    Notes: We describe a patient with a large nodular basal cell carcinoma (BCC), which was excised following apparent failure to respond to radiotherapy. Histological examination of the fully excised lesion demonstrated a solid mass of amyloid, which stained positively for cytokeratins, but there was no residual BCC.

    Notes: The development of a new integrated materials science facility (station 9.3) on the wiggler line of the Daresbury Synchrotron Radiation Source is outlined. The facility combines instrumentation for data acquisition using both spectroscopic (XANES and EXAFS) and diffraction techniques and is optimized towards industrial applications such as catalysis and electrochemistry using in situ techniques.

    Notes: Two patients had generalised convulsions which occurred several hours afer anaesthesia supplemented with enpurane. One patient had to be treated for life threatening status epilepticus 7 hours after anaesthesia and the other had a myoclonic convulsion 17 hours after recovery from anaesthesia. Neither had any relevant pre-operative history of epilepsy or convulsions. Possible synergistic roles of the metabolite of atracurium. laudanosine, as well as of cephazolin and photic stimulation, are discussed.

    Notes: Alpha cell tumours of the pancreatic islets of Langerhans are rare. The glucagonoma syndrome is caused by excess glucagon secretion from such a tumour. Physiologically, glucagon is important in the control of the homeostasis of glucose and certain amino acids. Pharmacologically, it has been used to treat heart failure. Problems with both glucose homeostasis and myocardial function could, therefore, theoretically be anticipated following resection of a glucagonoma. This paper describes the peri-operative management of such a case, where, despite measured changes in glucagon, no problems of this nature were encountered.

    Notes: Deposition of amyloid β-protein (Aβ) in the brain plays a key role in the pathogenesis of Alzheimer's disease. Head injury is an epidemiological risk factor for Alzheimer's disease, and deposition of Aβ occurs in approximately one third of individuals dying shortly after a severe head injury. Of the three common apolipoprotein E alleles (APOE-ε2, ε3, and ε4) APOE-ε4 allele is a strong risk factor for both sporadic and some familial cases of Alzheimer's disease and there is in vitro evidence that apolipoprotein E is directly involved in Aβ deposition. We have examined the frequency of APOE-ε4 in those individuals with Aβ deposition following head injury and found that the APOE-ε4 frequency (0.52) is higher than in most studies of sporadic Alzheimer's disease. In those head-injured individuals without amyloid deposition the APOE-ε4 frequency (0.16) is similar to that in non-Alzheimer's disease controls (p 〈 0.00001). Our data indicate an interaction between known environmental and genetic risk factors for Alzheimer's disease and underlines the importance of convergence of data around the common mechanism of Aβ deposition. Furthermore, it indicates a genetic susceptibility to the effects of a head injury which may be of significance both to those who have recently sustained such an injury and to those whose activities put them at risk of trauma.

    Notes: Abstract The nature of the cosmic redshift is one of the most fundamental questions in modern science. Hubble's discovery of the apparent Expansion of the Universe is derived from observations on a small number of galaxies at very low redshifts. Today, quasar redshifts have a range more than 1000 times greater than those in Hubble's sample, and represent more than 100 times as many objects. A recent comprehensive compilation of published measurements provides the basis for a study indicating that quasar observations are not in good agreement with the original predictions of the Expanding Universe theory, but are well fit by the predictions of an alternative theory having fewer adjustable parameters.

    Notes: Summary Frozen samples from 92 malignant astrocytomas were stained with a panel of monoclonal antibodies directed against macrophages and lymphocytes. A follow-up to death was available on 68 cases which form the basis of this study. Large numbers of macrophages were found in all cases; T lymphocytes, mostly of the CD8 phenotype were also seen in moderate numbers in 70% of cases. CD4-positive cells were present in small numbers in 32% and B cells were seen in only 8% of cases. Analysis of the survival showed no demonstrable correlation between the numbers of macrophages or CD4 lymphocytes and survival. The survival curves for parencymal CD8 infiltration diverged after 9 months suggesting increased survival for those patients without such an infiltration but the difference failed to reach statistical significance (P=0.37). No correlation between lymphocytic cuffing and survival was seen after studying all paraffinembedded material. We conclude that there is no significant statistical correlation between survival and the various types of mononuclear cell infiltrating malignant astrocytomas.

    Notes: Abstract While neuropathological studies have established the pathology of dementia pugilistica to be similar to that of Alzheimer’s disease, there is little information about the early histological changes caused by the repetitive trauma that eventually produces dementia pugilistica. We have examined the brains of four young men and a frontal lobectomy specimen from a fifth, age range 23– 28 years, all of whom suffered mild chronic head injury. There were two boxers, a footballer, a mentally subnormal man with a long history of head banging, and an epileptic patient who repeatedly hit his head during seizures. The four autopsy cases were widely sampled; the lobectomy specimen was serially sliced after fixation. Routine stains were performed; immunostaining included β-amyloid precursor protein, amyloid β-protein (Aβ), tau and apolipoprotein E (apoE). Pathological findings in all five cases were of neocortical neurofibrillary tangles (NFTs) and neuropil threads, with groups of tangles consistently situated around blood vessels in the worst affected regions. No Aβ immunoreactivity was detected. The amount of neuronal apoE expression varied widely between the cases with no clear relation to the NFTs. The apoE genotype was determined in only two cases (both ɛ3/ɛ3). It appears that repetitive head injury in young adults is initially associated with neocortical NFT formation in the absence of Aβ deposition. The distribution of the tau pathology suggests that the pathogenesis of cytoskeletal abnormalities may involve damage to blood vessels or perivascular elements.
