    Notes: Abstract Commercially pure titanium (Ti cp) has been used successfully as an implant material in fracture fixation devices for many years. Ti cp is comparatively soft, but the mechanical properties, such as strength and ductility, can be adjusted by different means over a wide range. Titanium changes its crystal structure from a hexagonal (alpha) phase to the cubic (beta) phase at about 882 °C. Cubic titanium has the advantage of being very malleable (ductile), but in order to stabilize it at room temperature, additions of suitable alloying elements are required. In this study the soft tissue reaction to implants made from a beta titanium alloy (Ti−Mo−Zr−Al) with four different surface treatments is evaluated. The results are compared to Ti cp implants having the same surface conditions, and to electropolished stainless steel plates as controls. A minimum of four small plates of each group were implanted in rabbit tibiae for 3 months. Histomorphometric results show that the thickness of the soft tissue reaction layer, and the number of blood vessels, connective tissue cells (fibroblasts, fibrocytes), lymphocytes, and foreign body giant cells are not significantly different between beta titanium and Ti cp plates. For stainless steel plates the soft tissue reaction layer is thicker, and the numbers of macrophages and connective tissue cells are higher. Excellent biocompatibility was observed for this beta titanium alloy. The mechanical properties of this alloy surpass those of Ti cp, and because of the good tissue tolerance, this material seems to be advantageous and should enter into clinical testing.

    Description / Table of Contents: Zusammenfassung Die dynamische Kompressionsplatte stellt eine Weiterentwicklung der bewährten AO-Rundlochplatte dar. Die experimentelle Abklärung zeigt, daß rich mit der DCP günstige Kompressionswerte mit geringer Streuung realisieren lassen. Die initial erreichte Kompression nimmt in vivo als Ausdruck erhaltener Stabilität nur langsam ab. Im Tierexperiment konnte selbst unter erschwerten Bedingungen regelmäßig eine primary Knochenheilung beobachtet werden.
    Notes: Summary The dynamic compression plate represents a further development of the conventional ASIF compression plate with round screw holes which has stood the test of time. Experimental investigation of its technical characteristics has proved that adequate compression is achieved with little variation. The compression initially obtained decreases very gradually in vivo, which means that the rigidity of fixation is maintained. In the animal, primary fracture healing was regularly observed even under difficult experimental conditions.

    Description / Table of Contents: Zusammenfassung Die einer Knochenoberfläche exakt angepaßte Kompressionsplatte bewirkt bei der Stabilisierung einer Fraktur asymmetrische Kompression. Um ein plattenfernes Aufklaffen des Bruchspaltes zu verhindern, muß die Platte über dem Bruchspalt überbogen oder aber mit einer schrägen Plattenzugschraube am Knochen angeschraubt werden. Biege- und Torsionsstabilität wurden für verschiedene Techniken der Platten-Osteosynthese in vitro an Querosteotomien der Tibia untersucht. Die Ergebnisse zeigen, daß die schräge Plattenzugsehraube gleiche stabilitätsverbessernde Wirkung hat wie die für die klinische Anwendung sinnvolle Überbiegung der Platte. Die Kombination von schräger Zugschraube und Plattenüberbiegung bewirkt ein Höchstmaß an Biege- und Torsionsstabilität. Dieses Verfahren scheint daher für die Klinik empfehlenswert.
    Notes: Summary A compression plate, exactly contoured to the bone surface, produces asymmetric compression. To avoid opening of the fracture site opposite to the plate, the compression plate is to be used with additional means of pre-bending the plate or an inclined plate lag screw. Flexural and torsional stability provided by a plate lag screw, a pre-bent plate or the combination of both techniques have been studied in in vitro experiments in transverse osteotomies of the human tibia. The tests revealed that the use of an inclined plate lag screw increases the stability comparable to that produced by a pre-bent plate (for pre-bending in the range of a reasonable midspan deflection). The combination of both techniques seems to offer utmost stability.

    Notes: Summary Conventional interlocking nails for the femur use an inclined proximal interlocking screw; the AO/ASIF universal interlocking nail utilizes a transverse interlocking screw which is anchored in cortical bone. The transverse position of the screw allows one to employ the nail for either the right or left side. The yield strength of the thinner diameter transverse screw, (thread 4.5 mm, core 3.0 mm) was compared with that of the Grosse-Kempf nail oblique screw (thread 6.3 mm, core 4.5 mm) by testing in cadaver femora to evaluate the risk of fatigue failure. The test, performed using slow loading rates (50 N/s), simulated either a diaphyseal or a subtrochanteric fracture situation. The test rig applied a load corresponding to body weight and pull at the trochanter. Taking the highest load without plastic deformation as a criterion, the mechanical strength of the two screws was evaluated for each fracture situation. The thinner but shorter transverse interlocking screw demonstrated higher strength450O N vs. 3000 N; P ≤ 0.02) than the thicker inclined screw in the diaphyseal and the subtrochanteric fracture situation (2300 N vs. 1100 N; P ≤ 0.06). The biomechanical test showed the transverse screw to have sufficient and superior strength.
