    Notes: Summary The total and free steady-state plasma levels of nifedipine in patients with renal failure have been compared with those in subjects with normal renal function. Studies were done after administration of nifedipine 10 mg t.d.s. p.o. for 5 days, after i.v. infusion of 4·4 mg, and after a single 10 mg oral dose. The systemic clearance of nifedipine after a single i.v.-dose was higher in subjects with renal insufficiency (854 ml/min) than in those with normal renal function (468 ml/min). After the single oral dose the AUC (6100 ng·min·ml−1) and maximum plasma concentration (75.0 ng·ml−1) were lower than in subjects with normal renal function (19300 ng·ml−1; 122 ng·ml−1). The plasma protein binding of nifedipine averaged 95.5% in normal subjects and 94.8% in patients with renal failure. Although free and total steady-state plasma levels of nifedipine tended to be somewhat lower than normal in renal failure, the changes in pharmacokinetics and decreased protein binding of nifedipine did not result in a significantly different steady-state plasma level of the drug. The blood pressure response to a given plasma nifedipine level appeared to be enhanced in renal failure.

    Notes: Summary The volumes of distribution of theβ-adrenoceptor blocking agents propranolol and atenolol, and the calcium antagonist verapamil, during exercise have been investigated. Changes in the plasma concentrations of atenolol and propranolol during exhaustive exercise at 70% of maximal aerobic power were compared after 1 week of oral treatment (propranolol 80 mg b. d. and atenolol 100 mg once daily) in 12 healthy volunteers. In a second study the effect of 10 min exercise at 50 % of maximal aerobic power on steady state plasma concentrations of propranolol, atenolol and verapamil was compared in 7 healthy subjects. The drugs were administered by a continuous intravenous infusion. The plasma concentration of atenolol was not changed by exercise in either study, but the plasma concentrations of propranolol and verapamil were significantly increased in both studies. However, after correction for changes in plasma volume during exercise, the plasma propranolol concentration was not significantly elevated in the second study. From the results it is concluded that exercise led to a reduction in the volume of distribution of propranolol during prolonged exercise (25 min) at 70 % Wmax, which was not clearly demonstrable during 10 min exercise at 50 % Wmax. The volume of distribution of verapamil was reduced during 10 min exercise at 50 % Wm, No change in the volume of distribution of atenolol during exercise could be shown. The changes in the volumes of distribution of propranolol and verapamil during exercise may contribute to preventing an increase in the half-life of these drugs in patients performing prolonged physical exercise.

    Notes: Summary In a double-blind, placebo-controlled crossover study, the plasma atrial natriuretic peptide (ANP) levels of nine young and ten elderly hypertensive patients were compared after placebo and after treatment with 120 mg verapamil given three times daily over 4 weeks. During placebo, plasma ANP levels proved to be higher in elderly patients than in young subjects. Chronic treatment with verapamil induced a rise in ANP levels in both young and elderly patients with hypertension.

    Notes: Summary In 86% of 63 patients with spinal arachnoiditis the localization was lumbosacral, and in 14% it was cervical or thoracic. The most important aetiological factor was the combination of one or more myelographies with one or more operations. It is remarkable that in most patients with lumbosacral arachnoiditis the clinical picture did not show new or typical signs of arachnoiditis, but was the same as before the arachnoiditis had developed. In only 20% of patients with lumbosacral arachnoiditis, and in all patients with cervical or thoracic arachnoiditis, did new signs and symptoms occur concomitantly with and due to the development of the arachnoiditis. The results of this retrospective study suggest that lumbosacral arachnoiditis is a coincidental finding in the majority of cases, without clinical consequences.

    Notes: Summary The growing difference with regard to academic and professional accountability between neurosurgeons and neuroscience is alarming. Controlled clinical trials continue to be the most scientifically valid method of evaluating standard treatments against the risks and benefits of medical innovations. Clinical research becomes more rewarding as neurosurgeons gain familiarity with the particulars of the structure and relevance of expertly designed clinical cooperative trials. The basic principles and the significance of biometrical essentials (research hypothesis, comparability, randomized and nonrandomized trials, phases of studies and planning steps) are briefly discussed. A guide to the pertinent literature is provided.

    Notes: Summary A retrospective study is presented of 39 patients with ulnar nerve pathology at the elbow. All patients were treated by anterior transposition. Improvement to at least good functional recovery occurred in 46% of the patients. In the other 54% there was only slight or no improvement, or even deterioration. A further analysis of the results showed that in patients with objective signs of ulnar pathology, but without muscular atrophy, the results were good: 64% of these patients showed a good operative result. The most important factors in postoperative prognosis in the patients studied are the existence of muscular atrophy, age, and, in the moderately severe group, the length of history before operation. The aetiology was not important with respect to the outcome. On the basis of the recent literature and the results of this study suggestions are presented for a rational choice between the different possibilities of treatment for ulnar nerve pathology at the elbow.

    Notes: Summary Factors that might have contributed to failure of high cervical percutaneous cordotomy in 23 patients with intractable pain were investigated. Cordotomy failed in 3 patients, 20 had initially good pain relief (87%). True recurrence occurred in 5 patients, 3 of them developed pain elsewhere. Analysis of these three types of failure showed a purely technical cause in 2 cases; other failures or recurrences were due to the underlying disease being not strictly unilateral, or possibly to the existence of other ascending pathways carrying nociceptive impulses apart from the spinothalamic tract. There was no influence of age, sex, type of cancer or previous medication on the result of percutaneous cordotomy. Patients with plexus involvement did better than those with bone metastasis.
