Zusammenfassung In einer multizentrischen Studie, bei der die orale Einzeittherapie von Fosfomycin Trometamol (3 g als Fosfomycin) mit der von Co-trimoxazol (1,92 g) oder Ofloxacin (200 mg) verglichen wurde, wurden so viele Erreger wie möglich zur Auswertung an ein Zentrallabor geschickt. Die Erreger wurden identifiziert und ihre Empfindlichkeit als minimale Hemmkonzentration (MHK) von Fosfomycin, Trimethoprim allein oder in Kombination mit Sulfamethoxazol, Ofloxacin, Ampicillin, Amoxicillin in Kombination mit Clavulansäure und Cephadroxil bestimmt. Die Elimination (Urinkeimzahl 〈 103/ml eine Woche nach Einzeittherapie) der ursprünglichen Erreger wurde entsprechend der Species und der MHK des verabreichten Antibiotikums analysiert. Insgesamt wurden die Urinkulturen von 349 Patientinnen ausgewertet.Escherichia coli war die häufigste Species, gefolgt von Staphylokokken undProteus mirabilis. Enterokokken fanden sich meistens nur in Mischkulturen. Bei den 284 Patientinnen, die eine Monoinfektion aufwiesen, wurden die ursprünglichen Erreger bei 87,1% durch Fosfomycin Trometamol, bei 88,9% durch Co-trimoxazol und bei 86,4% durch Ofloxacin eliminiert. Die MHK der fünf Antibiotika und der zwei Kombinationen ergaben bei den 253 getesteten Erregern, daß keinE. coli- Stamm gegen Ofloxacin resistent war. Drei Stämme (MHK=128 mg/l) waren gegen Fosfomycin, 3,6% gegen Co-trimoxazol, 6,2% gegen Trimethoprim, 8,8% gegen Ampicillin und 5,7% gegen Amoxicillin/Clavulansäure resistent. Die Eliminationsraten in Korrelation zu den MHK des verabreichten Antibiotikums waren nach Gabe von Fosfomycin Trometamol bis zu einer MHK von 64 mg/l gleich gut. Bei höheren MHK wurden noch zwei von drei Stämmen durch Fosfomycin Trometamol eliminiert. Für Co-trimoxazol und für Ofloxacin standen keine intermediären oder resistenten Stämme für die Auswertung der Korrelation zur Verfügung. Innherhalb der MHK-Bereiche waren die Eliminationsraten für beide Antibiotika gleich gut. Aus der Studie ergibt sich, daß Fosfomycin Trometamol, Ofloxacin und Co-trimoxazol als Antibiotika der ersten Wahl für die Einzeittherapie der akuten unkomplizierten Harnwegsinfektion bei Frauen eingesetzt werden können.

Summary In a multicentric study comparing oral single-dose therapy of fosfomycin trometamol (3 g as fosfomycin) versus co-trimoxazole (1.92 g) or ofloxacin (200 mg) as many as possible of the pathogens were sent to and analysed in a central laboratory. The pathogens were identified and minimal inhibitory concentrations (MIC) of fosfomycin, trimethoprim alone and in combination with sulfamethoxazole, ofloxacin, ampicillin, amoxicillin combined with clavulanic acid, and cephadroxil were determined. The eradication of pathogens (cfu 〈 103/ml at one week after single-dose therapy) was analysed according to species and MIC of the antibiotic used. Urine cultures of 349 patients were analysed.Escherichia coli was the predominating species followed by staphylococci andProteus mirabilis. Enterococci were mostly found in mixed culture. Baseline pathogens of monoinfections were eradicated in 87.1%, in 88.9% and in 86.4% of 284 patients treated with fosfomycin trometamol, co-trimoxazole and ofloxacin, respectively. The MICs of the five antibacterial agents and the two antibiotic combinations for 253 baseline pathogens showed that of theE. coli strains none was resistant to ofloxacin, three (MIC=128 mg/l) were resistant to fosfomycin, 3.6% to co-trimoxazole, 6.2% to trimethoprim, 8.8% to ampicillin, and 5.7% to amoxicillin/clavulanic acid. The eradication rates according to the MICs of the corresponding drugs showed equally good eradication rates for fosfomycin up to an MIC of 64 mg/l. Above this level two out of three strains were also eradicated by fosfomycin trometamol. For co-trimoxazole and ofloxacin no intermediately sensitive or resistant strains were found. Within the range of MICs found there were equally good eradication rates for both antibacterial agents. It was concluded that fosfomycin trometamol, ofloxacin and trimethoprim plus sulfamethoxazole can be used as first line antibiotics for single-dose therapy of acute uncomplicated UTI in female patients.
