    Notes: Abstract We compared the results of curative resection for carcinoma of the stomach in 254 patients who underwent simple resection (SR) and 454 patients who underwent extensive regional lymph node dissection (ELD). The 5-year survival rates of the 2 procedures were significantly different in carcinoma involving the serosa of the stomach; it was 45% in the ELD group and 18% in the SR group (p〈0.001). In patients with regional lymph node metastasis we obtained a 5-year survival rates of 39% and 18% by ELD and SR, respectively (p〈0.001). The incidence of metastasis to the secondary lymph nodes, removable only by ELD, was higher in cases with carcinomatous invasion of the deeper layers of the gastric wall, and this may have been the reason why ELD proved to be more effective than SR. ELD is discussed in relation to the site of the primary carcinoma and the extent of lymph node metastasis.

    Description / Table of Contents: Résumé Le traitement du cancer du foie associé à des varices oesophagiennes fut pratiqué chez 8 cirrhotiques qui présentaient une hypertension portale. Chez 5 malades, admis pour le traitement chirurgical de varices oesophagiennes, un cancer limité du foie fut décelé au moment de l'exploration opératoire. Chez 3 malades le diagnostic d'hépatome avait été porté avant l'intervention. L'exérèse de la tumeur du foie suivie d'une anastomose porto-cave fut pratiquée avec succès chez 6 malades. Le traitement simultané des varices oesophagiennes et du cancer du foie est possible quand la tumeur hépatique est découverte à un stade précoce. Le procédé le plus fiable pour aboutir à un diagnostic exact est représenté par l'artériographie hépatique. Notre propre expérience nous incite donc à recommander l'étude artériographique chez les cirrhotiques, particulièrement quand on envisage de traiter chirurgicalement les varices oesophagiennes.
    Notes: Abstract Surgical treatment of liver cancer associated with esophageal varices was carried out in 8 cirrhotic patients with evidence of portal hypertension. In 5 patients admitted for elective surgery of esophageal varices, minute hepatomas were detected during the course of preoperative investigation. The remaining 3 patients were referred to our department when esophageal varices plus hepatoma had become evident. Extirpative treatment of the liver tumor followed by selective shunt operations were successfully carried out in 6 patients, and interrupted procedures were used in 2 patients. Treatment of both esophageal varices and liver cancer appears to be effective when the tumor in the liver is detected in an early stage. The most reliable diagnostic tool at the present time seems to be hepatic arteriography. Our experience strongly suggests that arteriographic studies should be seriously considered in cirrhotic patients, particularly when surgery for varices is being contemplated.

    Notes: Abstract This study was done on 110 lung cancer patients who had received surgical resection consisted of two groups; one group of 43 who survived more than 5 years without recurrence and the other group of 67 who died within one year following surgery. Prognostic significance of the histopathological features at the primary tumor site as well as the regional lymph nodes were compared between the two groups. Blood vessel invasion by the tumor and lymph node metastasis appeared to be equally significant prognostic factors. Patients having the both factors had little chance for survival. Abundant lymphoid cell infiltration around the tumor was associated with longer survival. Lymphoid cell infiltration at the site of blood vessel invasion also was associated with better grognosis. Follicular hyperplasia and paracortical hyperplasia in the regional lymph nodes were favorable prognostic indicators, whereas sinus histiocytosis was poorly significant prognostic indicator.

    Notes: Abstract The use of free peritoneal patch was evaluated for the effect on protection against intestinal anastomotic leakage in dogs. Free peritoneal patch was found to be effective in preventing the anastomotic leakage of the alimentary tract in borderline situation. Free peritoneal patch was more effective when applied with the ‘smooth side out’ than with the “smooth side in’. The usefulness of free peritoneal patch was also demonstrated in the treatment of the already established fistula of the cervical esophagus.

    Notes: Abstract The recent advent of the small vessel suturing apparatus made possible the use of free intestinal segment for reconstruction of the resected cervical esophagus which has been one of the most difficult procedures. In the present communication, successful use of revascularized jejunal segment with Inokuchi’s vascular stapler in 13 cases of upper esophageal cancer was reported. Histological examination of the 10 resected specimens revealed that cancer invasion beyond the esophageal adventitia as well as risky cancer remnant at the resected margins were more frequently present when the tumor extended more than 5 cm longitudinally. Therefore esophageal cancer localized within the neck and extending less than 5 cm may well be indicated for the free intestinal transplantation. Details of the operative technique are described.

    Notes: Abstract Two patients having the abdominal aneurysm with aortoduodenal fistula were treated surgically, but graft infection occurred. One patient died about 6 months after and the other about one year after the operation due to massive hemorrhage from the postoperative aortointestinal fistula. The difficulties in the treatment of the aortoduodenal fistula were discussed.

    Notes: Abstract A postoperative long term chemotherapy was carried out against stage IV gastric cancer with Mitomycin-C, Futraful and a plant polysaccharide, PS-K. The 2-year survival rate was 15 per cent in patients who received an intraoperative Mitomycin-C alone, while it increased to 34 per cent in those receiving the anticancer agents for a prolonged period. The postoperative long term chemotherapy is assumed to be an effective means of improving the therapeutical results of gastric cancer.

    Notes: Abstract A patient with metastatic lesions of the stomach from the breast cancer was reported, in whom the gastric trouble appeared in the early stage of the disease. The patient had had a quiescent period of over nine years after the successful hormonal therapy, until when she began to get worse again. At autopsy extensive multinodular metastases were found in the both lungs, liver, spleen and many other organs except the brain.
