    Description / Table of Contents: Summary This is the first case of a megakaryoblastic myelosis, diagnosed in its early phase by bone marrow biopsy, with clinical follow up study and confirmation by autopsy findings. In the initial stage megakaryoblastic and erythroblastic proliferation in the bone marrow, later a few atypical megakaryoblasts and erythroblasts as well as hematopoietic stem cells in the peripheral blood, in association with constant pancytopenia. At the end stage of a 18 month clinical course almost exclusively megakaryoblasts in the bone marrow. Autopsy findings with generalized involvement of spleen, lymphnodes and liver by hematoblastosis without transgression of the confinements of individual organs. Quantitative and qualitative evaluations revealed a close relationship between proliferation of megakaryoblasts/erythroblasts and endothelial cells of the blood sinus of the bone marrow, which together with other observations points to a neoplastic proliferation of the later cells as basis for this disease. These findings, moreover, may indicate a post-partum, hematopoietic omnipotence of these endothelial cells.
    Notes: Zusammenfassung Erste klinisch-histologische und durch Obduktion gesicherte Verlaufsbeobachtung einer megakaryoblastischen Myelose mit anfänglich gemeinsamer megakaryoblastisch-erythroblastischer Proliferationssteigerung im Knochenmark. Im Verlauf Ausschwemmung von einzelnen atypischen Megakaryoblasten, Erythroblasten und undifferenzierten hämoblastischen Stammzellen ins Blut bei anhaltender peripherer Pancytopenie. Ende als fast reine Megakaryoblastose mit autoptischem Nachweis von systemisiertem Befall von Knochenmark, Milz, Lymphknoten und Leber, an keiner Stelle die Organgrenzen überschreitend. Die enge Beziehung zwischen megakaryoblastischer und erythroblastischer Zellproliferation und den Sinusendothelien des Knochenmarkes, bestätigt durch quantitative und qualitative Auswertung, läßt zusammen mit anderen Überlegungen und weiteren Fallbeobachtungen den Ausgang der neoplastischen Störung vom Sinusendothel in Betracht ziehen. Diese Annahme erscheint als ein Argument für eine post-embryonale hämopoetische Pluripotenz dieser Zellen.

    Description / Table of Contents: Summary Wistar rats inoculated as newborns with polyoma virus, develop chiefly, almost exclusively, spindle cell sarcomas of the kidneys, and survive for few weeks only. Studies of the formal pathogenesis of these sarcomas demostrate primary multifocal growth with secondary confluence into sarcomatous areas. Pluricellular origin of initial sarcomas is suggested. This hypothesis is supported, by the early date of tumor manifestation following inoculation, the interval being too short for a major number of generation cycles necessary in case of development from single cells. Furthermore, correlation between dosis of virus-inoculum and size of tumors can best be explained by primary transformation of pluricellular target groups.
    Notes: Zusammenfassung Polyoma-Virus verursacht bei Wistar-Ratten fast auschließlich Spindelzellsarkome der Niere, die in wenigen Wochen zum Absterben der Tiere führen. Untersuchungen über die formale Pathogenese zeigten, daß die Sarkome primär multizentrisch entstehen und dann zu einem Knoten zusammenwachsen. Eine pluricelluläre Cancerisierung ist anzunehmen, weil die Zeitspanne zwischen Virusgabe und Tumormanifestation zu kurz für die Entwicklung aus einer Zelle ist; auch ist die Größe und Wachstumsgeschwindigkeit der Tumoren von der Virusdosis abhängig.

    Description / Table of Contents: Zusammenfassung Bei 5 von 39 Patienten wurde während Kreislaufschock eine Hypoglykämie beobachtet. Die Blutzuckerwerte waren auf 12–25 mg-%, in einem Fall unter 5 mg-% vermindert. Biochemische und histologische Befunde machen eine hepatogene Genese der Hypoglykämie wahrscheinlich. Aufgrund der Beobachtungen werden häufige Blutzuckerkontrollen und routinemäßige Glucoseinfusionen während Kreislaufschock empfohlen. Bei Hypoglykämie ist eine intensive Glucosetherapie indiziert.
    Notes: Summary Hypoglycemia was observed during shock in 5 of 39 patients. Blood sugar levels ranged from 12 to 22 mg% and below 5 mg% in one case. Biochemical and histological findings suggest hepatic origin of the hypoglycemia. Based on these findings repeated blood sugar determinations and routine glucose infusions in shock are recommended. Hypoglycemia is an indication for intensive glucose therapy.

    Description / Table of Contents: Zusammenfassung Nach subcutaner Inoculation neugeborener, nicht ingezüchteter Wistar-Ratten mit Polyoma-Virus entstehen fast nur zellreiche, faserarme Spindelzellsarkome der Niere. Die histologische Auswertung ergibt eine multizentrische Sarkombildung, wobei eine Dosis-Wirkungs-Beziehung zur Zahl der tumortiere, zur Zahl der multizentrischen Sarkomknoten, sowie zur Größe und zur Wachstumsgeschwindigkeit der Sarkome besteht. Besonders eindrucksvoll ist eine scheinbare Wachstumsruhe zwischen Virusinfektion bzw. Zelltransformation und mikroskopisch erfaßbarer Zellproliferation.
    Notes: Summary Non-inbred newborn Wistar-rats inoculated subcutaneously with Polyoma-Virus develop almost exclusively highly cellular spindle cell sarcomas of the kidneys with only scanty fibrous stroma. Histological evaluation of these sarcomas shows multifocal growth, with correlations between the dosage of virus-inoculum and the response of the animal, the percentage of tumor-animals, the number of sarcomas in one kidney, and the size and growth rate of tumors. The latency period between virus infection, that is, cell transformation, and the first microscopic sign of cell proliferation is impressive.

    Notes: Summary A 77-year-old man with chronic obstructive pulmonary disease was treated with low-dose methotrexate (7.5–15 mg per week). After 15 months a diagnosis of urothelial carcinoma of the bladder was made; after a further 6 months pneumonitis and pancytopenia developed. The patient died due to massive pulmonary hemorrhage. A malignant teratoma was diagnosed in a 65-year-old asthmatic man 16 months after initiation of methotrexate therapy (15 mg per week). The patient died 4 months later due to fulminant progression of the neoplasm. A third malignant neoplasm (dermal squamous cell carcinoma) was seen in a 64-year-old woman with rheumatoid arthritis after 13 months treatment with 7.5 mg methotrexate per week. These three cases, while obviously not proving a causal relationship between long-term treatment with low-dose methotrexate and development of malignant neoplasm, do call for stringent treatment criteria, close surveillance, and prospective studies.
