    Description / Table of Contents: Summary Electron microscopic studies of particles of the human fundic mucosa aspirated by suction biopsy suggest a functional cycle, especially in the cymogenic cells. When gastric secretion has been stimulated by pentagastrin an accumulation of lysosomes, i.e. granules, and their expulsion into the lumen of the gastric glands has been observed, as well as a disappearance of the endoplasmatic reticulum. The secretory phase has been followed by a phase of regeneration of the endoplasmatic reticular structures. 5 1/2 hours after a secretory stimulus the ultrastructure of the fundic mucosa has been identical with the state of secretory rest observed at the beginning of the experiment. The establishment of phasic functional changes in the ultrastructure of the gastric mucosa with a definite time course may be important for further studies of gastritis and of the mode of action of anti-inflammatory drugs.
    Notes: Zusammenfassung Elektronenmikroskopische Untersuchungen an bioptisch gewonnenen Partikeln der Korpusschleimhaut lassen einen Funktionscyclus vor allem an den Hauptzellen erkennen. Nach Stimulation der Magensekretion mit Pentagastrin kommt es zur Häufung von Lysosomen und ihrer Ausstoßung in das Lumen sowie zu einem Verlust des endoplasmatischen Reticulums. Dieser Sekretionsphase folgt eine Regenerationsphase mit Wiederaufbau des endoplasmatischen Reticulums. Nach 5 1/2 Std ist der Zustand der sekretorischen Ruhephase wieder erreicht. Die ultrastrukturellen Veränderungen der Magenschleimhaut in der Sekretionsphase ähneln dem nach Betazol-Injektion beschriebenen Bild, sind jedoch schwächer ausgeprägt. Die Festlegung des zeitlichen Ablaufs der Ultrastrukturveränderungen ist für weitere Untersuchungen über die Wirkung entzündungshemmender Pharmaka und auch für Funktionsstudien bei der Gastritis bedeutsam.

    Description / Table of Contents: Summary A 100% increase of the urinary excretion of vanilmandelic acid (3-methoxy-4-hydroxy-mandelic acid) was observed in motorcar drivers and accompanying persons. This result was throught to correspond to an elevated endogenous turnover of catechol amines due to stressful traffic situations.

    Notes: Zusammenfassung Beim Menschen wurde der Einfluß von hypertonischer Glucoselösung, verdünnter Salzsäure und destilliertem Wasser bei Zufuhr in den Dünndarm auf das Verhalten der Oxyindolessigsäure-Ausscheidung untersucht. Unsere Ergebnisse sprechen gegen eine Steigerung des Serotoninumsatzes und eine vermehrte Freisetzung des Amins in die Blutbahn unter diesen Versuchsbedingungen. Beziehungen zwischen Veränderungen des Serotonin-Stoffwechsels und dem Dumping-Syndrom bestehen nicht.

    Notes: Abstract Objective: To evaluate the accuracy and precision of continuous thermodilution (CCO) by using a validated bolus thermodilution (BCO) reference technique as criterion standard. Design: Under circulatory steady state conditions, a CCO system (Vigilance, software versions 4.35 and 4.39) was validated with regard to CCO as well as iced and room temperature BCO. Setting: Intensive care unit at a university hospital. Patients: Method comparison was conducted in 56 cardiac surgical patients, 28 patients being allocated to one of the two software versions, and 14 within each group to either iced or room temperature BCO. Measurements and results: CCO readings were registered in duplicate before and after three to five bolus injections conducted with both the Vigilance and reference systems. Iced BCO showed excellent agreement between the Vigilance and reference systems, yielding SDs of bias of 0.41 and 0.37 l/min and linear correlation coefficients (r) of 0.97 and 0.96. With room temperature BCO, agreement was significantly less. CCO, irrespective of software version, showed higher SDs of bias (0.90 and 0.84 l/min) and lower r values (0.84 and 0.81) than iced BCO (p 〈 0.0001). CCO measurements with software version 4.39 yielded a similar SD of bias to that with room temperature BCO. Conclusion: Decreased precision of CCO as compared to iced BCO may, in clinical settings, be outweighed by the advantages of automated and continuous monitoring. Under research conditions, however, iced BCO remains the method of choice.

    Notes: Summary 1. A spectrophotofluorometric procedure is described by which the potassium-retaining diuretic amiloride is isolated from the urine by means of thin-layer chromatography. - 2. After administering single doses of 10 mg, the drug can be demonstrated in the urine up to 24 h, and after giving 20 mg, up to 48 h. The percentage recovered is independent of the dosage applied and is not influenced by diuresis, or by the concomitant administration of the saluretic, etacrynic acid. In patients with cirrhosis of the liver and portal hypertension the recovery of amiloride is identical to that of healthy controls. After fasting for four hours following the ingestion of the drug, the quantity of the diuretic excreted in the urine is approximately twice that after administration of amiloride at breakfast or shortly before.
