Summary In 90 adult patients with chronic obstructive lung disease the concentrations of chlorides, sodium, potassium and calcium in bronchial secretions were determined and the values compared with the electrolyte concentrations found by other authors in mucoviscidosis, bronchiectasis and healthy persons. In contrast to the results in infantile mucoviscidosis no different behaviour of electrolyte concentrations in the bronchial secretions was observed between a group of adult patients with chronic bronchitis and elevated sodium concentration of thermal sweat and a control group without abnormalities of sweat sodium concentration. An influence of enzymatic and hormonal factors on sodium concentration of bronchial secretions, such as increased carboanhydrasis and aldosterone activity following hyperkapnia and/or pulmonary hypertension, could not be confirmed. However, the comparison of patients with chronic bacterial bronchitis and of patients with paroxysmal bronchial asthma shows an elevation of chloride and sodium concentration of bronchial secretions and thermal sweat in the first group. A causal relationship between the elevated electrolyte concentration of bronchial secretions and sweat, as well as in the secretions of other exocrine glands, and a decreased adrenocortical activity on electrolyte metabolisme in patients with chronic bacterial bronchitis is discussed.

Zusammenfassung Bei 90 Patienten mit chronischen obstruktiven Bronchialerkrankungen wurde die Chlorid-, Natrium-, Kalium- und Calciumkonzentration des Bronchialsekrets bestimmt. Die Ergebnisse wurden mit den Befunden anderer Autoren bei Mucoviscidosis, Bronchiektasen und bei Lungengesunden verglichen. Im Gegensatz zu den von anderen Autoren bei der Mucoviscidosis erhobenen Befunden wurde bei Erwachsenen mit chronischer Bronchitis und erhöhter Natriumkonzentration des Schweißes kein von der Kontrollgruppe abweichendes Verhalten der Elektrolytkonzentrationen des Bronchialsekrets festgestellt. Eine Beeinflussung der Natriumkonzentration des Bronchialsekrets durch enzymatische und hormonelle Faktoren auf der Basis der Hyperkapnie und pulmonalen Hypertonie ist nicht nachweisbar. Dagegen fand sich bei Patienten mit chronischer bakterieller Bronchialinfektion im Vergleich zu Patienten mit krisenhaft verlaufendem Asthma bronchiale eine höhere Chlorid-und Natriumkonzentration des Bronchialsekrets und des Schweißes. Als Ursache der gleichgerichteten Steigerung der Natriumkonzentration in verschiedenen Drüsensekreten bei chronischen Entzündungsprozessen werden übergeordnete, mineralstoffwechselwirksame hormonelle Faktoren vermutet.
