    Notes: Summary Data concerning width and surface of the basement membrane of cortical capillaries in three patients with Alzheimer's disease five age-matched controls, have been submitted to statistical analysis to ascertain the role of the aging process upon thickening of the basement membrane itself. The analysis showed that there was a significant difference between patients with Alzheimer's disease and controls (P〈0.01) as to both width and surface of the basement membrane, so suggesting that in these patients aging was not the only factor which thickening of the basement membrane of cortical capillaries was related to.

    Notes: Summary The effect of a single oral dose of bromocriptine 2.5 mg was evaluated in 11 normotensive and 6 hypertensive volunteers. 150 min after drug administration, a significant decrease in plasma noradrenaline concentration from 202 to 124 pg/ml in normotensive and from 197 to 119 pg/ml in hypertensive patients was observed. Plasma 3,4 dihydroxyphenylacetic acid, a major metabolite of dopamine, fell from 1132 to 956 pg/ml in normal subjects and from 1242 to 807 pg/ml in hypertensives. No change in plasma adrenaline was found. At the same time, mean arterial pressure showed a significant decrease from 90 to 81 and from 132 to 111 mmHg in normotensive and hypertensive subjects, respectively. Bromocriptine also inhibited the increase in noradrena-line level that occurred when the subjects changed from the supine to the standing position. The inhibition was more evident in hypertensive subjects. It is suggested that the hypotensive effect of bromocriptine is mediated by the inhibition of noradrenaline release due to the stimulation of dopamine receptors on noradrenergic nerve terminals.

    Description / Table of Contents: Zusammenfassung Es werden 13 FÄlle mit Aneurysmen der Bifurkation der A. carotis interna beschrieben. Vorgeschichte, Symptome, angiographische Befunde und chirurgische Behandlung werden dargestellt. 10 Patienten wurden operiert: eine Unterbindung der Karotis am Hals, 6 Klippungen und 3 Ligaturen des Aneurysmahalses. Die chirurgische Behandlung führte in 6 FÄllen zu guten Ergebnissen, in einem Fall zu mÄ\igen Ergebnissen und in 3 FÄllen zu schlechten Ergebnissen. Die verschiedenen chirurgischen Techniken, die bei dieser Art von Aneurysma zur Verfügung stehen, werden beschrieben und an Hand der eigenen Erfahrungen und der Berichte der Literatur besprochen. Es wird die Ansicht vertreten, da\ — auf der Basis gewisser embryogenetischer, strömungsdynamischer und chirurgischer Gegebenheiten — die Aneurysmen der Karotisbifurkation von anderen Aneurysmen der Carotis interna unterschieden und als eigene Gruppe betrachtet werden müssen.
    Abstract: Résumé Les auteurs analysent l'histoire, la symptomatologie, l'aspect angiographique de 13 cas d'anévrisme de la bifurcation de la carotide interne dont 10 furent traités chirurgicalement. Chez un patient la carotide fut ligaturée dans le cou; chez six patients on choisit le clippage du collet de l'anévrysme tandis que chez trois autres le collet de l'anévrysme fut ligaturé à la soie. Les résultats furent bons dans 6 cas, médiocres dans un cas et mauvais dans trois cas. En tenant compte des cas rapportés dans la littérature et de leur propre expérience les auteurs engagent la discussion sur les méthodes de traitement chirurgical de ces anévrysmes. On suggère de considérer les anévrysmes de la bifurcation de la carotide interne comme entité distincte des autres anévrysmes de la carotide interne en raison de leurs particularités embryologiques, staticodynamiques et chirurgicales.
    Notes: Summary Thirteen cases of aneurysm of the bifurcation of the internal carotid artery are described. Clinical history, symptoms, angiographic aspects and surgical treatment are all described. Ten patients had an operation: 1 ligature of the carotid in the neck, 6 clipping and 3 ligature of the aneurysmal neck. Surgery was followed by good results in 6 cases, poor results in 1 case and bad results in 3 cases. The various surgical techniques available for this type of aneurysm are described and discussed on the basis of personal experience and of reports in the literature. The suggestion is made that—on account of certain embryogenetic, statico-dynamic and surgical factors—aneurysms of the carotid bifurcation be regarded separately from other aneurysms of the internal carotid artery.

    Description / Table of Contents: Riassunto L'ulteriore personale esperienza sugli aneurismi della giunzione carotidecorioidea anteriore permette di prospettare la possibilità che la salvaguardia dell'arteria corioidea anteriore, benchè importante, non costituisca un problema di assoluta necessità. Nella serie degli Autori, infatti, solo un caso sui quattro con occlusione chirurgica dell'arteria corioidea ha presentato transitori disturbi post-operatori attribuibili all'occlusione stessa; d'altra parte, analoghi transitori disturbi — attribuibili ad uno spasmo dell'arteria — sono comparsi anche in un caso dei sei con la corioidea intatta. Gli Autori, quindi, pur riconoscendo l'opportunità di evitare l'occlusione chirurgica della corioidea anteriore, sono del parere che ove ciò non sia possibile, i risultati possono essere egualmente buoni.
    Notes: Summary Further experience with carotid-choroidal aneurysms leads the authors to believe that whilst it is desirable to safeguard the anterior choroidal artery it is not always essential. In this series, only one out of four patients with a surgical occlusion of the choroidal artery had transient post-operative troubles. Of six patients with an intact choroidal artery, one showed transitory post-operative troubles, which were ascribed to spasm of the vessel. It is concluded that although care should be taken to avoid choroidal artery occlusion, when it must be occluded in order to treat the associated aneurysm, a neurological deficit does not inevitably occur.

    Notes: Summary In a series of 88 cases of chronic subdural haematoma, the occurrence of dilatation of the posterior portion of the contralateral ventricle was found to be unrelated to any major neurological variable. There was, however, a prevalence of recurrences of the subdural effusions in the patients with partial hydrocephalus, compared with patients without ventricular dilatation.

    Notes: Summary Eleven consecutive cases of acute traumatic vertex epidural haematomas (VEDHs) among 416 epidural haematomas (EDHs) operated on during the same period in the Neurosurgical Clinic of the University of Genoa, are reported. Clinical features, neuroradiological aspects (X-ray, angiography and CT scan) and results are discussed, in order to point out the problems encountered in the diagnosis, expecially due to the mystifying clinical picture and the unsuitability of the axial CT scan in the detection of these lesions. In this series mortality rate was 18%, against 50% reported in the literature.

    Description / Table of Contents: Zusammenfassung Sechs Aneurysmen der Ausgangsstelle der A. chorioidea anterior von der A. carotis interna wurden durch direkten Eingriff operiert. Das Aneurysma wurde in 5 Fällen mit Seide oder Clip ausgeschaltet, in einem Fall mit dem „wrapping“ behandelt. 5 Patienten wurden komplett geheilt, 1 Patient starb (neue Blutung nach dem „wrapping“). Das klinische Bild und die Ergebnisse des chirurgischen Eingriffes werden anhand der Literaturberichte besprochen. Das wichtigste technische Problem ist die Schonung der A. chorioidea.
    Abstract: Summary Six aneurysms of the junction a. carotis interna—a. chorioidea anterior have been operated on by direct approach. 5 cases were treated with silk ligature or clipping; 1 case with wrapping. 5 patients recovered completely and 1 died (rebleeding after wrapping). The clinical aspects as well as the results of surgery are discussed in relation to the data in the literature. The most important technical problem concerns the necessity to safeguard the integrity of the chorioidal artery.
    Notes: Résumé Six anévrysmes de la jonction a. carotide interne — a. choroïdienne antérieure ont été opérés par attaque direct. En 5 cas les anévrysmes ont été exclus par soie ou clip; en 1 cas on a réalisé le « wrapping ». On a eu 5 guérisons complètes et 1 décès (nouveau saignement après le «wrapping»). Les aspects cliniques et les résultats opératoires sont discutés en rélation aux données de la litérature. Le problème technique principal est représenté par la nécessité de sauver l'a. choroïdienne.

    Notes: Summary The case of a 35 years old woman affected by endometriosis located inside the spinal canal in the extradural space at the level of the third left lumbar root, and developing through the corresponding foramen into the paraspinal muscles, is presented. The clinical aspect, radiological picture and surgical treatment are described. Pathogenesis is discussed on the basis of the literature. Furthermore it is stressed that only the histopathological examination gave the correct diagnosis and permitted the definitive hormonal treatment. To our best knowledge no comparable case has been published in the literature.

    Notes: Summary Fifteen consecutive cases of subdural haematomas (SDHs) of arteriolar origin operated on during the last twenty years in the Neurosurgical Clinic of the University of Genoa are reported. These SDHs are rare and form as a result of a rupture of a cortical arteriolar branch typically on the vicinity of the Sylvian fissure. They are always in a pure form and never associated with parenchymal lesions. Their aetiology is spontaneous, as in the 40% of our series, or more frequently related to minor traumas. These SDHs are often encountered in elderly patients generally presenting with severe clinical conditions (in the 60% of our patients GCS on admission was 〈8). Prompt surgical treatment is required in all cases before further worsening of their neurological condition, even if CT scanning does not reveal parenchymal lesions. The mortality rate is over 50% and mainly relates to the pre-operative GC score. Clinical features, neuroradiological aspects and treatment are discussed and literature reviewed.
