    Notes: Summary The authors studied retrospectively 23 recurrent meningiomas of a total series of 277 intracranial meningiomas operated upon in the Department of Neurosurgery, University of Würzburg, between 1983 and 1989. All tumours were diagnosed by CT scan or MRI and operated upon using modern neurosurgical operation technology such as CUSA, Neodym-YAG-laser and if necessary operating microscope. Age, sex and tumour site could not be demonstrated as an influencing factor of tumour recurrence. Histological subtype and cellular criteria (mitoses, nuclear pleomorphism, focal necrosis) also were not predictive for tumour recurrence in the individual case. We found longer recurrence-free intervals after complete tumour removal (Simpson's grade I or II) than after subtotal tumour excision (Simpson's grade III or IV), but this tendency was not statistically significant. Obviously the analysed factors especially histomorphological criteria and surgical grading do not reflect the total biological behaviour of recurrent meningiomas. Further studies on growth kinetics and tumours metabolism may help to solve this problems.

    Notes: Summary  In this prospective study the role of intra-operative Colour-Duplex-Sonography (=CDS) during surgery of arteriovenous malformations (=AVM) is evaluated.  During the last three years 20 consecutive patients with supratentorial AVMs were examined by intra-operative CDS in order to evaluate the potential of CDS to 1) localize the AVM, 2) differentiate between embolized and perfused parts, 3) identify feeding and draining vessels and 4) control the complete excision of the AVM.  All AVMs were localized supratentorially, 9 were grade I and II (according to Spetzler and Martin [31]), 8 grade III and 3 grade IV. 11 were partly embolized and 8 associated with an intracerebral bleeding.  In all cases the nidus was correctly localized sonographically by its typical bidirectional flow pattern in Colour-mode. CDS guided the surgeon directly to all (11 cases) deep-seated AVMs (2 to 4cm subcortically). The smallest nidus measured 10 mm.  28 of 34 angiographically defined main feeding and 18 of 23 draining vessels were identified. 14 patients were controlled sonographically at the end of the resection regarding the completeness of excision. In 11 patients CDS was negative and was confirmed by either postoperative angiography or MRI in 10 patients. In one case residual AVM tissue was missed by CDS. Positve CDS findings in 3 cases were all confirmed by microscopic re-inspection, angiography and CCT.  Our results suggest that CDS is able to localize AVMs intra-operatively with minimal instrumentation. It allows safe navigation to deep-seated malformations with high accuracy. Feeding and draining vessels can be identified and completeness of resection can be controlled.

    Notes: Summary Steroid receptor binding activity was evaluated in specimens of 28 human cerebral meningiomas using a dextran coated charcoal (DCC) assay. Oestrogen receptor (ER) binding activity at significant levels (〉 10 fmol/mg protein) could be detected only in three postmenopausal females (11% positive cases) at low levels. Positive progesterone receptor (PR) binding was detected in eighteen of the twenty-eight analysed meningioma tissues (64% positive cases). A significantly higher level of PR in male than in female patients could be demonstrated. The degree of peritumoural oedema could be evaluated from CT scans. There was no significant correlation between lack and amount of peritumoural oedema and quantity of cytosolic PR binding activity. Therefore we conclude, that peritumoural oedema is related to other factors and a possible role of PR activity in development of peritumoural oedema and growth control of meningiomas could not be demonstrated.

    Notes: Summary As a reliable continuous monitoring of cerebral blood flow and/or cerebral oxygen metabolism is necessary to prevent secondary ischaemic events after severe head injury (SHI) the authors introduced brain tissue pO2 (ptiO2) monitoring and compared this new parameter with TCD-findings, cerebral perfusion pressure (CPP) and CO2-reactivity over time on 17 patients with a SHI. PtiO2 reflects the balance between the oxygen offered by the cerebral blood flow and the oxygen consumption by the brain tissue. According to TCD-CO2reactivity PtiO2-CO2-reactivity was introduced. After initally (day 0) low mean values (ptiO2 7.7 +/−2.6 mmHg, TCD 60.5 +/−32.0 cm/sec and CPP 64.5 +/−16.0 mmHg/, ptiO2 increased together with an increase in blood flow velocity of the middle cerebral artery and CPP. The relative hyperaemic phase on days 3 and 4 was followed by a decrease of all three parameters. Although TCD-CO2-reactivity was except for day 0 (1.4+/−1.5%), sufficient. ptiO2-CO2-reactivity sometimes showed so-called paradox reactions from day 0 till day 3, meaning an increase of ptiO2 on hyperventilation. Thereafter ptiO2-CO2-reactivity increased, increasing the risk of inducing ischaemia by hyperventilation. The authors concluded that ptiO2-monitoring might become an important tool in our treatment regime for patients requiring haemodynamic monitoring.

    Notes: Summary Although meningiomas are of benign character and generally of encapsulated growth, recurrence is a known problem in treatment. The authors present the time course of a recurrent parasagittal meningioma of the falx, which recurred eleven times. Despite modern radiological diagnostic methods, which made early diagnosis of recurrent tumour possible, and the use of modern microsurgical techniques with radical tumour extirpation and followed by radiotherapy, the fatal course of benign tumour disease could not be stopped.

    Notes: Summary The present study aimed at relating dementia, pseudo-neurasthenic and affective organic brain syndromes to underlying type of CSF flow disorder and to subsequent alteration of anatomy. T2*-weighted magnetic resonance imaging (MRI) in the midsagittal plane permitted an analysis of aqueductal CSF flow phenomena and hydrocephalus-induced elevation, thinning and dorsal impingement of the corpus callosum. Furthermore, the width of the third ventricle was measured on the transverse scout images. 72 patients with communicating hydrocephalus (increased aqueductal CSF pulsations) and 26 patients with aqueductal stenosis (absence of aqueductal flow phenomena) were compared with 22 controls. Dementia and affective disorders were distributed equally among both CSF flow subgroups whereas pseudo-neurasthenic syndromes were observed more frequently in non-communicating hydrocephalus (p 〈 0.03). Alzheimer-type and multiinfarct dementia syndromes were found more frequently in communicating hydrocephalus whereas non-classifiable dementia showed some predilection for non-communicating hydrocephalus. Callosal height, area and third ventricular width did not predict affective or pseudoneurasthenic disorder whereas third ventricular width (p 〈 0.01) and callosal area (p 〈 0.05) discriminated between demented and non-demented patients. Dorsal impingement of the corpus callosum by the falx was a non-specific finding.

    Description / Table of Contents: Zusammenfassung Ein 48jähriger Alkoholiker stürzte nach zweitägiger Abstinenz unbeobachtet und war kurzzeitig bewußtlos. Nach Aufnahme in die Klinik war er orientiert, aber persistierend psychomotorisch unruhig. Ein fokales neurologisches Defizit, Kopfschmerzen oder Nackensteife fanden sich nicht. Die Diagnose eines Alkoholentzugsanfalles wurde gestellt. Ein „sicherheitshalber“ durchgeführtes CCT zeigte überaschenderweise eine ausgedehnte SAB. Arteriographisch wurde ein Aneurysma der A. com. ant. nachgewiesen und raschestmöglich operiert. Anschließend war der Pat. beschwerdefrei.
    Notes: Summary After two days of abstinence of prior alcohol abuse, a 48 year old man fell unobserved and was unconscious for a short time. On admission he was reoriented but showed a persisting psychomotoric restlessness. There was no focal neurological deficit, especially no headache nor meningism. Diagnosis of an alcohol withdrawal seizure was made. A CT-scan unexpectedly showed a severe basal subarachnoid hemorrhage. Arteriography revealed an aneurysm of the anterior communicating artery which was quickly operated. After surgery the patient recovered completely.
