Zusammenfassung Die medikamentöse Thromboembolieprophylaxe (Intensivstation: 10 000 E Heparin/24 h, Normalstation: 1 x tgl. Embolexe NM) ist nicht mit einem perioperativen Risiko - erhöhte Infektionsrate, Wundhaematome - verbunden. Bei 1209 Sektionen fanden wir 209 Thromboembolien, davon 70 letale Lungenembolien (Kliniksletalität 4,2%, Obduktionsrate 58%). Mit Einführung der medikamentösen Prophylaxe sank die letale Lungenembolierate im Sektionsgut von 16,7 über 5,8 auf 1,3% ab. Die Thrombosen waren uni- und multiloculär verteilt, im Vordergrund stand die untere Extremität, in 25% das Zustromgebiet der Cava superior. Die klinische Diagnose einer Beinvenenthrombose wurde nur bei 9, die einer letalen Lungenembolie nur bei 11 der 70 Patienten gestellt. Die generelle medikamentöse Prophylaxe hat im eigenen Krankengut die Rate der letalen Lungenembolie von 1,9% auf 0,1% senken lassen. Eine verspätet eingeleitete Prophylaxe kommt der Unterlassung gleich. Das Risiko einer Thromboembolie ist auch bei Eingriffen unter 45 min Dauer vorhanden und schliesst keine chirurgischen Erkrankungen aus.

Summary Medical prophylaxis for the prevention of thromboembolism (intensive care unit: 10,000 IU heparin/24 h, normal ward: 1 x daily Embolex NM) carries no perioperative risk, increased infection rate, or rate of wound hematoma. In 1,209 autopsies, we found 209 thromboembolisms: 70 lethal pulmonary embolisms (clinical lethality: 4.2%; autopsy rate: 58%). With the introduction of medical prophylaxis the rate of lethal pulmonary embolisms in autopsy material decreased from 16.7% to 5.8% and eventually 1.3%. The thromboses were uni- and multilocular localized in the lower extremities, and in 25% in areas drained by the vena cava superior. A clinical diagnosis of thrombosis of the lower extremities was made only 9 times, and of a lethal pulmonary embolism in 11 of the 70 patients. In the patients of the surgical division, the rate of lethal pulmonary embolism decreased from 1.9% to 0.1% by means of general medical prophylaxis. Delayed prophylaxis showed no reduction in thromboembolism. The risk of thromboembolism is neither dependent on operating time nor on surgical disease.
