    Description / Table of Contents: Summary Functional brain imaging with positron emission tomography (PET) and single photon emission computerized tomography (SPECT) enables the in vivo study of specific neurochemical processes in the context of normal regulatory mechanisms and pathophysiological alterations of the brain. By combining these methods with pharmacological challenge-paradigms, the study of functional interactions of different neurotransmitter systems is possible. This review will present data from animal and healthy volunteer studies as well as first data from investigations in different patient populations with regard to this research direction. Especially, interactions of different neurotransmitter systems with the dopaminergic and the cholinergic system will be discussed. The database acquired so far confirms existing models of neuronal feedback -circuits, and the first clinical results are consistent with the hypothesis of an increased dopaminergic responsivity in schizophrenic patients. These results open up new perspectives for a further evaluation of treatment response predictors from drug -challenge studies and for the development of new drug treatments for neuropsychiatric disorders.
    Notes: Zusammenfassung Positronenemissionstomographie (PET) und Single Photonenemissionscomputertomographie (SPECT) ermöglichen als funktionelle bildgebende Verfahren die In-vivo-Untersuchung von spezifischen neurochemischen Prozessen im Zusammenhang mit physiologischen Regulationsvorgängen und pathophysiologischen Veränderungen des Gehirns. Durch die Kombination mit pharmakologischen Challenge-Paradigmen kann auf dieser methodischen Grundlage auch den funktionellen Interaktionen zwischen Neurotransmittersystemen nachgegangen werden. In dem vorliegenden Übersichtsartikel werden unter dieser Fragestellung durchgeführte tierexperimentelle Studien, Untersuchungen bei gesunden Probanden sowie klinische Ergebnisse dargestellt. Insbesondere der vielfältige Einfluss auf das dopaminerge und das cholinerge System wird dabei näher betrachtet. Die gewonnenen Daten bestätigen existierende Modelle von neuronalen Feedback-Regelkreisen. Erste klinische Resultate sind konsistent mit Hypothesen einer verstärkten dopaminergen Response bei schizophrenen Patienten. Die bisherigen Ergebnisse eröffnen eine Perspektive für die Entwicklung neuer Strategien zur Therapie neuropsychiatrischer Erkrankungen und bereiten zugleich den Boden für weitere Untersuchungen zur prädiktiven Bedeutung einer Challenge-Antwort bezüglich eines nachfolgenden Therapieerfolgs.

    Notes: Abstract Single photon emission tomography (SPECT) imaging of the central benzodiazepine receptor (BZr) became possible with the newly developed ligand123I-IOMAZENIL. The BZr binding was investigated in ten patients with panic disorder (PP) compared to ten epileptic patients (EP). Panic patients had lower IOMAZENIL uptake rates in the frontal, occipital and temporal cortex than EP, indicating the involvement of the BZr complex in panic disorder.

    Notes: Abstract. Objective: Comparison of three different methods of fetal lung maturity (FLM) testing in amniotic fluid (AF) to predict fetal respiratory distress syndrome (RDS): phospholipid concentration, phosphatidylglycerol (PG) and lamellar body count (LBC). Study design: 315 amniotic fluid (AF) samples were tested, 291 by determination of phospholipid, 237 by PG and 68 by LBC. Eighty one patients (26%) delivered within 3 days, incidence of RDS was 7% (n=6). Correlation of phospholipids and LBC to gestational age was analyzed as well as correlation of LBC and PG versus phospholipid concentration. Sensitivity and specificity for all three methods were calculated. Results: Phospholipids and LBC increased exponentially with gestational age (r regr=0.54, p〈0.01, r regr=0.76, p〈0.01) showing a curvilinear function. There was a linear correlation between both methods (r corr=0.51, p〈0.01, n=47). Sensitivity for prediction of RDS was 80% for phospholipids compared to 83% for LBC and 67% for PG. Specificity was best for PG (92%) followed by phospholipids (85%). While positive predictive value was low (27% for phospholipids and 50% for LBC), negative predictive value varied between 97% and 100%. Conclusion: LBC is a simple and reliable method for FLM testing, with a sensitivity comparable to phospholipid testing and a somewhat lower specificity (67% vs. 85%). It may be useful for screening of FLM in a cascade approach.

    Notes: Abstract Paroxetine is a selective and potent serotonin reuptake inhibitor and its efficacy for the treatment of depression has been proven. Under acute and subchronical treatment regimens, disturbances of the regular sleep pattern are a reported side effect of the drug. The present study was therefore performed to investigate the impact of subchronic treatment with the selective serotonin reuptake inhibitor paroxetine on the microstructure of the sleep EEG. The study especially addressed the question of subchronic effects of paroxetine medication (30 mg/day) in eight healthy male volunteers in a double blind, placebo-controlled crossover design. Conventional sleep EEG parameters and a spectral power analysis for different sleep stages after 4 weeks of treatment were computed. Additionally, the correlation of certain EEG rhythms across the night was calculated in order to detect subtle dynamical EEG alterations, not necessarily obvious when regarding conventional EEG analysis. Although we could not detect any alterations of the spectral power values in certain frequency bands either during NREM nor during REM sleep following subchronic paroxetine medication, the dynamical EEG attributes across the night revealed a significant enhancement of the correlation between certain EEG rhythms mainly during NREM sleep.

    Notes: Abstract By the application of123([123I]IBZM), an iodine-labelled dopamine D2-receptor antagonist, brain D2 receptors in humans can be visualized with single photon emission computed tomography (SPECT). The ratio of IBZM binding to striatal regions versus binding to frontal cortex (ST/FC ratio) provided a semiquantitative measurement of D2 receptor binding in the striatum. This study investigated the relationship between receptor occupancy and plasma prolactin levels in 12 male patients treated with haloperidol, benperidol or clozapine. Prolactin levels were positively correlated with D2 receptor occupancy, reflecting at least in part a comparable dopamine receptor antagonism in different dopaminergic pathways.

    Notes: Summary D2-dopamine receptors can be visualized in the human brain in vivo by Single Photon Emission Tomography (SPECT) and the radiolabeled benzamide [123I]IBZM. The present paper reviews the current status of this type of functional brain imaging with respect to basic methodological aspects, data analysis and quantification. The results from published clinical studies in different psychiatric patient populations and normal controls with [123I]IBZM are reviewed. [123I]IBZM-SPECT is a powerful tool for the investigation of D2-dopamine receptor status in psychiatric disorders, different types of drug treatment as well as therapeutic and side effects of pharmacologic agents. However, there still is a need for standardized imaging times and image-processing procedures. Advantages and disadvantages of SPECT with special regard to Positron Emission Tomography (PET) are also discussed.

    Notes: The karyotypes of four species of Dyscophinae and eight species of Cophylinae were analyzed. The chromosome number was 2n=26 in all cases. Between the two subfamilies a difference in the form of the karyotype was observed; the chromosomes show a gradual decrease in length in the Dyscophinae, whereas in the Cophylinae the karyotype demonstrates a clear discontinuity of size between pairs 5 and 6. Chromosomal polymorphism was found in Plethodontohyla tuberata, the chromosomes of pair 4 were subtelocentric in the homozygous specimens, whereas this pair showed a subtelocentric and a submetacentric chromosome of equal length in the heterozygous one, suggesting a pericentric inversion. Although in the Cophylinae the chromosome number is constant, the number of chromosome arms is variable. Pericentric inversions seem to play an important role in the chromosomal evolution of the Cophylinae.

    Description / Table of Contents: Zusammenfassung Von Januar bis Dezember 1988 wurden 383 Früh- und Neugeborene auf unsere neonatologische Intensivstation aufgenommen. Von diesen Kindern wurden 1991 Abstriche und Blutkulturen bakteriologisch untersucht. Bei 41 Patienten konnte in 90 FällenAcinetobacter calcoaceticus nachgewiesen werden. Während dieser Zeit entdeckten und behandelten wir drei Fälle einer Sepsis durchA. calcoaceticus, in drei weiteren Fällen zeigten sich positive Blutkulturen ohne klinische Zeichen einer Infektion. Als Quelle der Infektionen konnten Warmluftvernebler entdeckt werden. Eine Untersuchung sämtlicher bakteriologischer Ergebnisse Monate vor Ausbruch der Infektion zeigte einen Anstieg der Häufigkeit vonA. calcoaceticus. Die betroffenen Kinder erhielten eine spezifische antibiotische und intensivmedizinische Therapie. Alle Kinder überlebten. Bedingungen, die ein Wachstum dieses primär apathogenen Keimes begünstigen, sowie präventive Maßnahmen werden diskutiert. Die Notwendigkeit kontinuierlicher bakteriologischer Untersuchungen und die sorgfältige Desinfektion von Ausrüstungsgegenständen einer Intensivstation werden betont.
    Notes: Summary Between January and December 1988, 383 neonates were admitted to our neonatal intensive care unit. 1,991 swabs and blood cultures were tested bacteriologically. Among them 90 specimens obtained from 41 patients were positive forAcinetobacter calcoaceticus. During this period we discovered and treated three cases withA. calcoaceticus sepsis. Three additional cases had blood cultures positive for this bacterium without demonstrating any clinical signs of infection. There is good evidence that contaminated warm air humidifiers were the source of infection. A review of microbiological data for several months preceding the outbreak showed a definite increase in the presence ofA. calcoaceticus. The affected neonates required specific antibiotic therapy and intensive care. All of them survived. Conditions favoring the spread of these generally non-pathogenic bacteria and modes of preventive measures are discussed. The necessity of continuous bacteriological surveillance and careful disinfection of intensive care equipment is emphasized.
