    Notes: Abstract The spinal dorsal horn is known for its important functional role in the field of transmission and modulation of sensory afferents. Because of this, the dorsal horn represents a target for numerous analgesic and antispastic procedures. Thus, it would be interesting to develop imaging dedicated to this spinal structure. The purpose of this study was to investigate the radiologic anatomy of the cervical dorsal horn by magnetic resonance imaging (MRI) (1.5T). The first step consisted in the validation of the anatomic information provided by MRI on 5 human cadavers. A spin-echo sequence (T2, 2000/45) enabled the demonstration of good correlations between histologic sections and axial MRI slices performed at the corresponding cervical levels. The second step was the 〈〈in vivo〈〈 exploration of 20 subjects, aiming at the development of a gradient echo sequence (T2*) with a conventional MRI unit, compatible with a routine clinical examination. The dorsal horn was clearly identified in 77% of the axial slices performed (n = 300). The angle between the dorsal horn axis and the sagittal plane was measured as from 25.5˚ at C2 to 40˚ at C8 segments. The results of this anatomico-radiologic study of the cervical dorsal horn suggest that preoperative MRI could be useful to design the surgical approach to this structure, as performed during cervical microsurgical drezotomy (DREZ = dorsal root entry zone) for the treatment of selected cases of chronic pain or disabling spasticity in the upper limbs.

    Notes: Abstract Clinical data and autopsy findings in a case ¶of new variant Creutzfeldt-Jakob disease (vCJD) are reported. this case, the first histologically confirmed case described outside the United Kingdom, very much resembles the cases described by Will et al. [(1996) Lancet 347 : 921–925] and Zeidler et al. [(1997) Lancet 350 : 903–908, 908–910]. Neuropathological studies failed to reveal any conspicuous clues that could be relevant for understanding the pathophysiology of the disease. For epidemiological surveillance, neuropathologists should scrutinize suspected cases keeping in mind the possibility of vCJD.

    Notes: Abstract Abstract In this study, 144 consecutive percutaneous liver biopsies performed with a 1.6 mm Menghini needle, during a 2-year period were reviewed. All the children were aged under 15 years, 57 patients less than 1 year and 87 more than 1 year. All biopsies were adequate and the mean number of portal tracts examined was 17.6 per biopsy (14.3 in patients weighing less than 10 kg and 19.1 in the others). There were no deaths and we observed only bleeding complications. In patients with normal coagulation (128 cases), 1 bleeding requiring transfusion occurred; and in patients with abnormal coagulation (16 cases), we observed 2 bleeding cases requiring transfusion. Conclusion Percutaneous liver biopsy can be performed with 1.6 mm needles in children. For increased safety, ultrasound-guided biopsies are recommended.

Proteinuria disappeared at the time of the remission of the tumor.     Notes: Abstract Our aim was to collect a large number of casesto characterize clinical presentation, outcome, andprognosis of chronic intestinal pseusoobstruction inchildren. We conducted a retrospective multicenter study that included children treated forchronic intestinal pseusoobstruction defined asrecurrent episodes of intestinal obstruction with nomechanical obstruction, excluding Hirschsprung'sdisease. In all, 105 children, 57 boys and 48 girls, werestudied, including five familial forms. Prenataldiagnosis was made in 18 patients. Eighty patients wereless than 12 months old at onset; the disease began at birth for 37 patients. The most frequentsigns were abdominal distension, vomiting, andconstipation. Megacystis was noted in myopathies (7cases), neuropathies (10 cases) and unclassified forms(13 cases). For all but three cases (two patientswith CMV infection, one with Munchhausen-by-proxysyndrome), the associated diseases and disorders couldnot account for chronic intestinal pseusoobstruction as a secondary disorder. At least onefull-thickness biopsy from the digestive tract wasstudied for 99 patients. The diagnosis recorded wasvisceral neuropathy in 58 cases, visceral myopathy in 17cases, and uncertain or normal biopsy results in 24cases. Seventy-eight children were fed intravenously,and only 18 were able to be fed orally throughout theirillness. Seventy-one patients underwent surgery during their illness, and 217 surgicalprocedures, a mean of 3 per patient, were performed.Ostomy was the most performed procedure. Follow-upcontinued in 89 patients for 3 months to 16 years (mean85 months). Forty-two patients were still fed byparenteral (39 patients) or enteral nutrition (3patients) at the time of the study. Eleven patients diedbetween the age of 1 month and 14 years 7months.

    Description / Table of Contents: Zusammenfassung Screening durch Anti-HBc-Antikörper-Bestimmung bei schwangeren Trägerinnen des Hepatitis B-Virus (HBV) zur Vorbeugung der perinatalen Übertragung von HBV. Screening auf anti-HBc wurde vorgeschlagen, um die HBV-Träger unter Blutspendern zu ermitteln. Wir haben die Wirkung eines solchen Screenings während der Schwangerschaft untersucht. Unter 4023 schwangeren Frauen, welche aufeinanderfolgend auf anti-HBc getestet wurden, zeigten sich 539 (13,4%) positiv und wurden auf HBsAg und anti-HBs getestet. HBsAg wurde bei 73 (1,81%) und anti-HBc allein bei 66 (1,64%) Frauen nachgewiesen. Unter den 73 HBsAg-positiven Frauen wiesen sieben HBV-DNA im Serum, zwei in der Nabelschnur und drei in der Plazenta auf. Unter den 58 Kindern, die HB-Immunglobulin und HBV-Impfstoff erhalten hatten, war nur bei vier vorübergehend HBsAg nachzuweisen. Von den 66 nur anti-HBc-positiven Frauen wies keine anti-HBc, IgM-Antikörper, HBeAg oder HBV-DNA im Serum, Nabelschnur, Blut oder Plazenta auf, aber fünf dieser Frauen wurden HBsAg-positiv vor, während oder nach der Niederkunft. Drei der von diesen 66 Müttern entbundenen Kinder hatten erhöhte ALT-Werte, zwei HBsAg und eines HBV-DNA ohne HBsAg. Das Screening auf anti-HBc kann zumindest in Gegenden mit geringer HBV-Prävalenz als effizient angesehen werden, da offensichtlich nur bei anti-HBc-positiven Frauen Infektiosität besteht.
    Notes: Summary As anti-HBc screening has been proposed for blood donor testing, we investigated its effectiveness during pregnancy. Among 4,023 successive pregnant women screened for anti-HBc, 539 (13.4%) were positive and further tested for HBsAg and anti-HBs. HBsAg was found in 73 (1.81%) and anti-HBc only was positive in 66 (1.64%). Among the 73 women positive for HBsAg, HBV DNA was found in the serum of seven, the cord blood of two, the placenta of three. Of the 58 infants given HBV immunoglobulins and vaccine, only four had transient HBsAg. None of the 66 women positive for anti-HBc only had anti-HBc IgM, HBeAg, or HBV DNA in serum, cord blood or placenta but five women became HBsAg positive before, at, or after delivery. Among the infants born of these 66 mothers, three had high ALT, two had HBsAg and one HBV DNA without HBsAg. Screening for anti-HBc may be cost effective, at least in low HBV prevalence areas, since there is evidence for infectivity of pregnant women positive for anti-HBc only.
