    Notes: Summary A 42-year-old woman had a 10-year history of external ophthalmoplegia, malabsorption resulting in chronic malnutrition, muscle atrophy and polyneuropathy. Computer tomography revealed hypodensity of her cerebral white matter. A metabolic disturbance consisted of lactic acidosis after moderate glucose loads with increased excretion of hydroxybutyric and fumaric acids. Post-mortem studies revealed gastrointestinal scleroderma as the morphological manifestation of her malabsorption syndrome, ocular and skeletal myopathy with ragged red fibers, peripheral neuropathy, vascular abnormalities of meningeal and peripheral nerve vessels. Biochemical examination of the liver and muscle tissues revealed a partial defect of cytochrome-c-oxidase (complex IV of the respiratory chain). This mitochondrial multisystem disorder may represent a separate entity to be classified between the spectrum of myoencephalopathies and oculo-gastrointestinal muscular dystrophy.

    Description / Table of Contents: Zusammenfassung Die Konzentrations-Quotienten zahlreicher Proteine zwischen Serum und Liquor cerebrospinalis korrelieren besser mit den hydrodynamischen Volumen als mit den Molekulargewichten. Die hydrodynamischen Radien wurden der Literatur entnommen, sowie mit Hilfe der analytischen Säulenchromatographie und Gelelektrophorese bestimmt. Diskrepanzen zwischen Molekulargewicht und hydrodynamischem Volumen werden vorwiegend bei den großmolekularen Proteinen gefunden, da hier die Molekülformen stärker variieren. Die Permeabilität der Blut-Liquor-Schranke wird mit der Blut-Lymph-Schranke verglichen, und die Daten werden unter dem Aspekt der derzeitigen Filtrationstheorien diskutiert. Die Basalmembran, die durch Endothelfenster direkten Kontakt mit dem Kapillarlumen erhält, ist der wahrscheinlichste Ort für den Proteindurchtritt an der Blut-Liquor-Schranke.
    Notes: Summary The concentration ratios of several proteins between serum and cerebrospinal fluid (CSF) can be correlated more easily to hydrodynamic volumes than to molweights. Hydrodynamic radii were calculated from literature diffusion data and determined by analytical column chromatography and gel electrophoresis. Discrepancies between molweight and hydrodynamic volume become evident, especially with the high-molecular proteins because of stronger conformational variability in this size range. The permeability of the blood-CSF-barrier is compared to the blood-lymph-barrier and the results are discussed with reference to current views about capillary permeability. The basement membrane, exposed to the capillary lumen by endothelial fenestrations is considered the most likely site of protein passage at the blood-CSF-barrier.

    Notes: Summary An attempt is made to evaluate those portions of low molecular hydrophilic solutes that obey apparent passive transfer principles at the blood cerebrospinal fluid (CSF) barrier under steady state conditions. Twenty-four electrolytes, metabolites, and amino acids are determined in serum and CSF obtained simultaneously. The correlation found between the serum/CSF concentration ratios and the molecular radii of most compounds is indicative of the prevalence of passive transfer processes in CSF formation, as has been already demonstrated for proteins. The level of some constituents in the primary filtrate may be secondarily increased by influx from the adjacent extracellular space, e.g. glutamine and creatinine, or decreased by specific uptake from the CSF compartment, e.g. glycine. The CSF levels of all compounds are dependent upon the actual state of the blood CSF barrier as evaluated by the serum/CSF concentration ratios of albumin and α2-macroglobulin. The results do not support the assumption that CSF may be used to test the permeability state of the blood brain barrier propery.

    Notes: Summary To establish a reliable procedure for the early detection of central nervous system involvement in HIV infection, paired cerebrospinal fluid and serum samples of 59 patients were analysed. Fifteen were HIV antibody positive without clinical symptoms (stage I), 12 had lymphadenopathy syndrome or AIDS-related complex (stage II), and 32 had AIDS (stage III). Intrathecal synthesis of HIV antibodies was determined by a modified ELISA. Antibodies in CSF and serum were evaluated at identical immunoglobulin G levels to correct for the actual blood-CSF-barrier permeability. A CSF/serum quotient above 1.5 is indicative of intrathecal antibody synthesis, which was found in 47% of the patients in stage I, 67% in stage II, and 84% in stage III. These findings indicate an early and frequent invasion of the CNS.

    Notes: Summary Combined medical, neurological, and serological investigations were carried out in 59 patients infected with human immunodeficiency virus (HIV). In stage I clinical and neuropsychiatric testing did not reveal evidence for HIV encephalitis as diagnosed by local antibody production in CSF. Neuropsychiatric abnormalities, brain atrophy, memory and cognitive impairment reliably indicated HIV encephalitis in later stages. The commonest symptoms were cerebellar and brainstem signs, followed by dementia. Epileptic fits and hemipareses always were associated with cerebral toxoplasmosis. A polyneuropathy was frequently found but other causes have to be considered.

    Description / Table of Contents: Summary The protein patterns of normal serum and cerebrospinal fluid after polyacrylamide electrophoresis are described and compared. After densitometric recording the frequency of occurrence and the relative percentage of all those bands are determined, which can be identified by their relative mobility. Cerebrospinal fluid contains two peculiar bands. The level of CSF highmolecular proteins is lower than in serum and penetration rate of these proteins through the barrier between blood and cerebrospinal fluid depends mainly on their molecular weight. The highmolecular parts of haptoglobins 2–1 and 2–2 are detectable only after disturbance of the barrier. Some technical improvements are described, which facilitate standardisation of the method.
    Notes: Zusammenfassung Es werden die Pherogramme normaler Seren und Liquores nach Polyacrylamidgel-Elektrophorese beschrieben und miteinander verglichen. Nach densitometrischer Registrierung wurden Häufigkeit und relativer Anteil aller mit Hilfe des Rf-Wertes identifizierbaren Banden ermittelt und statistisch ausgewertet. Der Liquor enthält zwei typische Banden und ist arm an großmolekularen Proteinen, die je nach Molekulargewicht an der Blut-Liquor-Schranke zurückgehalten werden. Die großmolekularen Anteile der Haptoglobine 2–1 und 2–2 werden im Liquor erst bei einer Schrankenstörung nachweisbar. Es werden einige technische Variationen und Hilfsmittel beschrieben, die eine Standardisierung der Methode erlauben.

In mehreren Seren, die ein einzigesγM-Paraprotein bei der Immunelektrophorese zeigten, wurden jetzt zwei oder drei abnorme Banden sichtbar.
    Notes: Summary A transparent upper gel disc electrophoretic technique was applied for the detection of mono- and oligo-clonal immunoglobulins. This method allows the visualization and densitometrical recording of those proportions of IgG and IgM retained in the upper concentration gel. WhileγA-globulins generally reach the lower separation gel, manyγG-globulins are retained in the transparent upper gel. In several cases, two or three abnormal bands were detected in sera containing a singleγM-paraprotein on immunoelectrophoresis.
