Zusammenfassung Die Pathogenese der IPP ist bis heute nicht geklärt. Histopathologische Untersuchungen zeigen, daß der Kollagenstoffwechsel eine Rolle spielt. Dies veranlaßte uns zur genauen Analyse der Kollagenstoffwechselparameter bei IPP. Bei 11 Patienten mit IPP und 11 Kontrollpersonen wurden Kollagenstoffwechselparameter [Prokollagen III (P III P), PMN-Elastase (PMN-E), Fibronektin (FN), Fibronektinrezeptor (FN-R) und α-1-Proteinaseinhibitor (α-1-PI)] im kavernösen und peripheren Blut bestimmt. PMN-E, P III P, FN und FN-R waren in den beiden Untersuchungsgruppen nicht unterschiedlich. Die peripher venösen unterschieden sich nicht von den kavernösen Blutproben. Sowohl im peripher venös (158,55 mg/dl) als auch im kavernös entnommenen Blut (168,29 mg/dl) war die Konzentration des α-1-PI signifikant niedriger als in der Kontrollgruppe (214,82 mg/dl). Die α-1-PI Erniedrigung der IPP Patienten ist signifikant und liegt unterhalb des Normalwertbereichs für die gesunde Population. Ein Mangel an α-1-PI kann zur Aktivitätserhöhung der Proteasen und zum Umbau des Kollagengewebes führen. Möglicherweise kommt es zusammen mit einem lokal wirksamen Einflußfaktor schließlich zur dystrophen Kalzifikation.

Summary The excact pathogenesis of Peyronie's Disease (PD) is still unknown. Histopathological investigations suggest, that the metabolism of the collogenious tissue plays an important role. Therefore, we started a prospective study in order to evaluate the metabolic situation of 11 patients with PD and 11 healthy controls. We mesured biochemical parameters of penile and venous blood, wich correspond to the collagen metabolism (procollagen III [P III P], PMN-elastase [PMN-E], fibronectin [FN], fibronectinreceptor [FN-R] und α-1-proteinase-inhibitor [α-1-PI]). There was no difference in PMN-E, P III P, FN and FN-R in all samples. There was no difference in penile and venous blood as well. But we found out a highly significant lower concentration of α-1-PI in patients with PD (158,55 mg/dl) than in controls (214,82 mg/dl). A deficiency of α-1-PI can lead to a higher activity of serum proteases, such as PMN-E. This can result in a change in collagen metabolism by an increased synthesis of collagen type III, that is found in PD. This general pathophysiologic factor may be competed by an additional local alteration of the tissue.
