Summary Intravenous injections of35S-d,l-cystine led to the urinary excretion of 38 per cent of the total activity within 60 hours in two patients with cystinosis, but of 61 per cent in two normal controls (mean value). In cystinosis the sulfate sulfur amounted to 52 per cent (mean value) of the35S-activity excreted whilst in the controls the relation was 75 per cent. This shows that in cystinosis the breakdown of S-containing metabolites is impaired for reasons which are still unclear. In three patients with cystinosis the taurine content of the thrombocytes was reduced. The cysteic acid concentration in plasma and hepatic tissue was increased and also showed a rise in the thrombocytes above the corresponding control values after incubation with cysteine. Cysteic acid may contain cysteine sulfinic acid, which occupies a key position in the metabolism of S-containing aminoacids and which is readily oxydized to cysteic acid. The activity of the cysteine sulfonate decarboxylase in the liver of cystinosis patients is normal. The increase of cysteine sulfinic acid and sulfonic acid (which cannot be separated by column chromatography because cysteine sulfinic acid is readily oxydized to sulfonic acid) raises the problem whether a genetic block exists between cysteine sulfinic acid and the end product sulfate.

Zusammenfassung Intravenöse Injektionen von35S-d,l-Cystin bewirkten bei zwei Cystinose-Kranken eine Urinausscheidung von 38% der applizierten Gesamtaktivität, während diese bei zwei Kontrollen 61% betrug (Mittelwerte). Der Anteil des Sulfatschwefels war bei der Cystinose 52% der ausgeschiedenen Gesamtaktivität, bei den Kontrollen 75%. Diese Resultate zeigen, daß die Oxydation und Elimination der schwefelhaltigen Aminosäuren aus noch nicht geklärten Gründen gestört ist. Bei drei Cystinose-Kranken war der Tauringehalt der Thrombocyten vermindert. Der Cystein(sulfon)säure-Gehalt in Plasma und Leberbiopsiematerial war erhöht und stieg bei Inkubation der Thrombocyten mit Cystein höher an als bei Normalen. Cystein(sulfon)säure und Cysteinsulfinsäure können jedoch nicht getrennt bestimmt werden, da letztere spontan oxydiert. Die Aktivität der Cysteinsäure-Decarboxylase war in der Leber der Cystinose-Kranken normal, so daß der erhöhte Gipfel für Cysteinsulfin + sulfonsäure der Cysteinsulfinsäure zugeschrieben werden muß Dies läßt vermuten, daß der genetische Block zwischen Cysteinsulfinsäure und dem Endprodukt Sulfat liegt.
