    Notes: Summary The respective rôles of arterial blood pressure and metabolic control in different stages of diabetic nephropathy were analyzed retrospectively in 52 sequentially-followed Type 1 (insulin-dependent) diabetic patients. A negative correlation was found between median post-prandial blood glucose and median duration of diabetes until onset of persistent proteinuria (p〈0.01). Systolic blood pressure was higher in patients who subsequently developed persistent proteinuria than those who did not (140 versus 121 mmHg; p〈0.05), but duration of the interval until onset of persistent proteinuria was not related to blood pressure. After onset of persistent proteinuria, hypertensive diabetic patients developed elevated serum creatinine concentrations more frequently than normotensive diabetic patients (67% versus 14%, p〈0.05). In these patients, the delay until elevation of serum creatinine concentration was negatively correlated with blood glucose (p〈0.01). Once serum creatinine was raised, decay of renal function occurred faster in patients with persistent than intermittent hypertension (p〈0.05). No effect of metabolic control was demonstrable at this stage of nephropathy. It is concluded that metabolic control determines the early course of diabetic nephropathy, whereas blood pressure is more important in advanced stages of nephropathy.

    Notes: Summary Glomerular basement membrane synthesis was measured in vivo in diabetic rats at various times after streptozotocin injection. Simultaneously the type IV collagen concentration in serum was determined by a radioimmunoassay specific for the terminal cross-linking domain, 7S collagen. Basement membrane collagen synthesis and the 7S collagen levels were markedly raised in diabetic animals 18–34 days after streptozotocin injection and showed a significant correlation. The results suggest that the amount of serum 7S collagen reflects increased basement membrane synthesis in a diabetic situation. Both basement membrane collagen synthesis and serum 7S collagen concentration also show a positive correlation with the blood glucose levels of diabetic rats. This indicates that the degree of metabolic dysregulation influences basement membrane synthesis.

    Notes: Summary The effects of combined renovascular hypertension and diabetes mellitus on the rat heart were investigated in order to detect possible synergistic effects of the two conditions. Hypertensive diabetic and hypertensive nondiabetic young male Wistar rats were compared with diabetic and nondiabetic controls. Since the normal body weight increase of the diabetic animals was markedly suppressed a weight-matched nondiabetic control group was introduced in addition. Hypertension was established for eight weeks by a surgical stenosis of the left renal artery, diabetes mellitus was maintained for four weeks after a single intraperitoneal injection of 75 mg/kg streptozotocin. Light and electron microscopic stereological parameters were obtained for the left ventricular papillary muscles. The whole hearts were also investigated histologically. Qualitative morphology failed to substantiate synergistic effects in the hypertensive diabetic rats. Vascular abnormalities were not observed. The stereological parameters, however, revealed microstructural reactions which were observed exclusively in the hypertensive diabetic group: the volume ratio of mitochondria-to-myofibrils was decreased, the surface-to-volume ratio of mitochondria was increased (reduction of mitochondrial size) and the mean cross sectional area of capillaries was decreased. Similar quantitative mitochondrial changes have been frequently described in long-standing hypertension, but in the present investigation, they were not found in the nondiabetic hypertensive group. It is therefore concluded that diabetes mellitus potentiates the effects of chronic pressure overload on myocardial cells. However, the myocardial fibrosis which has been found by other groups at later stages of hypertension and/or diabetes mellitus was not detected in the present study. The reduced mean cross sectional area of capillaries in hypertensive-diabetic rats may be correlated with early molecular changes of the myocardial interstitium or with early abnormalities of small arteries. Thus our stereological results support the hypothesis that a non-coronary hypertensive diabetic cardiomyopathy occurs in mammalian hearts.

    Description / Table of Contents: Zusammenfassung Unsere Befunde zeigen, daß es während der Perfusion mit der Herz-Lungenmaschine regelmäßig zur Ausschüttung eines hochwirksamen Fibrinolyseaktivators kommt, der zur Spontanfibrinolyse des geronnenen Venenblutes führt, jedoch nicht zu einer Spaltung des zirkulierenden Fibrinogens. Zusätzlich tritt im Wundbereich eine wahrscheinlich durch Gewebsaktivatoren in Gang gesetzte lokale Fibrinolyse auf. Mit einer AMCHA-Dosis, die den intravasalen Fibrinolyseaktivator vollständig hemmt, wird die lokale Fibrinolyse nur zum Teil unterdrückt. Trotzdem ist eine Verminderung des postoperativen Blutverlustes festzustellen, sofern die Perfusionsdauer 40 min nicht überschreitet. Der blutstillungsfördernde Effekt des Trasylols bei länger perfundierten Patienten läßt sich kaum mit einer Fibrinolysehemmung erklären, da die gemessene antifibrinolytische Wirkung auf Venenblut- und lokale Fibrinolyse schwächer war als die von AMCHA. Nebenwirkungen, insbesondere eine Begünstigung von Thrombosen, sahen wir bei der antifibrinolytischen Prophylaxe nicht.
    Notes: Summary In 60 patients, who were operated with the use of extracorporeal circulation, intravascular and local thoracic fibrinolytic activity was measured by immunological methods. The increased fibrinolytic activity was caused by liberation of a plasminogen activator, and enhanced postoperative blood loss. This was shown by the effect of an antifibrinolytic prophylaxis with AMCHA.

    Notes: Summary Diabetic rats were treated i.p. for 1 week with calcium dobesilate (200 mg/kg body wt.) and the influence of this substance on protein and basement membrane collagen synthesis of isolated glomeruli was investigated. Calcium dobesilate did not lead to an alteration of the diabetic metabolic situation. Compared to the non-diabetic rats, raised basement membrane collagen synthesis of diabetic rats was markedly inhibited by calcium dobesilate. However, this effect does not appear to be specific, since the raised general protein synthesis was also reduced. The findings show that the disturbed basement membrane metabolism in diabetes can be influenced without altering the metabolic situation.

    Notes: Summary Diabetic rats were treated with insulin at various dosages and for various periods of time. The influence of metabolic control on the synthesis of protein and basement membrane collagen of isolated glomeruli was investigated. The protein and basement membrane collagen synthesis was increased in the untreated diabetic rats compared to non-diabetic controls. The synthesis was not affected by high doses of insulin with brief normalization of the blood sugar. Insulin treatment from the beginning of diabetes only led to a normalization of protein synthesis in moderate metabolic control. On the other hand, a rise of the basement membrane collagen synthesis could only be prevented by a strict metabolic control of the rats. The results show that basement membrane collagen synthesis reacts very sensitively to the diabetic metabolic situation. Insulin deficiency itself does not appear to be alone responsible for alterations of basement membrane synthesis in diabetes.

    Notes: Summary In rats with chronic diabetes, the synthesis of glomerular basement membrane was investigated in vivo by injection of3H-proline. Compared to the nondiabetic controls, the basement membrane synthesis was markedly raised. The formation of hydroxyproline as parameter for the synthesis of the collagen portion of the basement membrane increased to a somewhat greater extent than the incorporation of3H-proline. Hydroxyproline formation and proline incorporation showed a close correlation with the blood sugar level in the diabetic rats.
