    Notes: Summary To establish a reliable procedure for the early detection of central nervous system involvement in HIV infection, paired cerebrospinal fluid and serum samples of 59 patients were analysed. Fifteen were HIV antibody positive without clinical symptoms (stage I), 12 had lymphadenopathy syndrome or AIDS-related complex (stage II), and 32 had AIDS (stage III). Intrathecal synthesis of HIV antibodies was determined by a modified ELISA. Antibodies in CSF and serum were evaluated at identical immunoglobulin G levels to correct for the actual blood-CSF-barrier permeability. A CSF/serum quotient above 1.5 is indicative of intrathecal antibody synthesis, which was found in 47% of the patients in stage I, 67% in stage II, and 84% in stage III. These findings indicate an early and frequent invasion of the CNS.

    Notes: Summary Combined medical, neurological, and serological investigations were carried out in 59 patients infected with human immunodeficiency virus (HIV). In stage I clinical and neuropsychiatric testing did not reveal evidence for HIV encephalitis as diagnosed by local antibody production in CSF. Neuropsychiatric abnormalities, brain atrophy, memory and cognitive impairment reliably indicated HIV encephalitis in later stages. The commonest symptoms were cerebellar and brainstem signs, followed by dementia. Epileptic fits and hemipareses always were associated with cerebral toxoplasmosis. A polyneuropathy was frequently found but other causes have to be considered.

    Notes: Summary The clinical and laboratory records of 13 patients with HIV-related lymphomas diagnosed between 1983 and 1987 are reviewed. 8 cases were classified as high-grade Non-Hodgkin-lymphomas (NHL), 4 as low-grade NHL (including 2 plasmocytomas), and one as Hodgkin's disease (HD). 11 of the 12 NHL were of the B-cell lineage. The mean patient's age was 39 years. At initial staging evaluation extranodal disease was present in 10 patients, in one of them as primary central nervous system (CNS) lymphoma; 9 patients presented with stage III or stage IV involvement. 9 patients were eligible for multidrug chemotherapy. In 5 cases an initial complete remission (CR) was observed, followed, however, by CNS relapse within a few months in 3 patients. 3 patients did not respond to chemotherapy and died within 4 to 6 weeks. Of the 9 patients treated chemotherapeutically, 5 died within 1 to 5 months, 1 is still alive in CR and 3 are still alive in partial remission (PR).

    Notes: Summary Background: The objective of this cross-sectional, non-randomized, prospective study was to generate data on the prevalence of GB virus C (GBV-C)/hepatitis G virus (HGV) in a cohort of HIV-infected homosexuals from Munich. Patients and Methods: A total of 71 HIV-infected homosexual men were analyzed for prevalence of GBV-C RNA and antibodies to the E2 envelope glycoprotein (E2Ab). 475 healthy volunteer blood donors in southern Bavaria served as a control group. Results: The prevalence of GBV-C RNA was 27% (control group: 2.3%) and the prevalence of E2Ab was 35% (control group: 6%). The total prevalence for present and past infection was 62%. The differences between the HIV-infected patients and the control group were significantly (p 〈 0.0001). GBV-C RNA and E2Ab were not detected simultaneously in any serum sample. The E2Ab positive patients were older than the GBV-C RNA positives (mean 46 years versus 39 years, p = 0.0350). The GBV-C RNA and E2Ab negative patients were older than the GBV-C RNA positives (mean 47 years versus 39 years, p = 0.0236). The E2Ab positive patients had suffered sexually transmittet diseases more frequently than the patients negative for markers of GBV-C infection (p = 0.0308). E2Ab positive patients also had higher mean levels of alanine aminotransferase compared to patients without evidence of GBV-C infection (p = 0.0164). 59.4% of all individuals were anti-HBc IgG positive. Conclusion: The data can be interpreted as indirect evidence for sexual transmission of GBV-C.

    Description / Table of Contents: Summary An African from the Comoros Islands, who has been living in Berlin for eleven years and who had made his last journey to Africa two years before, fell ill with high fever upon returning from a visit to Mayotte and Grande Comore. His blood smear revealed trophozoites ofPlasmodium falciparum. After intake of 25 mg of chloroquine base per kg body weight within 66 hours, defervescence and clearance of blood ensued and by the fourth day after start of treatment the parasites could no longer be detected in the blood. On the 25th day high fever reoccurred andP. falciparum was found again. After chloroquine treatment had been repeated with an additional single dose of 75 mg pyrimethamine and 1.5 g sulphadoxine the patient remained healthy. During observation over a period of eight months plasmodia could no longer be detected in thick films.
    Notes: Zusammenfassung Ein von den Komoren-Inseln stammender Afrikaner, der seit elf Jahren in Berlin lebt und zuletzt vor zwei Jahren in Afrika war, erkrankte bei der Rückreise von einem Aufenthalt auf Mayotte und Grande Comore hochfieberhaft. Im Blut wurden Trophozoiten vonPlasmodium falciparum nachgewiesen. Nach Einnahme einer Gesamtdosis von 25 mg Chloroquinbase pro kg Körpergewicht innerhalb 66 Stunden kam es zur Entfieberung und zur Klärung des Blutes; am vierten Tag nach Behandlungsbeginn waren im Blut keine Parasiten mehr nachweisbar. Am 25. Tag traten erneut fieberhafte Symptome auf und wieder wurdeP. falciparum nachgewiesen. Nach erneuter Behandlung mit Chloroquin und einer zusätzlichen Einzeldosis von 75 mg Pyrimethamin mit 1,5 g Sulfadoxin blieb der Patient gesund. In dicken Tropfen wurden während einer achtmonatigen Beobachtung keine Plasmodien mehr gefunden.

    Notes: Summary In 36 HIV seropositive patients with the clinical manifestation of AIDS and a suspectedPneumocystis carinii infection, lymphocyte subpopulations were analzyed in the peripheral blood (PBL) and compared with the results of the bronchoalveolar lavage (BAL). Of those 36 patients, 29 showed a highly abnormal CD4/CD8 ratio in both the PBL and the BAL. The clinical course of these 29 patients was unpredictable. In seven patients, however, the CD4/CD8 ratio in the BAL was normal or only slightly altered, despite a highly abnormal CD4/CD8 ratio in the PBL. Five of these seven patients improved greatly during the clinical course. The positive outcome of the clinical course was even more strongly correlated with the number of macrophages in the BAL. Twelve of the 36 patients showed normal or only slightly changed numbers of macrophages in the BAL. Eleven of these twelve patients (92%) improved rapidly during antibiotic therapy, while the clinical course was unpredictable in patients with markedly reduced macrophage counts in the BAL.
