Zusammenfassung Aortographische Erfahrungen haben ergeben, daS ein doppelseitiger Verschluß der Ärteria ilica interna durch eine Arteriosclerosis obliterans bei fehlendem Ersatzkreislauf zu einer Impotentia coeundi führen kann. Erhalten die Corpora cavernosa kein Blut, ist eine Erektion unmöglich. Eine Analyse von 1500 Aortogrammen mit 2170 Extremitäten wies eine kombinierte Verschlußhäufigkeit der Arteria ilica interna in 21,8% und isoliert in 10,2% nach. Bericht über einen 44jährigen Patienten mit einer seit 2 Jahren bestehenden Impotenz; Wiederherstellung einer normalen Vita sexualis durch Ausräumung der Arteria ilica interna, beiderseits; Bestätigung dieses Vorgehens an weiteren 50 Eingriffen. Konsequenz: Die Beurteilung der Impotentia coeundi beginnt mit einer Darstellung der Ilicalarterien durch ein Aortogramm. Sind die Arteriae ilicae internae verschlossen, ist jede konservative, speziell hormonelle oder psychotherapeutische Behandlung sinnlos. Angezeigt ist allein eine rekonstruktive Gefäßoperation.

Summary Aortographic experience has demonstrated, that a bilateral occlusion of the arteria ilica interna can lead to an impotentia coeundi in the absence of a collateral circulation, through an ateriosclerosis obliterans. If the corpora cavernosa receive no blood, an erection is impossible. An analysis of 1500 aortogrammes with 2170 extremities disclosed a combined occlusion incidence of the arteria ilica interna, in 21.8% and isolated in 10.2%. A report on a 44 year old patient with a 2 year history of impotence: restoration of a normal sexual life by bilateral clearing of the arteria ilica interna; confirmation of this process in a further 50 operations. Consequence: The assessment of impotentia coeundi begins with a presentation of the ilical arteries by means of an aortogramme. If the arteriae ilicae internae are occluded, any form of conservative, special hormonal or psychotherapeutic therapy is senseless. The only indication is a reconstructive vascular operation.
