    Description / Table of Contents: Summary Simultaneous clearances with ortho-J131-hippuric acid (oJ131H) and paraamino-hippuric acid (PAH) were determined in 11 patients without functional disturbances and 5 patients with renal diseases in order to study the possibility using oJ131H for measuring renal plasma flow. In all cases the results of oJ131H-clearance using the classic technique of permanent infusion were significantly lower than those of PAH-clearance (128 ml/min). The free J131H identified by paper chromatography could not explain the difference. Several other reasons will be discussed. Today oJ131H-clearance cannot substitute PAH-clearance. The results of oJ131H-clearance estimated from the administration in steady state according to a purposal ofTaplin was significantly higher than those of oJ131H-clearance using permanent infusions technique (113,8 ml/min). In steady state renal clearance is not corresponding with total clearance.
    Notes: Zusammenfassung Simultane Clearance-Untersuchungen mit J131H und PAH wurden an 11 nierengesunden Versuchspersonen und 5 Patienten mit Nierenerkrankungen durchgeführt, um die Brauchbarkeit der oJ131H zur Bestimmung der renalen Plasmadurchströmung zu überprüfen. In allen Fällen waren die Ergebnisse der oJ131H-Clearance bei der Anwendung der klassischen Infusionstechnik signifikant niedriger als die der PAH-Clearance, im Mittel 128,0 ml/min. Zur Erklärung dieser Differenz reicht der papierchromatographisch nachweisbare freie J131H-Anteil der oJ131H-Clearance nicht aus. Mögliche Ursachen werden diskutiert. Die oJ131H-Clearance kann die PAH-Clearance noch nicht voll ersetzen. Die Werte der oJ131H-Clearance nach dem Vorschlag vonTaplin, errechnet aus der Zufuhr im “steady state”, lagen gegenüber der klassischen oJ131H-Clearance um durchschnittlich 113,8 ml/min höher. Im “steady state” entsprach die renale Clearance nicht der totalen.

Die Berechnung der Clearance aus der zugeführten Aktivität während des steady state (Totalclearance) ergab bei paarweisem Vergleich eine signifikante Erhöhung gegenüber der aus der Aktivitätsausscheidung errechneten Clearance (renale Clearance; mittlere Differenz = 32,9 ml).

    Notes: Summary The role of lipoxygenase products was studied in children suffering from chronic diseases of the lung. Leukotrienes C4, D4, E4 and B4 were measured by high performance liquid chromatography (HPLC) and a specific radioimmunoassay (RIA) for C4. Elevated levels (up to 40 ng/ml), especially for leukotriene E4, were found in plasma of asthmatic and bronchitic patients (leukotriene C4 concentrations varied between 0.05 and 40 ng/ml, mean 4.9±7.8 ng/ml). In healthy donors the concentrations were below the detection limits of HPLC, leukotriene C4 ranging between 5±4 ng/ml (RIA data). The conversion of leukotriene C4 to D4 and E4 was observed by incubating the samples with synthetic leukotriene C4. The half-life of leukotriene C4 in plasma varied greatly, ranging from 〈12 min to 72 min (mean 39±16 min). Bronchial lavages yielded leukotriene C4 concentrations of 0.2 to 7 ng. Leukotriene E4 was detected in 10 of 41 cases. Conversion of leukotriene C4 did not occur in 50% of all cases, but was regularly observed in putrid lavages. These data suggest that leukotrienes play an important role in allergic and infectious lung diseases.

    Notes: Abstract The geographic distribution and origin of CFTR mutations in Germany was evaluated in 658 three-generation families with cystic fibrosis (CF). Fifty different mutations were detected on 1305 parental CF chromosomes from 22 European countries and overseas. The major mutation ΔF508 was identified on 71.5% of all CF chromosomes, followed by R553X (1.8%), N1303K (1.3%), G542X (1.1%), G551D (0.8%) and R347P (0.8%). According to the grandparents’ birthplace, 74% of CF chromosomes had their origin in Germany; the ΔF508 percentage was 77%, 75%, 70% and 62% in northern, southern, western and eastern Germany, respectively. Ten or more mutant alleles in the investigated CF gene pool originated from Austria, the Czech Republic, Poland, Russia, Turkey and the Ukraine. This widespread geographic origin of CFTR mutations in today’s Germany reflects the many demographic changes and migrations in Central Europe during the 20th century.

    Notes: Abstract Clinical and laboratory findings in 26 children with atypical spondyloarthritis were compared with those of 76 children with juvenile rheumatoid arthritis. The sensitivity, specificity, predictive value, and efficiency for diagnosis were calculated. The following findings (major criteria) were much more common in atypical spondyloarthritis than in juvenile rheumatoid arthritis: (1) spondyloarthritis within the family; (2) enthesopathy; (3) arthritis of digital joints; (4) sacro-iliitis; (5) presence of HLA-B27; (6) frequent recurrence of arthritis and arthalgia. Six additional findings (minor criteria) were significantly more common in atypical spondyloarthritis (SA): (1) disease onset after the age of 10 years; (2) male sex; (3) involvement of the lower extremities; (4) acute iridocyclitis or conjunctivitis; (5) arthritis of the hip joints; (6) manifestation following a history of enteritis. In the presence of 4 major criteria or 3 major and 3 minor criteria, the diagnosis of an atypical SA was established with a sensitivity of 84.6%, a specificity of 100%, and an efficiency of 96.1%.

    Description / Table of Contents: Zusammenfassung Bei 94 6- bis 15jährigen gesunden Kindern wurde das intrathorakale Gasvolumen am Ende der normalen Ausatmung (TGV) mit einem druckkorrigierten, volumenvariablen Ganzkörperplethysmographen gemessen und mit der funktionellen Residualkapazität (FRC) verglichen, die gleichzeitig mit dem TGV mit Hilfe der Heliumverdünnungsmethode bestimmt wurde. TGV ist um 1,9% (±11,7% sd.) kleiner als FRC, doch ist diese Differenz statistisch nicht signifikant (P〉0,05). Ein Normwertdiagramm für TGV (in ml) bezogen auf die Körpergröße (in cm) wird für Jungen und Mädchen vorgestellt. Bei 82 6- bis 15jährigen gesunden Kindern wurde der Atemwegswiderstand (Raw) plethysmographisch gemessen. Raw (in cmH2O/1/sec) ist kleiner, je größer das Kind ist (r=-0,57; P〈0,01), die Reststreuung um die Regressionsgerade ist beträchtlich (29%) und entspricht den bereits veröffentlichten Werten für den Gesamtlungenwiderstand. Es werden die Schwierigkeiten der Interpretation der Atemschleifen als auch der Berechnung und Lungenvolumenkorrektur der Raw-Werte diskutiert.
    Notes: Abstract In 94 healthy children, 6–15 years of age, the intrathoracic gas volume at resting expiratory level (TGV) was measured by means of a pressure corrected flow body plethysmograph and compared to functional residual capacity (FRC), measured simultaneously to TGV by means of the Helium dilution technique. TGV is 1.9% (±11.7% SD) smaller than FRC, this difference being not significant (P〉0.05). A predicted equation for TGV (in ml) in correlation to standing height (in cm) is published in boys and girls. In 82 healthy children, 6–15 years of age, airway resistance (Raw) was measured plethysmographically. Raw (in cmH2O/1/s) is smaller, the larger is the child (r=-0.57; P〈0.01), the residual standard deviation around the regression line is considerable (29%) and corresponding to the value, published previously for total pulmonary flow resistance. Difficulties in the evaluation of recorded resistance curves as well as calculation and lung volume correction of the Raw-value are discussed.

    Notes: Abstract Outbreaks of necrotising enterocolitis (NEC) have often been related to specific pathogens such as Enterobacteriaceae. This relationship, however, remains uncertain because of the retrospective nature of the studies addressing this issue. We performed a prospective study to investigate whether there is indeed an association between NEC and specific pathogens. Between April 1993 and March 1997, stools of neonates of 〈36 weeks admitted to our neonatal unit were investigated for bacteria in weekly intervals. Clinical and bacteriological data from each infant who developed NEC were compared with those from two control infants matched for gestational age and date of admission. Eighteen infants developed 19 episodes of NEC (clinical signs + air in portal vein); 8 of these had laparotomy; two died. Occurences of NEC were homogeneously distributed over the 4- year study period. The only significant differences in the clinical course prior to NEC were a more severe stage of respiratory distress syndrome [median 2 (0–4) vs. 0 (0–3), P 〈 0.05] and a higher proportion of infants who had only been formula fed (63 vs. 32%, P 〈 0.05) in the cases. Within the last week prior to NEC, potentially pathogenic bacteria were identified in stools of all cases and 79% of controls (P 〈 0.05). However, there was no significant difference in the occurrence of specific pathogens or groups of pathogens in cases compared with controls. Conclusion Although gut colonisation with potential pathogens appeared to be a prerequisite for the development of NEC, there were no specific bacteria associated with this disease if data from infants with NEC were compared with those from time- and gestational age-matched controls.
