    Notes: Summary We report the first case of acquired immunodeficiency-syndrome (AIDS) with Kaposi's sarcoma which at the same time has also a changing biclonal gammopathy (initial: double IgG-paraproteinaemia, lateron IgA-IgG Double-paraproteinaemia) due to different lymphocyte clones. The relationship of a B-cell neoplasia to a concurrent disease of T-cells is discussed.

    Notes: Abstract Sera from 32 homosexual men were studied for the presence of antibodies against humanT-lymphotropic virus type III (HTLV III) and acidlabile interferon (IFN) alpha. Infection with HTLV III was found to be associated with the presence of serum IFN. IFN was detected in 74% of sera from male homosexuals with HTLV III antibody, but in only 6% of sera from antibody-negative individuals. More than 80% of sera from HTLV III-infected patients with the acquired immune deficiency syndrome (AIDS) or pre-AIDS conditions (generalized lymphadenopathy or the AIDS-related complex) were positive for IFN, while IFN was not present in sera from healthy homosexual men with HTLV III antibody. In conclusion, the presence of serum IFN may be predictive of the development of AIDS or pre-AIDS conditions in male homosexuals exposed to HTLV III.

    Notes: Abstract Monoclonal antibodies with specificities for subsets of human leukocytes have been used for the characterization of interferon (IFN)γ-producing cells. The production of IFNγ was demonstrated to be a function of OKT3+ T lymphocytes. The capacity to secrete IFNγ was not restricted to the OKT4+ or the OKT8+ T-cell subset. BA-1+ B lymphocytes and Leu7+ natural killer cells did not contribute to the production of IFNγ. Ia+, OKM1+ monocytes served an auxiliary function in the production of IFNγ. The requirement for accessory monocytes, however, was not absolute, because monocyte-free preparations of long-term cultured IL2-dependent T lymphocytes retained the capacity to secrete IFNγ.

    Notes: Abstract We have studied the prevalence of antimicrobial resistance ofStreptococcus pneumoniae in Southwest Germany. One hundred seventy-four clinical isolates of pneumococci collected from hospitalized patients between October 1992 and April 1994 were used for MIC determinations. MICs for penicillin, ceftriaxone, erythromycin, and rifampicin were assessed by the E test. Eleven of the 174 strains (6.3%) were intermediately resistant to penicillin (MIC between 0.1 and 1.0 µg/ml) and four of the 174 strains (2.3%) were intermediately resistant to ceftriaxone (MIC between 0.1 and 1.0 µg/ml). All four isolates with a reduced susceptibility to ceftriaxone also demonstrated intermediate resistance to penicillin. Six of the 174 strains (3.5%) were highly resistant (MIC ⩾8 µg/ml) to erythromycin. Resistance to rifampicin was not observed. Our results demonstrate that pneumococcal resistance to penicillin and erythromycin has increased markedly in Germany over the last decade. Our findings underline the need for continuous surveillance of antimicrobial resistance ofStreptococcus pneumoniae.

    Description / Table of Contents: Zusammenfassung Hämatologische und immunsuppressive Wirkung von einmaligen Stickstofflost(HN3)-Gaben wurde an 20 Hunden untersucht. HN3 verursachte bei allen Tieren eine starke Erniedrigung der peripheren Blutwerte. Bei Hunden, die die akute gastrointestinale Toxizität von HN3 überlebten, war die Erholung der Gesamtleukozyten am Tag 15 vollständig. Die Wiederherstellung der Ausgangswerte von Thrombozyten und Lymphozyten nahm einen protrahierteren Verlauf. Granulopoetische Vorläuferzellen (CFU-C) im Knochenmark wurden durch in vitro Kulturen bestimmt. Die CFU-C-Konzentrationen waren einen Tag nach der Behandlung mit HN3 deutlich vermindert, zeigten aber selbst nach nahezu letalen Dosen schnelle Regeneration zu Normalwerten innerhalb 4 bis 7 Tagen. Die reduzierte Fähigkeit von Hundelymphozyten, nach Stimulation mit Mitogenen in vitro zu proliferieren, wies auf eine langanhaltende Beeinträchtigung der zellulären Immunfunktion bei HN3-behandelten Hunden hin. Die humorale Immunfunktion war ähnlich betroffen, wie sich aus der verminderten und verzögerten Bildung von Antikörpern gegen Schaferythrozyten ergab. Zusammenfassend lassen unsere Ergebnisse eine unterschiedliche Wirkung von HN3 auf lymphatische und myeloische Vorläuferzellen vermuten.
    Notes: Summary Hématologic and immunosuppressive effects of single doses of nitrogen mustard (HN3) were evaluated in 20 dogs. HN3 caused profound depression of peripheral blood counts in all animals. Recovery of total white blood cell counts in dogs surviving the acute gastrointestinal toxicity of HN3 was complete by day 15. Recovery of platelet and lymphocyte counts to initial levels took a more prolonged course. Granulopoietic progenitor cells (CFU-C) in the bone marrow were assayed by an in vitro culture system. Concentrations of CFU-C were markedly decreased one day after HN3-treatment, but showed at near-lethal doses rapid restoration to normal values within 4 to 7 days. Cellular immune function of HN3-treated dogs was impaired for prolonged periods as indicated by the reduced capacity of canine lymphocytes to prolifferate in vitro in response to stimulation with mitogens. Humoral immune function was similarly affected as determined by the depressed and delayed antibody formation against an intravenous challenge with sheep red blood cells. In conclusion, our results suggest a different effect of HN3 on lymphoid and myeloid precursor cells.

    Description / Table of Contents: Zusammenfassung Wir haben Messungen der Serum-Interferon-Spiegel und der Interferon-Bildung durch Leukozytenin vitro bei homosexuellen Männern mit AIDS oder generalisierter Lymphadenopathie und bei gesunden homosexuellen oder heterosexuellen Männern durchgeführt. Säurelabiles Interferon alpha wurde im Serum von zwei Patienten mit AIDS und bei drei von neun Patienten mit Lymphadenopathie, aber nicht im Serum gesunder homosexueller oder heterosexueller Kontrollpersonen gefunden. Das Vorhandensein von Serum-Interferon bei Patienten mit AIDS oder Lymphadenopathie korrelierte mit einer deutlichen Verminderung der Interferon-alpha-Synthese ihrer Leukozyten nach Induktion mit Virus oder Tumorzellen, beeinflußte jedoch nicht signifikant die Bildung von Interferon gamma nach Induktion mit Lektin oder bakteriellem Enterotoxin. Unsere Ergebnisse lassen eine inverse Korrelation zwischen Serum-Interferon alpha und der Menge des in Leukozytenkulturen gebildeten Interferon alpha bei männlichen Homosexuellen mit AIDS oder generalisierter Lymphadenopathie vermuten.
    Notes: Summary We measured serum interferon (IFN) levels and leukocyte IFN productionin vitro in homosexual men with AIDS or generalized lymphadenopathy and in asymptomatic homosexual or heterosexual men. Acid-labile interferon alpha was detected in the serum of two patients with AIDS and in three out of nine patients with lymphadenopathy, but not in the sera of healthy homosexual or heterosexual controls. The presence of serum interferon in patients with AIDS or lymphadenopathy was correlated with a marked deficiency of their leukocytes to synthesize interferon alpha in response to viruses or tumor cells, but did not significantly interfere with the production of interferon gamma in response to lectin or bacterial enterotoxin. Our results suggest an inverse correlation between serum interferon alpha and the amount of interferon alpha produced in leukocyte cultures in male homosexuals with AIDS or generalized lymphadenopathy.

    Description / Table of Contents: Zusammenfassung Der Nachweis von Human Immunodeficiency Virus Typ I (HIV I) Antigen und die Entwicklung von Anti-HIV I war bei einem 27 Jahre alten Homosexuellen mit akuter Enzephalopathie verbunden. Die Untersuchung von sequentiell entnommenen Patientenseren ergab, daß zirkulierendes Interferon (IFN) alpha gleichzeitig mit dem Auftreten von HIV-I-Antigen, aber vor der Entwicklung von Anti-HIV I nachgewiesen werden konnte. Serum IFN alpha war nur für einen begrenzten Zeitraum und nicht nach der Rückbildung der neurologischen Symptome nachweisbar. Die mögliche Beteiligung zirkulierender antiviraler Aktivität an den klinischen Manifestationen der akuten Infektion mit HIV I wird diskutiert.
    Notes: Summary Acute encephalopathy was associated with the presence of human immunodeficiency virus type I (HIV I) antigen and seroconversion to anti-HIV I in a 27-year-old homosexual man. Examination of consecutive sera from the patient revealed circulating interferon (IFN) alpha which became detectable with the appearance of HIV I antigen but before development of anti-HIV I. Serum IFN was present for only a limited time and was not demonstrable after neurological symptoms resolved. It may be speculated that circulating antiviral activity contributed to the clinical manifestations of acute HIV I infection.
