    Notes: Abstract. In western European countries most blunt chest traumas are associated with motor vehicle and sport-related accidents. In Switzerland, 39 of 10,000 inhabitants were involved and severely injured in road accidents in 1998. Fifty two percent of them suffered from blunt chest trauma. According to the Swiss Federal Office of Statistics, traumas represented in men the fourth major cause of death (4 %) after cardiovascular disease (38 %), cancer (28 %), and respiratory disease (7 %) in 1998. The outcome of chest trauma patients is determined mainly by the severity of the lesions, the prompt appropriate treatment delivered on the scene of the accident, the time needed to transport the patient to a trauma center, and the immediate recognition of the lesions by a trained emergency team. Other determining factors include age as well as coexisting cardiac, pulmonary, and renal diseases. Our purpose was to review the wide spectrum of pathologies related to blunt chest trauma involving the chest wall, pleura, lungs, trachea and bronchi, aorta, aortic arch vessels, and diaphragm. A particular focus on the diagnostic impact of CT is demonstrated.

    Notes: Abstract We compared the value of T2-weighted and Gd-DOTA-enhanced T1-weighted images for the detection and characterisation of 33 small renal masses (14 clear cell carcinomas, 6 angiomyolipomas, 3 angiomyomas, 4 adenomas, 3 papillary carcinomas, 3 oncocytomas, 1 haemorrhagic cyst). Dynamic enhanced MRI was performed to study the tumoral vascular supply (19 cases). MRI depicted all the masses more than 1 cm in diameter, but missed all the lesions less than 1 cm (4 false-negative). The results of T2-weighted images and Gd-DOTA-enhanced images were similar as regards detection; however, Gd-DOTA-enhanced images depicted more clearly the tumours smaller than 2 cm (11 cases). MRI enabled the characterisation of only 3 masses (2 angiomyolipomas, 1 haemorrhagic cyst). New MRI features are described for oncocytomas (low signal intensity on T1-weighted images, high signal intensity on T2-weighted images, early and marked enhancement on dynamic enhanced MRI). Dynamic enhanced MRI did not contribute to the differentiation of benign from malignant tumours.

    Notes: Abstract. The aim of this study was to describe the intrahepatic hemodynamic modifications induced by right portal vein embolization (RPVE) using Doppler ultrasound. Eighteen patients with hepatocellular carcinoma (n = 8), liver metastases (n = 9), or multiple adenomas, underwent RPVE 1 month before right hepatectomy in order to increase the size of future remnant liver. Doppler ultrasound was performed before and 1 month after RPVE. The portal vein flow and the hepatic artery resistive index in right and left lobes (segments V and III) were calculated. We recorded simultaneously artery and portal vein of segment III to measure the arterioportal ratio calculated as follows: (maximal arterial systolic velocity minus maximal portal vein velocity)/maximal arterial systolic velocity. Results were compared in cirrhotic patients (group A) and in healthy liver patients (group B). In both groups, portal flow was not significantly modified following RPVE. In the left lobe, in both groups the hepatic artery resistive index was not significantly modified. In group B, the arterioportal ratio decreased significantly (0.71 ± 0.18 and 0.42 ± 0.23, respectively, before and after embolization; p 〈 0.01), whereas it was not statistically modified in group A (0.75 ± 0.17 and 0.69 ± 0.14, respectively, before and after embolization). The right hepatic arterial resistive index decreased significantly in both groups after embolization (0.74 ± 0.07 to 0.66 ± 0.07, p 〈 0.05; and 0.66 ± 0.07 to 0.61 ± 0.06, p 〈 0.05, respectively, before and after RPVE for groups A and B). Total portal flow was unchanged after RPVE (750 ± 337 ml/mn and 696 ± 231 ml/mn, respectively, before and after RPVE). The hepatic artery resistive index was unchanged before and after embolization in the left lobe (0.75 ± 0.13 and 0.74 ± 0.14, respectively), but significantly decreased in the right lobe (0.7 ± 0.08 and 0.62 ± 0.06 respectively, p 〈 0.05). The left arterioportal ratio decreased significantly from 0.76 ± 0.17 to 0.52 ± 0.23 after PVE, p 〈 0.02). Our study confirms that right portal occlusion induces a decrease in hepatic artery resistive index in the right lobe and does not modify total portal flow. The left and right lobes of the liver have separate arterioportal regulation.

    Notes: Abstract A review of self-injurious behavior (SIB) indicates several biological variables that should be considered in future research. A current hypothesis, that SIB is reinforced by attention, receives support but inadequately accounts for evidence that SIB can arise in the absence of reinforcement, is often difficult to modify, and has features unlike those of conditioned behavior. Alternative possibilities are suggested. Neurological damage is common in populations displaying SIB and may produce inadequate pain perception or avoidance. Stereotyped behavior, also common in these populations, may also be related to SIB. Stress and isolation, both strongly correlated with SIB, may evoke this behavior by altering levels of biogenic amines and affecting the probability of aggressive responses. Finally, SIB may have been favored in evolution by group and kin selection, as the loss of a damaged individual may actually favor his genotype through promotion of relatives. These alternatives are not necessarily mutually incompatible nor do they preclude a degree of flexibility in SIB. SIB may result from the interaction of several variables that independently increase its probability.
