    Notes: Abstract Background Sclerosing peritonitis (SP) is a serious complication of chronic ambulatory peritoneal dialysis (CAPD) that is characterized by thickened peritoneal membranes, leading to impaired ultrafiltration and intestinal obstruction. The aim of this study was to analyze the radiographic signs of SP in symptomatic patients and to compare them with the histological severity of the disease. Methods In 14 patients with symptoms of intestinal obstruction related to SP, plain films of the abdomen, upper GI follow-through examinations, ultrasonography, and computed tomography were evaluated retrospectively. Imaging findings such as bowel motility, bowel wall and peritoneal thickness, and the presence of intraperitoneal fluid collections were correlated to the histological degree of the fibrosis. Results Signs of intestinal obstruction and disturbed motility were present in all cases. In addition, patients with histologically severe SP had loculated fluid collections, thickening of the bowel wall and/or peritoneum, peritoneal calcifications, and thickened peritoneal membranes, and suffered more often from postoperative complications. Conclusion The detection of characteristic radiographic signs in CAPD patients presenting with symptoms of intestinal obstruction may suggest the presence of severe SP and should lead to cautious surgical interventions.

    Notes: Summary In a prospective study circulating immune complexes (CIC) were analyzed before and serially after renal transplantation in 141 consecutive patients. CIC were measured using the Raji cell assay as originally described by Theofilopoulos and Dixon. The amount of CIC was expressed as µg heat aggregated human immunoglobulin G (IgG) equivalent/ml serum. The upper limit of normal sera was 25 µg/ml. The values are expressed as geometric means (−1 SD/+1 SD). In 86 of 133 rejection episodes a renal biopsy was performed and the histopathologic changes were semiquantitatively assessed and classified in a cellular or vascular type of rejection. Before transplantation CIC were detected in 104 of 141 patients (73.8%) and the mean value was 65.6 (27.8–154.9) µg/ml. The level of CIC was positively correlated with the number of grafts (r:0.43;P〈0.01) and the occurrence of chronic active hepatitis (r:0.31;P〈0.01). No correlation was found between CIC and the underlying kidney disease, the number of blood transfusions prior to transplantation, and the pre-existing lymphocytotoxic antibodies. Graft survival and number of rejection episodes were not influenced by the level of CIC prior to transplantation. After transplantation CIC were elevated in 60 patients (41%), appeared transiently in 49 patients (35%) and were never detectable in 32 patients (23%). In patients with a graft survival ≦11 months the average and peak post-transplant CIC levels were significantly higher than patients with a graft survival of 12 months: 64.4 (21.8–191.0); 87.7 (26.0–295.8) versus 39.6 (18.4–85.3); 56.8 (21.0–150.1) µg/ml;P〈0.01. There was a positive correlation between CIC and serum creatinine in the post-transplant period (P〈0.001). The histopathologic severity and morphological type of rejection did not correlate with CIC. In patients without rejection episodes CIC were significantly lower: 41.2 (39.6–42.9) than patients with rejection episodes: 61.8 (56.2–68.0);P〈0.05.

    Description / Table of Contents: Abstract Geostatistical analyses employed in the prospecting and for exploration of marine mineral resources, must be aimed at particular problems and goals. The quality of navigation, sampling, mining technology and the profitability of the project strongly influence such analyses. The studies presented here, have two main objectives, namely, to estimate the reserves of manganese nodules and to determine in what order the explored areas should be mined. Several geostatistical methods were used. These were designed partly to supplement one another, but mostly to create a sequence of analytical steps which could be applied to any typical marine exploration project.
    Notes: Zusammenfassung Sollen im Rahmen von Prospektions- und Explorationsvorhaben auf marine Rohstoffe geostatistische Untersuchungen durchgeführt werden, so haben sich diese an den speziellen Problemen und Zielsetzungen zu orientieren. Dabei spielen Fragen der Navigation, der Probennahme, der Förderungstechnik und der Wirtschaftlichkeit eine wichtige Rolle. Im vorliegenden Falle war das Ziel der Untersuchungen nach der Schätzung der Vorräte an Mangan-Knollen die Erstellung einer Rangordnung der Explorations-Areale für den Abbau. Hierzu wurden verschiedene Verfahren der Geostatistik eingesetzt, die sich zum Teil gegenseitig zu ergänzen haben, im Großen gesehen jedoch eine Methoden-Folge bilden, die als typisch für Explorations-Projekte auf marine Rohstoffe angesehen werden kann.

    Notes: Abstract Therapy of steroid-dependent idiopathic nephrotic syndrome is often unsatisfactory. Since 1986 we have treated nine children (six male and three female), aged 3–16 years, with cyclosporin A (CsA) during 2.0–5.2 (median 3.1) years. All had minimal change disease on renal biopsy and had previously received cyclophosphamide. Mean daily dosage of CsA was 4.1 mg/kg (range 2.7–5.8) and mean whole blood trough level was 220ng/ml (range 141–271). The relapse rate decreased from 3.4/patient year before CsA to 0.55 on CsA. Discontinuation of CsA or reduction below 2 mg/kg daily was always followed by a relapse. The overall relapse rate, including the period with very low-dose CsA, was 0.95/patient year. Four patients required additional low-dose alternate-day prednisone. Repeat renal biopsy showed minimal change disease in eight patients and focal segmental glomerulosclerosis in one; CsA-toxicity was mild in two and moderate in one. The latter was the only patient with slightly reduced glomerular filtration rate. Two boys with delayed puberty spontaneously matured and reached expected final height. We conclude that long-term low-dose CsA is very effective and steroid-sparing. Its use is justified in selected patients, particularly in those with numerous relapses and in male patients before and during puberty, as long as renal function and CsA-toxicity are carefully monitored.
