Notes: Summary A 5-month-old boy died of progressive heart failure that started at the age of 3 months. Autopsy revealed a mitochondrial cardiomyopathy and a mitochondrial myopathy of the limb muscle and diaphragm. Cytochemically random defects of cytochromec oxidase were visualized by light and electron microscopy in the diaphragm and especially the heart muscle, the limb muscle showing a diffuse attenuation whereas the liver and kidneys reacted normally. The activities of NADH-dehydrogenase (complex I) and cytochromec oxidase (complex IV) were severely diminished (20% residual activity of controls) in the skeletal and heart muscle. In the heart, succinate cytochromec reductase (complex II/III) was additionally decreased to the same degree. Loss of cytochromec oxidase activity was based on a reduction of both mitochondrial and nuclear derived subunits in the heart and diaphragm as revealed by immunohistochemical analysis, whereas the limb muscle showed a normal immunoreactive protein content. The results illustrate heterogeneous tissue expression of respiratory chain enzyme defects and demonstrate that a cardiomyopathy may be the leading presentation of a mitochondrial disorder in early infancy.

Notes: Summary A report is given on a case of proliferative myositis in a 75 year old woman. By fine structural analysis it can be shown, that the charcateristic giant cells in proliferative myositis are mesenchymal cells with an intensive protein metabolism. They can be compared to fibroblasts; for a myogenic origin of these cells we found no evidence. Furthermore, various stages in the development and function of the proliferating cells were observed, by which the course of the disease can be explained.

Notes: Summary The paper presents the morphological findings and clinical data of 4 patients with veno-occlusive disease of the liver developing after immunosuppressive therapy with azathioprine and prednisolone. The pathogenesis and a possible causal relationship of hepatic veno-occlusive disease with long term azathioprine therapy are discussed.

Notes: Summary Clinicopathologic study of five cases of soft tissue tumors revealed distinct differences from skeletal Ewing's sarcoma in preferential localisation and mean age. The cases examined here are similar to cases described earlier as “extraskeletal Ewing's sarcomas”. They show light- and electronmicroscopical features analogous to skeletal Ewing's sarcoma. The term extraskeletal Ewing's sarcoma appears to be appropriate for this type of soft tissue tumor.

Notes: Summary A report is given of the clinical and autopsy findings of two men who died from malignant liver neoplasm following occupational exposure to vinyl chloride. The first patient was a 44-year-old man with an hemangiosarcoma of the liver, the second patient a 67-year-old man with an hepatocellular carcinoma. So far an hepatocellular carcinoma due to vinyl chloride has not yet been observed in man. Its occurrence, however, has been suggested from the results of animal experiments. The connection of hepatocellular carcinoma with exposure to vinyl chloride is discussed.

Notes: Summary The clinical and postmortem findings in a 26 year old man with Kearns-Sayre syndrome are described. In the last years of his life he suffered from cardiac arrhythmias and a congestive cardiomyopathy, dying of cardiac pump failure. The heart was enlarged, especially the left ventricle which was fibrotic and excessively dilated. Histological and fine structural investigation revealed an excessive loss of myofibrils and an increase of enlarged mitochondria with lamellar and atypically tubular cristae in widespread heart muscle cells. Mitochondrial anomalies were also observed in some cells of the conductive system. This patient thus suffered not only from a mitochondrial myopathy with ragged red fibers but also from a fatal mitochondrial cardiomyopathy. The anomalies observed in the mitochondria of the conductive system cells suggest that the well-known conductive abnormalities in patients with Kearns-Sayre syndrome might be at least partly caused by disturbed function of these mitochondria.

Notes: Summary Based on late postsurgical examinations of 954 patients, the etiology, distribution, frequency, and location of the disease in the hand are presented. In addition, the results of immunohistologic and electron-microscopic examinations for collagen structure and collagen type are presented. Biochemical tests on cell cultures proved types I and III to be predominant. Finally, the surgical procedure, postsurgical treatment, and surgical complications are demonstrated.
