with differentiated cancer of the thyroid was analysed retrospectively. All patients had persistent or developed elevated thyroglobulin (Tg) plasma levels (〉10 ng/ml) after total thyroidectomy and 131 I therapy. There was no evidence of thyroid remnants or metastases in the 131 I whole body scan, bone scan, chest X-ray or clinical investigation. Nine of 15 patients developed local or distant metastases during a follow-up period of 3–39 months (mean=18 months). Only one of seven recurrences tested were 131 I positive, Tg values of 9 from 12 patients showed no correlation to the Thyroid-stimulating hormone (TSH) level. It is concluded that: (1) this group of patients is at high risk of developing a recurrence (2) new methods have to be developed for the localization of 131 I negative but Tg positive metastases.

Schwangerschaftswoche und im Blut von 37 Neugeborenen sowie deren Müttern bestimmt. Der mittlere rT3-Gehalt der Fruchtwasser lag bei 0,769±0,47 ng/ml; ein leichter Abfall der rT3-Konzentrationen bei steigendem Gestationsalter war statistisch nicht zu sichern. Aufgrund der großen Streuungen scheint das rT3 zur praenatalen Hypothyreosediagnostik ungeeignet. Die mittlere rT3-Konzentration im Nabelschnurblut von 2,62 ng/ml war gegenüber dem durchschnittlichen rT3-Spiegel der Mutter unter der Geburt von 0,34 ng/ml stark erhöht. Die mögliche Verwendung der rT3-Messung im Nabelschnurblut zum Hypothyreose-Screening wird diskutiert.
    Notes: Abstract 3,3′,5-triiodothyronine (reverse T3, rT3) was measured in 122 samples of amniotic fluid obtained between the 29th–40th weeks of gestation, and in the blood of 37 newborn and their mothers. The mean rT3 concentration in amniotic fluid was 0.769±0.47 ng/ml, with a slight decrease from the 29th–40th weeks of gestation which was not statistically significant. Because of the great scatter of rT3 concentrations in amniotic fluid, its estimation does not seem to be useful in prenatal diagnosis of hypothyroidism. The mean rT3 concentration in cord blood (2.62 ng/ml; range 1.4–4.9 ng/ml) was greatly elevated in comparison to the mean maternal level at delivery (0.34 ng/ml; range 0.08–0.69 ng/ml). The possible use of rT3 measurements in cord blood as a screening test for congenital hypothyroidism is discussed.

Bei den Müttern fanden sich hoch normale T3-und T4-Spiegel. Im Nabelschnurblut war das TSH erhöht (9,2±7,4 uU/ml), das T4 im Normbereich, während das T3 eindeutig im hypothyreoten Bereich (0,5±0,17 ng/ml) lag. Innerhalb der ersten 24 Lebensstunden normalisierten sich die T3-Spiegel in fast allen gemessenen Fällen. Einige noch hypothetische Erklärungsmöglichkeiten dueses Phänomens werden vorgeschlagen.
    Notes: Abstract Triiodothyronine (T3) and Thyroxine (T4) were measured in women after delivery, in the cord blood of their children and in the first days of life. Furthermore TSH was determined in the cord blood. Women immediately after delivery showed T3 and T4 concentrations in the upper normal range. In the cord blood T4 was normal, TSH elevated (9.2±7.4 uU/ml) and T3 in the hypothyroid range (0.5±0.17ng/ml). T3 increased to normal concentrations in most cases within 24 hrs; possible explanations of the low T3 levels under fetal conditions are discussed.

    Notes: Abstract. In the present study a cell culture model of primary human osteoblasts based on degrees of confluence was investigated by measuring basal and 1,25(OH)2D3stimulated levels of the osteoblast characteristic proteins alkaline phosphatase (AP), procollagen I-peptide (PICP), and osteocalcin (OC), as well as the corresponding gene expression. Primary osteoblast-like cell cultures from seven donors were treated in the second passage with 1,25(OH)2D3 (5 × 10−8 M for 48 hours) and investigated at four stages of confluence (stage I 50%, stage II 75%, stage III 100%, and stage IV 7 days postconfluence). In untreated cultures passing through the different stages of confluence, we saw a 1.8-fold increase of AP activity, a 2.3-fold increase of OC secretion, but a decrease of PICP levels to 0.36-fold. Gene expression showed only minor variation between the different confluence stages. 1,25(OH)2D3 did not significantly affect PICP production. Alkaline phosphatase protein was stimulated during proliferation until confluence, with no effect thereafter. Surprisingly, OC secretion and mRNA expression were stimulated in all four stages to the same absolute level independent of basal values. We conclude that our results correspond to other studies showing differentiation-stage dependent changes of basal levels of osteoblast-specific proteins. However, 1,25(OH)2D3 stimulation decreased the confluence-dependent difference for AP and abolished it for osteocalcin, thus leading to a more differentiated phenotype of the osteoblast. Therefore, 1,25(OH)2D3 stimulation might improve the reproducibility of results obtained at different confluence stages from cultures of clinical samples.
